Intracranial Complication as a Manifestation of Clinical Onset in a Child With Insidious Ear Infection.

Acute mastoiditis is a common consequence of acute otitis media and may lead to intracranial complications. Common clinical presentations include otological complaints (i.e., otorrhea, otalgia, fever). Intracranial complication remains a rare manifestation of middle ear infection. We present the case of a child who presented with non-specific symptoms without any otological complaints. Prompt clinical assessment and imaging revealed an otogenic brain abscess with concurrent mastoiditis. Management of this child required both medical and surgical treatment by a multidisciplinary team.

Intramastoid solitary neurofibroma mimicking middle ear carcinoma.

We report an extremely rare case of intramastoid neurofibroma. A mass with destruction of the mastoid bone of a 51-year-old woman was examined with computed tomography. Subsequent magnetic resonance imaging demonstrated an ill-defined soft tissue mass with the opacification of mastoid air cells that had a mass effect in the same area. The patient underwent left subtotal temporal bone resection, and histological and immunohistochemical findings confirmed the lesion to be a neurofibroma. Given that similar imaging features of neurofibroma have been reported previously elsewhere in the head/neck …

Chronic otitis media with multiple complications.

Chronic otitis media (COM) is defined as persistent infection or inflammation of the middle ear and mastoid air cells. The proximity of the middle ear cleft and mastoid air cells to the intratemporal and intracranial compartments places structures located in these areas at increased risk of infectious complications. The complications of COM are divided into intracranial and extracranial complications. The development and appropriate use of antibiotics have led to a decrease in these potentially devastating complications. However, they continue to occur, and clinical vigilance is required for e…

The deformed petrous bone: a new plain film sign of premature lambdoid synostosis.

Towne-view skull radiographs of 38 infants with unilateral lambdoid synostosis were reviewed. The diagnosis of lambdoid synostosis was based on other plain film findings, CT findings, or clinical examination. All patients had deformity of the ipsilateral petrous bone, presumably because of pressure caused by redirected brain growth. Specific changes included flattening of the petrous ridge and depression of the roof of the mastoid portion of the petrous bone and adjacent structures such as the semicircular canals. Flattening of the petrous ridge and depression of the roof of the mastoid portio…

Surgical pitfalls encountered with the semi-implantable middle ear transducer ossicular stimulator (MET-Otologics).

OBJECTIVE: To determine whether surgical pitfalls can be anticipated through a preoperative CT-Scan in case of middle ear implantation with a semi-implantable middle ear ossicular stimulator from Otologics (SIMOS). A second outcome measure was to assess the auditory gain obtained in the series. MATERIALS AND METHODS: A retrospective analysis of 19 consecutive surgical procedure for a SIMOS implantations was achieved. Preoperative Ct-Scans were analyzed and several measurements of the mastoid were taken out from axial transverse views, ie. the distance between the incus and the cortical mastoi…

[Adjusting the geometry of implantable hearing aid components to human temporal bone. I: Electromechanical transducer].

Prerequisite to implantation of a piezoelectrical transducer of an implantable hearing aid is a shape allowing its implantation into human mastoid and middle ear. To approach this problem, a consecutive series of six transducer prototypes was created in an iterative process. Their functional geometry was evaluated in 50 human temporal bones. A shape for a functioning transducer was found which will enable implantation in 78% of the cases examined (confidence interval: 61.5%-89.2%). It will allow simultaneous implantation of the transducer into the mastoid and microphone, which is situated tran…

Tuberculous otitis media as a secondary infection to chronic otitis media with cholesteatoma.

A series of six cases of tuberculous otitis media is reviewed. All patients had a history of chronic otorrhea and were operated on with a presumptive diagnosis of chronic otitis media with cholesteatoma. Postoperatively the diagnosis of tuberculosis was established by histologic examination of the granulation tissue from the middle ear and mastoid. We believe that any patient with a long history of discharging ears needs histologic examination, as tuberculous otitis might be the cause of infection. We report our findings in these patients and discuss the possibility of penetration of tuberculo…

The use of fluoroquinolones in chronic otitis suppurativa.

The present review covers fluoroquinolone usage in chronic otitis suppurativa (COS) in case of chronic otitis media, cholesteatoma, radical mastoid cavity infection and chronic or relapsing otitis externa. A total of six publications were included in the final evaluation. Enoxacin was effective in 35%; ciprofloxacin (five publications) was used in 82 patients with 67% effectivity in otitis externa and otitis media and 61% effectivity in radical mastoid cavity infection. No serious adverse reactions were reported. The promising efficacy of otitis media and otitis externa and safety profile need…

Meningoencephalocele of the middle ear cleft: Three tier management.

UNLABELLED: Meningoencephalic herniation into the middle ear is a rare condition that can be life threatening for the patient due to eventual infective intracrunial complications. It is characterized by a bony and dural defect localized in the tegmen through which meninges and encephalic tissue can herniate. Meningoencephalic hernia of the middle ear can appear as an insidious complication of Chronic mastoiditis (infective) and its surgery, trauma or spontaneous. High resolution computed tomography (CT scan) and Magnetic resonance imaging (MRI) must be done to evaluate the extent and the conte…

Perception and Propagation Characteristics of Bone-Conducted Stimuli Presented to the Human Facial Cranium.

Since bone conduction (BC) has the advantage that it does not cover the ear canal and can be easily heard even when earplugs are worn, it has been applied to various communication devices. Conventional BC is mainly applied to the mastoid process of the temporal bone (the osseous bulge behind the ear), however, some of recent BC devices, such as smart glasses, present stimuli to faces. The face has very complex structures in the human body; therefore, it is highly likely that the hearing and propagation characteristics of sound will change depending on the part to which sound is presented. Howe…

Giant Spontaneous Epidural Pneumatocele: Case Report and Review of the Literature.

BACKGROUND: Pneumocephalus is a commonly encountered finding in neurosurgery in which air displaces intracranial cerebrospinal fluid after cranial surgery or a cerebrospinal fluid leak into paranasal or mastoid sinuses. When an intracranial air collection becomes chronically established in a fixed loculation causing mass effect, pneumatocele is a more appropriate term. We present an unusual case of a spontaneous giant frontotemporal epidural pneumatocele that persisted for more than 1 year before the patient presented for neurological treatment. CASE DESCRIPTION: A 40-year-old man with a remo…
