PURPOSE: Traditionally, patients with hemifacial microsomia (HFM) and significant ipsilateral hypoplasia or absence of the condyle and ramus undergo reconstruction on the ipsilateral side with autogenous tissues such as rib grafts, often with compromised results. This study analyzed the surgical treatment outcomes of nongrowing patients with HFM and reconstruction of the ipsilateral temporomandibular joint (TMJ) and advancement of the mandible with a patient-fitted TMJ total joint prosthesis (TMJ Concepts, Inc, Ventura, CA), a contralateral mandibular ramus sagittal split osteotomy, and maxill

PURPOSE: Vastly different surgical techniques have been advocated for osseous reconstruction of the severely atrophic mandible. Endosseous implants placed in autologous bone grafts have been proposed to minimize graft resorption and restore function; however, sufficient bone must exist to support the implants and prevent pathologic fracture. The purpose of this retrospective analysis was to assess the efficacy of autologous bone grafting and the subsequent placement of endosteal implants as a staged procedure in patients with severely atrophic mandibles.
MATERIALS AND METHODS: The records of

Gorham disease, a rare condition of unknown etiology, is characterized histologically by bone disintegration and endothelial proliferation. We describe serial imaging findings, the treatment course, and radiotherapy response in a patient with pathologically confirmed Gorham disease involving the right mandible. Progressive mandibular resorption was managed initially with multiple surgical resections and reconstruction, followed by external-beam radiotherapy, which was an effective treatment method in this case. The patient's reconstructed mandible is functional and he is asymptomatic. Our lite

Microvascular flap reconstructions of facial and mandibular defects are achieving increasingly widespread application and, as a result, more patients are considered for such procedures after large tumors, congenital abnormalities, previous surgery (including failed and successful microsurgical procedures) and radiation have compromised potential recipient vessels on the side of the defect. In three such cases, contralateral facial vessels with vein grafts have been used to provide flap-recipient vessels. Each flap was successful. Cross-facial vein grafting is a relatively simple solution for u

BACKGROUND: The purpose of this study was to compare complications and dental implant success between the bone-impacted fibula free flap (BIFFF) and the traditional fibular free flap used in mandibular and midface reconstruction.
METHODS: Retrospective review of all patients undergoing BIFFF or traditional fibular free flap reconstruction from 2001 to 2009 was undertaken. Complications related to the BIFFF and traditional fibular free flap site of reconstruction were compared. Dental implant success rates for each type of flap were compared at 1-year intervals for 5 years.
RESULTS: One hundr

Distraction osteogenesis or callostasis is a technique of new bone formation by gradual separation of bony fragments. The method was first developed for limb lengthening but recently this process was widely applied to the cranio-maxillofacial bones. The application included unlimited bone lengthening and reconstruction of segmental defects. Several designs of extra-oral and intra-oral distraction devices were invented to suit different areas of craniofacial bone. Nevertheless intraoral distractors have several advantages including minimal scarring and being less cumbersome. Clinical cases usin

We report on the pectoralis major composite (musculocutaneous or rib osteomusculocutaneous) flap used in bipaddled fashion for one-stage immediate reconstruction in 53 patients with oral cancer. Of these, 44 patients had lesions of buccal mucosa and lateral lower alveolus, and 9 patients had lesions of floor of mouth and middle third of lower alveolus. In one patient this flap was used as a free composite tissue transfer in bipaddled fashion. The flap can be used either before or after radiotherapy in oral cancer and provides both lining mucosa and skin cover. If necessary, a rib (vascularised

A 67-year-old male patient with persistent adenoid cystic carcinoma in the floor of the mouth underwent a glosso-mandibulectomy and left cervical lymph node dissection. After the defect was identified, the reconstructive team chose a modified osteocutaneous clavicular technique using a local internal mammary artery perforator for reconstruction due to the patient's preference to avoid lower extremity involvement. The flap was harvested and positioned over the defect, with the bone fixed using a 2.4 mm system reconstruction plate. In the postoperative period, the flap exhibited mild venous cong

Gap arthroplasty and costochondral rib bone graft are commonly performed by oral and maxillofacial surgeons to reconstruct the temporomandibular joint with ankylosis. However, unpredictable and unsatisfactory results such as re-ankylosis, growth disturbance, and facial asymmetry often occur. Even if the costochondral graft is successful, donor-site morbidity is inevitable. More recently, surgeons have become interested in distraction osteogenesis as a means of temporomandibular joint reconstruction. This case series presents the results of intraoral mandibular distraction osteogenesis and gap

BACKGROUND: Ameloblastoma is a locally aggressive tumor most often found in posterior body and angle of the mandible. Although ameloblastoma has histological characteristics of benignity, they have a high percentage of local recurrence and possible malignant development if treated improperly.
CASE REPORT: This report presents a treatment of unusual mandibular sequelae after tumor resection using recombinant human bone morphogenetic protein-2 (rhBMP-2) associated with hydroxyapatite (HA) and calcium triphosphate (TCP).
DISCUSSION: Seven months after surgery, the patient was asymptomatic, with

BACKGROUND: Intraoperative brachytherapy (IOBT) to the tumor bed coupled with surgery has been shown to increase survival and to improve locoregional disease control after head and neck tumor extirpation. Flap reconstruction attempts to restore patient anatomy, while also covering the radioactive implants. The purpose of this study was to better characterize the wound healing complications experienced by patients undergoing reconstruction in the setting of IOBT after tumor ablation, as well as to identify risk factors predicting complications and the need for reoperation.
METHODS: A retrospec

Defects of the temporomandibular joint (TMJ) severely compromise mandibular function and facial contour, rendering its reconstruction one of the most challenges in maxillofacial surgery. This study introduced a novel technique for TMJ reconstruction employing vascularized medial femoral condyle (MFC) osteochondral flap and aimed to evaluate its medium-term functional outcomes and stability. This observational study included 15 patients who received vascularized MFC osteochondral flap reconstruction for TMJ ankylosis or neoplasms. The follow-up ranged from 12 to 69 months (median:24 months). Th
