Complete bilateral ophthalmoplegia resistant to caloric stimulation in bilateral paramedian midbrain-thalamic infarction.

A 79-year-old woman who developed bilateral paramedian midbrain-thalamic infarction manifested complete bilateral ophthalmoplegia resistant to caloric stimulation, indicating impairment of the vestibulo-ocular reflex (VOR). Previous reports have mentioned this phenomenon but have not explicitly reported the results of caloric testing. Why a lesion apparently confined to the upper brainstem should produce impairment of the horizontal VOR remains unexplained.

Repeated digital substraction angiography after perimesencephalic subarachnoid hemorrhage?

BACKGROUND AND PURPOSE: In patients with perimesencephalic subarachnoid hemorrhage (pSAH) DSA is recommended to exclude aneurysms to due false negative findings in CT-angiography. However, whether a second DSA is indicated during the clinical course to exclude--in addition to aneurysms--fistulas, too, is still under debate. We aimed to evaluate the benefit of repeated DSA in patients with pSAH. METHODS: The source of data was a prospective database set up at the neurological, neurosurgical and neuroradiological departments in our institution. A total of 69 patients with pSAH were enrolled and

Superior divisional oculomotor nerve palsy caused by midbrain neurocysticercosis.

Superior divisional oculomotor nerve palsy caused by intrinsic brainstem disease occurs rarely. We herein report, to our knowledge, the first case of midbrain neurocysticercosis presenting as isolated superior divisional paresis of the oculomotor nerve. A 25-year-old woman presented with acute onset of headache and ptosis-upgaze palsy of the right eye. Results of the CT scan and MRI of the brain were compatible with neurocysticercosis at the left midbrain. She was dramatically responded to albendazole treatment. The diagnosis was confirmed by positive enzyme-linked immunosorbent assay (ELISA)

[Diffusion-weighted magnetic resonance imaging findings at early stage of acute Wernicke encephalopathy].

We describe a patient with acute Wernicke encephalopathy (WE) in whom diffusion-weighted magnetic resonance imaging (DWI) were helpful for early diagnosis. A 66-year-old alcoholic man was admitted to our department because of recurrent mild drowsiness. Thiamine concentrations in blood were at the lower limit of normal. DWI demonstrated an abnormal signal intensity in the dorsal part of the midbrain, and high-dose thiamine therapy was started. These lesions disappeared on DWI after one month of follow-up, in association with clinical improvement. These findings suggest that DWI is useful for de

Both neural crest and placode contribute to the ciliary ganglion and oculomotor nerve.

The chick ciliary ganglion is a neural crest-derived parasympathetic ganglion that innervates the eye. Here, we examine its axial level of origin and developmental relationship to other ganglia and nerves of the head. Using small, focal injections of DiI, we show that neural crest cells arising from both the caudal half of the midbrain and the rostral hindbrain contribute to the ciliary as well as the trigeminal ganglion. Precursors to both ganglia have overlapping migration patterns, moving first ventrolaterally and then rostrally toward the optic vesicle. At the level of the midbrain/forebra

Study of the rostral midbrain atrophy in progressive supranuclear palsy.

Rostral midbrain atrophy in progressive supranuclear palsy (PSP) is detected by mid-sagittal plain magnetic resonance imaging (MRI). The shape of the atrophy looks like the bill of a hummingbird (hummingbird sign). We studied this sign to elucidate the nature of midbrain atrophy in PSP. Eight patients with PSP, 12 with Parkinson's disease (PD), and 10 normal controls were studied. Using mid-sagittal plain MRI, we measured the rostral and caudal midbrain tegmentum (MT), superior and inferior colliculus, pontine base, and tegmentum. We compared the length of the interpeduncular fossa, which is p

Imaging of acute bilateral paramedian thalamic and mesencephalic infarcts.

Thalami and midbrain arterial supply arises from many perforating blood vessels with a complex distribution for which many variations have been described. One rare variation, named the "artery of Percheron," is a solitary arterial trunk that arises from one of the proximal segments of a posterior cerebral artery and supplies the paramedian thalami and the rostral midbrain bilaterally. Occlusion of this artery results in bilateral thalamic and mesencephalic infarctions. We describe three patients with a presumed occlusion of the artery of Percheron in whom MR imaging showed characteristic symme

[A case presenting vertical one-and-a-half syndrome and seesaw nystagmus due to thalamomesencephalic infarction].

A 58-year-old woman presented, conjugate upgaze palsy and monocular paresis of downward gaze in the ipsilateral eye (vertical one-and-a-half syndrome; VOHS) as well as seesaw nystagmus (SSN). Vertical oculocephalic response and conjugate horizontal gaze were preserved. Magnetic resonance imaging revealed a right thalamo-mesencephalic infarction including the rostral interstitial nucleus of the medial longitudinal fasciculus (riMLF) and the interstitial nucleus of Cajal. On the 22nd hospital day SSN was disappeared, and then on the 32nd day VOHS was improved. The lesions of VOHS may have affect

Brainstem infarctions with normal MRI.

Most studies on brainstem infarctions included only patients with lesions documented by CT or MRI. The aim of this study was to analyse the clinical symptomatology in patients with the classical signs of brainstem infarcts and normal MRI results. Frequencies of MR-positive and negative infarctions should be analysed according to their location. In a series of 30 consecutive patients with acute clinical symptoms of ischemic brainstem lesions and persistence of the symptomatology for more than 10 days, 8 patients had normal MRI. In these patients the location of the lesion was established by cli

Concurrent excitatory and inhibitory effects of high frequency stimulation: an oculomotor study.

OBJECTIVE: To describe a reversible neurological condition resembling a crossed midbrain syndrome resulting from high frequency stimulation (HFS) in the midbrain. METHODS: Postoperative evaluation of quadripolar electrodes implanted in the area of the subthalamic nucleus of 25 patients with Parkinson's disease (PD) successfully treated by HFS. RESULTS: Four of the 25 patients experienced reversible acute diplopia, with dystonic posture and tremor in the contralateral upper limb when the white matter between the red nucleus and the substantia nigra was stimulated. The motor signs resembled th

Peripeduncular lesion alters expression of c-fos induced in the female rat brain by male mounting.

In order to investigate the role of the peripeduncular nucleus (PP) in the control of lordosis in female rats, activation of neurons after mounts without intromission was investigated by means of FOS immunoreactivity (FOS-IR). Ovariectomized rats were injected with estradiol and progesterone and submitted to approximately 50 mounts by the male. The vaginal area was covered with masking tape to prevent intromission and vaginocervical stimulation. This limited stimulation produced FOS-IR in the ventrolateral division of the ventromedial hypothalamic nucleus (VMHVL), in the lateral periaqueductal

Effect of subglottic pressure on fundamental frequency of the canine larynx with active muscle tensions.

The relation between subglottic pressure and the fundamental frequency of vocal fold vibration was studied by means of evoked phonation in an in vivo canine model. The evoked-phonation model involved electrical stimulation of the midbrain that resulted in consistent responses by respiratory and laryngeal musculature, accompanied by phonation. The dynamic stiffness properties of the vocal folds, especially the "cover," were investigated by delivering various amounts of air pressure to the larynx from an opening in the trachea. The fundamental frequency of vocal fold vibration increased linearly

A modulatory role of central cholinergic transmission in control of the 10-Hz rhythm in sympathetic nerve discharge.

In 43 urethane-anesthetized or decerebrate, baroreceptor-denervated cats, spectral analysis showed that most of the power in sympathetic nerve discharge (SND) was at frequencies < 6 Hz. In 18 of these cats, physostigmine (100 micrograms/kg i.v.) induced a 10-Hz rhythm in inferior cardiac SND that was eliminated by atropine sulfate (0.25 mg/kg i.v.; n = 6). In contrast, the naturally occurring 10-Hz rhythm that appeared in SND in other experiments was atropine-insensitive (n = 6). The data indicate that central muscarinic cholinergic transmission is not essential for the naturally occurring 10-

Glutathione peroxidase, glial cells and Parkinson's disease.

Hyperoxidation phenomena are suspected to be involved in dopaminergic cell death in Parkinson's disease, which affects preferentially the neuromelanin-containing dopaminergic neurons of the substantia nigra. Glutathione peroxidase is the major protective enzyme against hydrogen peroxide toxicity. The distribution of glutathione peroxidase-containing cells was investigated by immunohistochemistry in the midbrain of four control subjects and four patients with Parkinson's disease. (1) Glutathione peroxidase-like immunoreactivity was detected exclusively in glial cells. (2) In control brains, the

Spontaneous midbrain hemorrhage: report of seven new cases.

Spontaneous midbrain hemorrhage is a unique brain-stem vascular lesion. Sixty-six cases have been reported in the literature, to which we add seven. Of the total of 73 patients reviewed, most presented acutely (66%), but subacute (22%) and chronic onsets (12%) also occurred. Neuro-ophthalmologic abnormalities were found in 88% of patients. Some alteration of consciousness and headache occurred in most patients. Hemiparesis, hemisensory loss, and ataxia were less frequent manifestations. Vascular malformations were suspected or proved in 37% of patients, hypertension was present in 21%, and a b

Mesencephalic cholinergic nuclei in progressive supranuclear palsy.

Using an antibody against choline acetyltransferase (ChAT), mesencephalic cholinergic cell nuclei were studied in autopsy material from 3 cases of progressive supranuclear palsy (PSP) and 4 controls. ChAT-immunoreactive neurons were quantified in sections that spanned the rostrocaudal extent of each nucleus. In PSP, there was a significant decrease in the number of neurons with detectable immunoreactivity for ChAT in and adjacent to the central gray substance in the following nuclei: the nucleus of Edinger-Westphal (69%); the rostral interstitial nucleus of the medial longitudinal fasciculus (
