Adaptive cancellation of floor vibrations in standing ballistocardiogram measurements using a seismic sensor as a noise reference.

An adaptive noise canceller was used to reduce the effect of floor vibrations on ballistocardiogram (BCG) measurements from a modified electronic bathroom scale. A seismic sensor was placed next to the scale on the floor and used as the noise reference input to the noise canceller. BCG recordings were acquired from a healthy subject while another person stomped around the scale, thus causing increased floor vibrations. The noise canceller substantially eliminated the artifacts in the BCG signal due to these vibrations without distorting the morphology of the measured BCG. Additionally, recordi

Ten years of change to coral communities off Mona and Desecheo Islands, Puerto Rico, from disease and bleaching.

Remote reefs off southwest Puerto Rico have experienced recent losses in live coral cover of 30 to 80%, primarily due to the decline of Montastraea annularis and M. faveolata from disease and bleaching. These species were formerly the largest, oldest, and most abundant corals on these reefs, constituting over 65% of the living coral cover and 40 to 80% of the total number of colonies. From 1998 to 2001, outbreaks of yellow band disease (YBD) and white plague (WP) affected 30 to 60% of the M. annularis (complex) colonies. Disease prevalence declined beginning in 2002, and then increased immedia

A review on techniques for tremor recording and quantification.

Tremor is the most common movement disorder and differs from other disorders by its repetitive, stereotyped movements, with regular frequency and amplitude. The three most frequent pathological forms of it are the essential tremor (ET), the Parkinson's disease (PD) tremor, and the enhanced physiological tremor. The ET and PD tremor affect the older population mostly. Although there are cases of tremor reported since ancient times, there is currently no consensus about its causes or about its main differential characteristics. In this article, we present a review of the methods more frequently

Gastroesophageal reflux disease. Heart-burn from a psychological view.

Although gastroesophageal reflux disease (GERD) can be traced back to disorders of the gastroesophageal junction, stress and other relevant psychological factors can play an important role in the process of GERD. It would seem that, primarily, altered symptom perception based on threshold reduction exists in some patients. In an effort to describe the sensitisation for reflux symptoms, both central and peripheral factors can be discussed. The following is conceivable: that well defined personality factors moderate the effect of stress on the gastroesophageal junction, just as they can influenc

Auditory evoked potentials.

Auditory evoked potentials (AEPs) are an electrical manifestation of the brain response to an auditory stimulus. Mid-latency auditory evoked potentials (MLAEPs) and the coherent frequency of the AEP are the most promising for monitoring depth of anaesthesia. MLAEPs show graded changes with increasing anaesthetic concentration over the clinical concentration range. The latencies of Pa and Nb lengthen and their amplitudes reduce. These changes in features of waveform are similar with both inhaled and intravenous anaesthetics. Changes in latency of Pa and Nb waves are highly correlated to a trans

Pathophysiology and management of abdominal compartment syndrome.

Abdominal compartment syndrome is a potentially lethal condition caused by any event that produces intra-abdominal hypertension; the most common cause is blunt abdominal trauma. Increasing intra-abdominal pressure causes progressive hypoperfusion and ischemia of the intestines and other peritoneal and retroperitoneal structures. Pathophysiological effects include release of cytokines, formation of oxygen free radicals, and decreased cellular production of adenosine triphosphate. These processes may lead to translocation of bacteria from the gut and intestinal edema, predisposing patients to mu

Multi-sensors acquisition, data fusion, knowledge mining and alarm triggering in health smart homes for elderly people.

We deal in this paper with the concept of health smart home (HSH) designed to follow dependent people at home in order to avoid the hospitalisation, limiting hospital sojourns to short acute care or fast specific diagnostic investigations. For elderly people the project of such a HSH has been called AISLE (Apartment with Intelligent Sensors for Longevity Effectiveness). For this purpose, system having three levels of automatic measuring (1) the circadian activity, (2) the vegetative state, and (3) some state variables specific of certain organs involved in precise diseases, has been developed

Controlling and monitoring polymyositis under steroid therapy.

This paper illustrates how considerable information may be extracted from the easily obtained serum creatine kinase levels as a function of prednisolone intake in controlling and monitoring polymyositis effectively. A simple process is developed to enable a clinician to determine readily the optimum prednisolone intake to give the minimum average steroid levels within the patient with an effective reduction in the muscle damage arising from the polymyositis. As part of the process it is demonstrated how the effect of other drugs or natural products may be ascertained and monitored. The effects

In neonates with paroxysmal events, video-EEG monitoring proves to be a powerful tool for diagnosis. It allows differentiation of epileptic seizures from other paroxysmal phenomena; a more precise analysis of clinical semiology of seizures; it may help in the localization of a seizure focus; it provides more comprehensive data for the evaluation of the interictal state.

Dengue hemorrhagic fever: clinical manifestations and management.

Dengue virus infection may remain asymptomatic or manifest as nonspecific viral infection to life threatening dengue hemorrhagic fever (DHF)/dengue shock syndrome (DSS). Patients with DHF/DSS have fever, hemorrhagic manifestations along with thrombocytopenia and hemoconcentration. Thrombocytopenia and hemoconcentration are distinguishing features between DHF/DSS and dengue fever (DF). Some patients with dengue fever may have significant bleed and mild thrombocytopenia but no hemoconcentration. These patients are labelled to have dengue fever with unusual bleeds. Laboratory findings in DHF/DSS

Expiratory flow limitation during mechanical ventilation detected by the forced oscillation method.

We have previously observed large phasic variations of respiratory mechanical impedance in chronic obstructive pulmonary disease (COPD) patients mechanically-ventilated for acute respiratory failure, and postulated that they were due to expiratory flow limitation (EFL). The aim of this study was to test that assumption experimentally and to assess the value of impedance for automatic and noninvasive detection of EFL during mechanical ventilation. The study was performed: 1) in a mechanical analogue, including a flow-limiting element; and 2) in eight anaesthetized and paralysed rabbits, before

[Anesthetic management of a neonate with critical pulmonary valve stenosis for balloon pulmonary valvuloplasty].

We administered general anesthesia for balloon pulmonary valvuloplasty (BPV) to a 19 day-old male infant, weighing 2,789g, with critical pulmonary stenosis. The patient had severe cyanosis and mild right heart failure. Atropine (0.01 mg.kg-1) was administered intravenously immediately before induction of anesthesia. Pancuronium (0.4 mg) was used to facilitate endotracheal intubation and for the subsequent control of ventilation. Anesthesia was maintained with oxygen and enflurane (0.25 approximately 0.5%) supplemented with intravenous administration of fentanyl (1.5 micrograms.kg-1). During ca

The Australian Incident Monitoring Study. Crisis management--validation of an algorithm by analysis of 2000 incident reports.

Anaesthetists are called upon to manage complex life-threatening crises at a moment's notice. As there is evidence that this may require cognitive tasking beyond the information-processing capacity of the human brain, it was decided to try and develop a generic crisis management algorithm analogous to the "Phase I" immediate response routine used by airline pilots. Such an algorithm, based on the mnemonic "COVER ABCD, A SWIFT CHECK", was developed and refined over 3 meetings, each attended by 60-100 anaesthetists and aviation psychologists. It was validated against 1301 relevant incidents amon

Carbamazepine induced bradycardia--a problem in general or only in susceptible patients? A 24-h long-term electrocardiogram study.

Carbamazepine is a first line drug in the treatment of epilepsy and trigeminal neuralgia, but may exert negative chronotropic and dromotropic effects on the cardiac conduction system. Bradyarrhythmias of different types and severity have been described, especially in the elderly, but the prevalence of arrhythmias in a larger group of carbamazepine treated patients is unknown. Forty-eight patients, 40 years of older, on continuous carbamazepine treatment because of various neurologic disorders were investigated by interview, physical examination, 12-lead surface electrocardiogram, and 24-h long

Prediction of nipple viability following reduction mammoplasty using laser Doppler flowmetry.

The assessment of viability of a pedicled nipple-areola complex after reduction mammoplasty frequently may be frustrating due to equivocal clinical signs of adequate blood flow. Although conversion to a composite graft is always a safe option, the aesthetic result may be inferior. An objective monitor might be beneficial to maximize the surgical outcome while minimizing the risk of nipple necrosis. Laser Doppler flowmetry provides a safe, simple, and noninvasive objective monitor that allows continuous intra-operative or postoperative evaluation of nipple perfusion. An evaluation of 31 nipples
