Streptococcus pneumoniae isolates (n=1191) were collected during a 1997-1999 European surveillance study. In addition to susceptibility data, a molecular epidemiological survey of their mechanisms of resistance to macrolides, tetracyclines, and quinolones was provided. Of the isolates tested, 72.6% were penicillin-susceptible, 19.9% penicillin-intermediate and 7.5% penicillin-resistant. There was an obvious relationship between resistance to penicillin and resistance to erythromycin (19% of all isolates), clindamycin (14%) and tetracycline (23%). Only one isolate was resistant to levofloxacin.…

From a series of macrocyclic diamides possessing the disulfide linkage, only SRR-SB3, a compound that complexes with zinc, was found to inhibit human immunodeficiency virus type 1 (HIV-1; strain IIIB) replication at a concentration of 1.8 to 6.5 micrograms/ml in MT-4, CEM, and peripheral blood mononuclear cells. SRR-SB3 was toxic to MT-4 cells at a concentration of 15.9 micrograms/ml, resulting in a selectivity index of 9 in these cells. This macrolide was also effective against various other HIV-1 strains, including clinical isolates and HIV-1 strains resistant to protease inhibitors and nucl

The widespread presence of antibiotics in the environment at sub-inhibitory concentrations imposes a selective pressure that promotes the spread of resistance. In the field, antibiotics interact with diverse physicochemical parameters that can attenuate or intensify their fitness effects. Gene expression is a central plastic trait that governs phenotypes at a higher level of integration and modulates the strength of selection, yet how synergistic or antagonistic fitness effects arise from interactions among transcriptional responses remains poorly understood. Here, we characterized gene-expres

Suspected macrolide-refractory Mycoplasma pneumoniae pneumonia can be a diagnostic challenge in immunocompromised hosts, particularly after allogeneic stem cell transplantation. Screening serologies may further complicate diagnosis when false-positive or discordant results divert therapy. We report a 63-year-old woman with B-cell acute lymphoblastic leukemia status postallogeneic stem cell transplantation and hypogammaglobulinemia who was transferred for persistent cough, exertional dyspnea, and right upper-lobe consolidation after limited improvement with azithromycin and broad-spectrum antib

BACKGROUND: Mycoplasma pneumoniae is a major cause of paediatric community-acquired pneumonia. The prevalence of macrolide-resistant M. pneumoniae (MRMP) has exceeded 80% in East Asia, posing significant challenges to clinical management. Increasing evidence suggests that disease progression is driven not only by direct microbial injury but also by host-mediated hyperinflammatory responses.
METHODS: This structured narrative review was conducted using PubMed and Embase databases to identify relevant studies published between 2000 and 2025. Clinical studies, biomarker analyses and regional gui

OBJECTIVES: Treatment for patients with macrolide-resistant Mycobacterium avium complex (MR-MAC) pulmonary disease is a major clinical challenge, as pharmacologic options are limited and outcomes with antibiotics alone are unsatisfactory. Although surgical intervention has been considered in selected cases, clinical evidence specific to MR-MAC is limited. This study aimed to compare the clinical outcomes of surgical intervention for MR-MAC pulmonary disease to those of non-resistant cases.
METHODS: This multicentre study included 248 patients who underwent pulmonary resection for MAC pulmonar

Macrolide-resistant Mycoplasma pneumoniae (MP) infections are reported worldwide. Smart Gene® (SG) Myco, a point-of-care testing tool, detects nucleic acids and macrolide-resistance gene mutations of MP. The objective of this study was to evaluate the clinical impact of the rapid detection of macrolide resistance gene mutations in children with MP. This retrospective study included children ≤ 18 years of age with microbiologically confirmed MP requiring antimicrobial treatment. Clinical outcomes were compared between the SG Myco test and untested groups. A total of 298 participants (121 in the

Streptococcus pneumoniae, the pneumococcus, is a cause of major illness globally. Invasive pneumococcal disease (IPD) is characterized by pneumococci invading blood (bacteremia), lungs (pneumonia), or brain and cerebrospinal fluid (meningitis). Meningitis remains an important global health concern because half of the survivors experience long-term neurological damage. The antibiotics commonly used to treat pneumococcal infections are β-lactams and macrolides, however, S. pneumoniae is nowadays often resistant to one or several antibiotics, therefore novel antimicrobials are needed. Here, we fo…

Inducible macrolide resistance mediated by the erm(41) T28 allele is a well-established intrinsic mechanism in Mycobacterium abscessus. Here, we report a case of M. abscessus pulmonary disease in which an initial erm(41) C28 isolate developed inducible resistance through a de novo C28T point mutation during prolonged therapy. To our knowledge, this case provides the first clinical evidence that erm(41)-mediated inducible resistance can arise in vivo under antimicrobial selective pressure.

The widespread application of macrolide antibiotics has caused antibiotic resistance pollution, threatening the river ecological health. In this study, five macrolide antibiotics (azithromycin, clarithromycin, roxithromycin, erythromycin, and anhydro erythromycin A) were monitored in the Zao River across three hydrological periods (April, July, and December). Simultaneously, the changes in antibiotic resistance genes (ARGs), mobile genetic elements (MGEs), and planktonic bacterial communities were determined using metagenomic sequencing. A clear pollution gradient was observed for azithromycin

The emergence of macrolide-resistant Bordetella pertussis (MRBP) is a significant problem because it reduces treatment options for pertussis and exacerbates the severity and spread of the disease. MRBP has been widely prevalent in mainland China since the 2010s and has been sporadically detected in other Asian countries. In Japan, two MRBP clinical strains were first isolated in Tokyo and Osaka between June and July 2018. The isolates BP616 in Osaka and BP625 in Tokyo harbored the same virulence-associated allelic genes (including ptxP1, ptxA1, prn1, fim3A, and fhaB3) and MT195 genotype and ex

Macrolides are empirically used to treat bacterial community-acquired pneumonia (CAP). Streptococcus pneumoniae, being the major pathogen responsible for bacterial CAP with high mortality rates, express MefA-MsrD efflux pumps to hinder macrolide susceptibility. Despite its importance, the structural features of the efflux-protein complex and its impact on macrolide susceptibility have not yet been elucidated explicitly. Therefore, in the present study, combining homology, threading, and dynamics approaches, MefA and MsrD proteins in pathogenic S. pneumoniae were modeled. Both membrane (lipid-b…

Neisseria commensals are an indisputable source of resistance for their pathogenic relatives. However, the evolutionary paths commensal species take to reduced susceptibility in this genus have been relatively underexplored. Here, we leverage in vitro selection as a powerful screen to identify the genetic adaptations that produce azithromycin resistance (≥ 2 μg/mL) in the Neisseria commensal, N. elongata. Across multiple lineages (n = 7/16), we find mutations that reduce susceptibility to azithromycin converge on the locus encoding the 50S ribosomal L34 protein (rpmH) and the intergenic region
