Percutaneous ultrasound-guided sclerotherapy with polidocanol microfoam for lymphatic malformations.

BACKGROUND: Lymphatic malformations (LMs) are low-flow congenital lesions that consist of cysts of varying size. Sclerotherapy with intralesional bleomycin and OK-432 has been reported to yield dramatically beneficial results for this disorder. However, inflammation-related symptoms are often seen after treatment with these sclerosing agents. On the other hand, polidocanol (POL) is reportedly associated with fewer allergic and inflammatory reactions. Up to now, however, very few reports have documented the use of POL microfoam for treatment of LMs. This study was performed to assess the effica

[Enigmatic lymphatic diseases involving the lung].

Lymphedema associated with other developmental malformations (Milroy syndrome, Hennekam syndrome, Noonan syndrome, Gorham-Stout syndrome, yellow nail syndrome) are unfrequent disease, but explorations led to the identification of genetic mutations that have then been validated in mouse models. However, lymphatic vessels complexity and its proximity with the venous system suggest the need for further researches, especially in the comprehension of pulmonary symptoms.

Congenital vascular malformations: a series of five prenatally diagnosed cases.

In the literature there are single case reports of mediastinal/chest and limb combined vascular malformations (previously labeled "hemangiolymphangiomas"). A variable outcome in such prenatally diagnosed cases is reported. Presented here is the only series of patients reported with these macrocystic, predominantly lymphatic malformations. Prenatal ultrasound scan and post-mortem examination findings are described. In our experience the outcome has been poor and this highlights the dilemma faced by clinicians and parents when these lesions are diagnosed prenatally. We present a series of five,

Combined capillary-venous-lymphatic malformations without overgrowth in patients with Klippel-Trénaunay syndrome.

OBJECTIVE: Klippel-Trénaunay syndrome (KTS) is described in the literature as a complex syndrome characterized by various combinations of capillary, venous, and lymphatic malformations associated with limb overgrowth. In the first description by Maurice Klippel and Paul Trénaunay, tridimensional bone hypertrophy was believed to be the cause of limb enlargement. The purpose of this study was primarily to assess the presence of real bone hypertrophy as a cause of enlargement of the limb and to underline the rare presence of undergrowth of the affected limb in patients with KTS. METHODS: A two-c

[Lymphangioleiomyomatosis: Update on Pathophysiology, Diagnosis, and Treatment].

Interstitial lung diseases (ILDs) include a group of rare and complex pathologies, among which lymphangioleiomyomatosis (LAM) stands out. LAM is considered a neoplastic disease characterized by abnormal proliferation of smooth muscle cells in the lung, forming cysts and causing severe respiratory alterations. LAM primarily affects women of childbearing age and can present in two forms: sporadic (S-LAM) and associated with tuberous sclerosis complex (TSC-LAM). Although its etiopathogenesis is not fully understood, genetic and hormonal pathophysiological mechanisms have been studied. Genetic alt

Lymphedema is a rare manifestation of lymphoma: A case series and literature review.

Lymphedema, which is characterized by impaired lymphatic drainage leading to tissue swelling, represents a relatively uncommon clinical entity, with an estimated prevalence of <1% being observed in the general population. Although most cases arise from postsurgical complications or filariasis, lymphoma-associated lymphedema remains an exceedingly rare manifestation, with only 19 cases documented in the medical literature prior to the present study. The current study presented a case series of 11 patients with histologically confirmed lymphoma manifesting with lymphedema, which represents the l

A model for personalized diagnostics for non-specific low back pain: the role of the myofascial unit.

Low back pain (LBP) is the leading cause of disability worldwide. Most LBP is non-specific or idiopathic, which is defined as symptoms of unknown origin without a clear specific cause or pathology. Current guidelines for clinical evaluation are based on ruling out underlying serious medical conditions, but not on addressing underlying potential contributors to pain. Although efforts have been made to identify subgroups within this population based on response to treatment, a comprehensive framework to guide assessment is still lacking. In this paper, we propose a model for a personalized mecha

[Edematous syndromes caused by capillary hyperpermeability. Diffuse angioedema].

Edema due to increased capillary permeability (ICP) may be diffuse or localized. Local edemas (Quincke edema, angioneurotic edema) are most often allergic or very rarely due to a defect in C1-inhibitor. Generalized edemas due to ICP share the following clinical features: Fluid retention (subcutaneous edema and diffused swelling) is predominant in lower limbs; it is worsened by orthostatism and warmth and alleviated by decubitus and cold, with important weight variations between morning and evening. It is associated with enhanced thirst, hypotension, oliguria, headaches and blood volume reducti

Operative Management of Neonatal Lymphatic Malformations: Lesson Learned From 57 Consecutive Cases.

Aim of the study: Lymphatic malformations (LMs) are rare entities, sometimes difficult to treat, that may be life-threatening when intricately connected to airway structures. Invasive treatments are occasionally required, with sclerotherapy considered the treatment of choice and surgery as a second-line approach. The aim of the present study was to evaluate our multidisciplinary team experience in treating newborns affected by LMs requiring operative management, while defining early outcomes. Methods: Retrospective review of all consecutive patients admitted for LMs requiring operative managem

Endoscope-Assisted Thoracic Duct Ligation and Cisterna Chyli Ablation Using Double O-Ring Wound Retractors in a Dog with Presumed Idiopathic Chylothorax.

Chylothorax is a challenging condition in dogs, and although endoscopic thoracic duct ligation (TDL) combined with cisterna chyli ablation (CCA) is an established surgical treatment, adequate operative exposure and visualization remain technically demanding. This report describes the use of double O-ring wound retractors (OWRs) with endoscopic assistance to perform TDL and CCA in a dog with presumed idiopathic chylothorax. A 5-year-old castrated male French Bulldog presented with dyspnea caused by chylous pleural effusion. Computed tomographic lymphangiography identified abnormal thoracic duct

Indocyanine Green Lymphography Findings in Primary Leg Lymphedema.

OBJECTIVES: Indocyanine green (ICG) lymphography has been reported to be useful for the evaluation of secondary lymphedema, but no study has reported characteristic findings of ICG lymphography in primary lymphedema. This study aimed to classify characteristic ICG lymphography patterns in primary lymphedema. METHODS: The study was a retrospective observational study. Thirty one primary lower extremity lymphedema (LEL) patients with a total of 62 legs were studied. ICG lymphography patterns were categorized according to the visibility of lymphatics and dermal backflow (DB) extension. Clinical

The value of ultrasound in detecting extra-axillary regional node involvement in patients with advanced breast cancer.

Assessment of the regional lymphatics is important for accurate staging and treatment of breast cancer patients. We sought to determine the role of regional ultrasound in providing clinically relevant information. We retrospectively analyzed data from patients who were treated curatively in 1996-2006 at The University of Texas MD Anderson Cancer Center for clinical stage III breast cancer. We compared differences in regional lymph node staging based on ultrasound versus mammography and physical examination in the 865 of 1,200 patients who had external-beam radiation as part of their treatment

Evaluation and management of the fat leg syndrome.

LEARNING OBJECTIVES: After studying this article, the participant should be able to: 1. Discuss the initial evaluation of a patient presenting with lower extremity enlargement. 2. Distinguish underlying medical conditions causing lower extremity enlargement, including lymphedema and lipedema. 3. Discuss appropriate management and treatment for patients presenting with these conditions. BACKGROUND: Given the epidemic of obesity in the United States, many patients will consult the plastic surgeon with complaints of lower extremity enlargement secondary to "fat legs." In addition to cosmetic dis
