Effect of critical access hospital conversion on patient safety.

BACKGROUND: The Medicare Rural Hospital Flexibility Program of the 1997 Balanced Budget Act allowed hospitals meeting certain criteria to convert to critical access hospitals (CAH) and changed their Medicare reimbursement mechanism from prospective payment system (PPS) to cost-based. OBJECTIVE: To examine the impact of CAH conversion on hospital patient safety. DATA SOURCE: Secondary data on hospital patient safety indicators (PSIs), hospital CAH status, patient case-mix, and market variables, for 89 Iowa rural hospitals during 1997-2004. STUDY DESIGN: We employed quasi-experimental designs

Antimicrobial consumption at Auckland City Hospital: 2006-2009.

AIMS: We aimed to determine the level of antimicrobial consumption by adult inpatients at Auckland City Hospital (Auckland, New Zealand) and to compare our findings with those in other developed nations. METHODS: We used the computerised records of the central Auckland District Health Board (ADHB) pharmacy to measure the amount of antimicrobials dispensed to inpatients (excluding psychiatric units, day stay units and outpatient clinics) during 2006 to 2009. The total weight of each antimicrobial dispensed was used to determine the number of defined daily doses (DDDs) dispensed. The Informatio

Healthcare Resource Utilization and Cost of Invasive Meningococcal Disease in Ontario, Canada.

BACKGROUND: Invasive meningococcal disease (IMD) is associated with significant morbidity and mortality, thus remaining a concern for healthcare providers and the public. Evidence of the longitudinal burden of IMD and associated costs are scarce. Here we have evaluated the healthcare utilization and cost associated with hospitalized IMD cases in Ontario, Canada. METHODS: Observational cohort study utilizing the Ontario provincial claims databases, comprising: (1) individuals hospitalized with IMD between January 1995 and June 2012 and (2) age-, gender- and area-matched non-IMD controls (1:20

The ethics of caring for hospital-dependent patients.

BACKGROUND: Hospital-dependent patients are individuals who are repeatedly readmitted to the hospital because their acute medical needs cannot be met elsewhere. Unlike the chronically critically ill, these patients do not have a continuous need for life-sustaining equipment and can experience periods of relative stability where they have a good quality of life. However, some end up spending months or even years in the hospital receiving resource-intensive care because they are unable to be safely discharged, despite an initial optimistic prognosis. It is hard to reliably identify these patient

Epidural vs. systemic analgesia in the Intensive Care Unit: retrospective study of patient outcomes.

BACKGROUND: Patients admitted to the Intensive Care Unit (ICU) often experience acute pain. Causes include major surgery, multisystem trauma, and pancreatitis. Most ICU patients who require pain management are treated with systemic analgesia, usually intravenous opioids. This study compared the rate of pain and delirium scores, as well as mortality and morbidity between ICU patients treated with systemic vs. epidural analgesia. METHODS: This retrospective analysis included patients who were in the ICU from January 2011 to June 2021, admitted due to thoracic, abdominal, pelvic, or lower limb s

Hospital evacuation: Exercise versus reality.

INTRODUCTION: The Dutch Major Incident Hospital (MIH) is a standby, highly prepared, 200-bed hospital strictly reserved to provide immediate, large-scale, and emergency care for victims of disasters and major incidents. It has long-standing experience training for various major incident scenarios, including functioning as a back-up facility for the Netherlands. In 1995, the MIH had experience with overtaking an evacuated hospital when that hospital was threatened by flooding. In November 2014, an exercise was performed to transfer an evacuating hospital to the MIH. The scenario again became re

Evaluation of anticholinergic burden in elderly outpatients and the risk factors.

OBJECTIVES: The use of anticholinergic drugs in the elderly may lead to negative events such as falls, delirium, urinary retention and cognitive decline, and the higher the number of anticholinergic drugs use, the more such negative events occur. This study aims to analyze the risk factors associated with the prescription of total anticholinergic drugs in elderly outpatients and evaluate the rationality of anticholinergic drugs, and to provide a reference for reducing the adverse effects of anticholinergic drugs. METHODS: A list of drugs with anticholinergic activity based on the Beers criter

The Role of Emergency Medical Services in Addressing Opioid Use Disorder-A Position Statement and Resource Document of NAEMSP.

Opioid overdose remains one of the most urgent public health crises in the United States, and for many individuals, the first and sometimes only encounter with health care comes through emergency medical services (EMS). As a result, the role of EMS has evolved far beyond reversing overdoses in the moment. Opioid use disorder (OUD) is a treatable medical disease with effective treatment, and EMS agencies are uniquely positioned to partner with local community resources and hospitals to combine efforts in implementing harm-reduction and treatment strategies. This document provides specific guida

Ethical challenges in medical error disclosure among healthcare professionals in teaching hospitals: a qualitative study.

BACKGROUND: Medical error disclosure is a fundamental component of patient safety and professional ethics in healthcare systems; however, in practice, it is frequently influenced by individual, organizational, and contextual factors. Although international research has examined issues such as blame culture and psychological safety, an in-depth qualitative understanding of the ethical challenges surrounding medical error disclosure in Iranian teaching hospitals remains limited. PURPOSE: This study aimed to explore the ethical challenges of medical error disclosure among healthcare professional

Natural Language Processing for Identification of Hospitalized People Who Use Drugs: Cohort Study.

BACKGROUND: People who use drugs (PWUD) are at heightened risk of severe injection-related infections. Current research relies on billing codes to identify PWUD-a methodology with suboptimal accuracy that may underestimate the economic, racial, and ethnic diversity of hospitalized PWUD. OBJECTIVE: The goal of this study is to examine the impact of natural language processing (NLP) on enhancing identification of PWUD in electronic medical records, with a specific focus on determining improved systems of identifying populations who may previously been missed, including people who have low incom

Hybrid electronic record: An error reduction strategy for diverse medical prescription formats.

BACKGROUND: This project is part of a broader effort to develop a new electronic registry for ophthalmology in the KwaZulu-Natal (KZN) province in South Africa. The registry should include a clinical decision support system that reduces the potential for human error and should be applicable for our diversity of hospitals, whether electronic health record (EHR) or paper-based. METHODS: Post-operative prescriptions of consecutive cataract surgery discharges were included for 2019 and 2020. Comparisons were facilitated by the four chosen state hospitals in KZN each having a different system for

Improving healthcare value: Optimizing billing and documentation for inpatient addiction consultation services.

Patients with substance use disorder (SUD) represent a high-complexity, high-cost population on hospital medicine services, yet their admissions are systematically under-reimbursed due to narrow clinical documentation. Increasingly, hospital-based addiction consultation service (ACS) clinicians provide dedicated addiction care to medically complex hospitalized patients. Hospitalizations coded solely for SUD fail to capture clinical complexity and concomitant diagnoses, depressing diagnosis-related group weights and Case Mix Index. We describe three actions for ACS clinicians, hospitalists, and

Radioimaging and Demographic Profiles of Patients with Spontaneous Intracerebral Hemorrhage: A Need for the Pragmatic Referral System.

Referring patients with severe medical conditions, like brain stroke, from small or rural hospitals to tertiary care centers is often overexploited leading to a kind of defensive medicine. The issue of a patient referral system remains unaddressed in Nepal. In this article, we investigated the legitimacy of referring patients with spontaneous intracranial hemorrhage (ICH) from country peripheral hospitals to the leading tertiary neurological center in Nepal. We found that 81 out of the 130 ICH patients reviewed in the study were referred. We further show that the classifiers to be considered m

Multisite Quality Improvement Initiative to Identify and Address Racial Disparities and Deficiencies in Delivering Equitable, Patient-Centered Care for Multiple Myeloma-Exploring the Differences between Academic and Community Oncology Centers.

Treatment of multiple myeloma (MM) is complex; however, with equal access to care, clinical outcomes for Black patients match those in other patient groups. To reveal and begin to address clinical practice barriers to equitable, patient-centered MM care, this quality improvement (QI) initiative assessed patient electronic medical records (EMRs) and surveyed patients and providers at two large hospital systems and four community-based practices. For the educational intervention, providers participated in feedback-focused grand rounds sessions to reflect on system barriers and develop action pla
