A strategic framework for managing gestational diabetes in Mexico.

Gestational Diabetes (GDM) is a prevalent health challenge in Mexico, affecting 10-14% of pregnancies but detected in only about 5.1% of cases, highlighting a critical gap in the healthcare system. This underdiagnosis poses severe health risks to mothers and children and reflects broader systemic healthcare failures. The disparity in detection rates points to insufficient screening protocols and uneven access to care, particularly affecting rural areas. Additionally, a lack of integrated digital health solutions exacerbates these issues, leading to inconsistent management and follow-up of diag

[Screening of pediatric oropharyngeal dysphagia in cerebral palsy].

Cerebral palsy is frequently associated with oropharyngeal dysphagia, affecting two out of three cases. Despite its high prevalence and the risk of serious complications (nutritional and respiratory), there are no standardized diagnostic protocols for its early detection in the pediatric population, highlighting a gap in the literature and clinical practice. The aim of this scoping review was to identify and describe the pediatric oropharyngeal dysphagia screening tools for cerebral palsy that have been validated into Spanish. A systematic search was conducted following PRISMA guidelines in Pu

Chronic kidney disease: Updates, priorities, and current challenges.

Chronic kidney disease (CKD) is a common condition affecting a significant proportion of the population but it is often unrecognized and underdiagnosed. Nurses should be aware of CKD because it is often comorbid with other, more widely recognized conditions such as diabetes and heart disease. This article discusses the prevalence, diagnosis, screening, and prevention of CKD, as well as guideline-directed interventions to delay progression and significant nurse strategies when caring for patients with CKD.

[Infectious disease screening of asylum seekers and refugees in Switzerland].

The number of asylum seekers and refugees in Switzerland has increased over the past 20 years, many of whom come from countries where the epidemiology of infectious diseases is different from that of Switzerland. In this review, we discuss the considerations taken into account when deciding which diseases to screen for in asylum seekers and refugees, and compare the strategies proposed by different countries. An overview of the epidemiology, clinical presentation, screening methods, and treatment strategies is provided for HIV, tuberculosis, hepatitis B and C, strongyloidiasis, schistosomiasis

Clinician-initiated opt-out HIV testing and the law.

BACKGROUND: 'Opt-out' HIV testing is a new approach to HIV health care whereby patients are routinely tested for HIV unless they explicitly decline. This new approach to testing for HIV appears in testing policy in Australia, guidelines in the US, and elsewhere. METHODS: This article examines the legal implications of opt-out testing in Australia. It uses legal doctrinal analysis to clarify the legal framework governing this practice in Australia, with a focus on 'informed consent' and requirements to inform the patient about the procedure and any associated risks. RESULTS: Our analysis find

Screening for Heart Failure: Biomarkers to Detect Heightened Risk in the General Population.

PURPOSE OF REVIEW: Heart failure (HF) represents a growing global burden of morbidity and mortality. Identifying individuals at risk for HF development is increasingly important, particularly given the advent of disease-modifying therapies for HF as well as its major risk factors such as obesity actalnd diabetes. We aim to review the key circulating biomarkers associated with future HF which may contribute to HF risk prediction. RECENT FINDINGS: While current guidelines recommend the use of natriuretic peptides and cardiac troponins in HF risk stratification, there are a diverse array of othe

A Computational Approach: The Functional Effects of Thyroid Peroxidase Variants in Thyroid Cancer and Genetic Disorders.

PURPOSE: Thyroid peroxidase (TPO) is essential for the synthesis of thyroid hormones. However, specific mutations render TPO antigenic and prone to autoimmune attacks leading to thyroid cancer, TPO deficiency, and congenital hypothyroidism (CH). Despite technological advancement, most experimental procedures cannot quickly identify the genetic causes of CH nor detect thyroid cancer in the early stages. METHODS: We performed saturated computational mutagenesis to calculate the folding energy changes (∆∆G) caused by missense mutations and analyzed the mutations involved in post-translational mo

Real world outcomes of distributing Lucira Check-It® COVID self-tests in Ontario, Canada: the GetaKit COVID study.

BACKGROUND: In Ontario, Canada we developed and implemented an online screening algorithm for the distribution of HIV self-tests, known as GetaKit. During the COVID pandemic, we adapted the GetaKit algorithm to screen for COVID based on population and infection data and distributed COVID rt-LAMP self-tests (using the Lucira Check-It®) to eligible participants. METHODS: GetaKit/COVID was a prospective observational study that occurred over a 7-month period from September 2021 to April 2022. All potential participants completed an online registration and risk assessment, including demographic i

Combined systematic screening for malnutrition and dysphagia in hospitalized older adults: a scoping review.

BACKGROUND: Dysphagia affects about 40% of patients admitted to acute geriatric wards, as it is closely associated with diseases that rise in prevalence with advancing age, such as stroke, Parkinson's disease, and dementia. Malnutrition is a highly associated predictive factor of dysphagia as well as one of the most common symptoms caused by dysphagia. Thus, the two conditions may exist simultaneously but also influence each other negatively and quickly cause functional decline especially in older adults. The purpose of this review was to determine whether institutions have established a proto

Trends in Detection of Adenoma and Sessile Serrated Lesions Over a Decade in a Community-Based Healthcare System.

INTRODUCTION: Adenoma detection rate (ADR) is an accepted benchmark for screening colonoscopy. Factors driving ADR and its relationship with sessile serrated lesions detection rate (SSLDR) over time remain unclear. We aim to explore patient, physician, and procedural influences on ADR and SSLDR trends. METHODS: Using a large healthcare system in northern California from January 2010 to December 2020, a total of 146,818 screening colonoscopies performed by 33 endoscopists were included. ADR and SSLDR were calculated over time using natural language processing. Logistic regression was used to c

Communicable Disease Screening and Human Immunodeficiency Virus Prevention in the Emergency Department.

Emergency departments (ED) provide care to populations with high rates of communicable diseases, like HIV, hepatitis C virus, and syphilis. For many patients, the ED is their sole entry point into the healthcare system and they do not routinely access screening and prevention services elsewhere. As such, the ED can serve an important public health role through communicable disease identification, treatment, and prevention. In this article, we examine national recommendations, peer-reviewed literature, and expert consensus to provide cutting edge strategies for implementing communicable infecti

Systematic screening for active tuberculosis amongst refugees and asylum seekers in Western Europe: Is universal chest radiography justified? A literature review.

INTRODUCTION: Tuberculosis (TB) prevalence is considered high amongst refugees and asylum seekers (RAAS). Active TB screening using universal (indiscriminate) chest radiography (CXR) has been adopted in several European countries. Universal CXR screening raises concerns for radiation protection; no existing research reviews the literature from a radiography perspective. This review aims to identify and analyse relevant primary research to discuss the justification of universal CXR for RAAS in an active TB screening context. METHODS: The search strategy included the databases CINAHL, MEDLINE,

Assessing the impact of increasing lung screening eligibility by relaxing the maximum years-since-quit threshold: A simulation modeling study.

BACKGROUND: In 2021, the US Preventive Services Task Force expanded its lung screening recommendation to include persons aged 50-80 years who had ever smoked and had at least 20 pack-years of exposure and less than 15 years since quitting (YSQ). However, studies have suggested that screening persons who formerly smoked with longer YSQ could be beneficial. METHODS: The authors used two validated lung cancer models to assess the benefits and harms of screening using various YSQ thresholds (10, 15, 20, 25, 30, and no YSQ) and the age at which screening was stopped. The impact of enforcing the YS

Stagnating rates of blood pressure control in Australia: insights from opportunistic screening of 10 046 participants of the May Measurement Month campaigns.

BACKGROUND: Raised blood pressure (BP) remains the single most important modifiable risk factor contributing to cardiovascular and all-cause mortality in Australia and worldwide. May Measurement Month , a global BP measurement and screening campaign initiated by the International Society of Hypertension and carried out in Australia since its inception in 2017, aimed at obtaining standardized BP measurements from members of the community to increase awareness of high BP and its associated risks. METHOD: Adults participants (≥18 years) were recruited through opportunistic sampling across Austra
