What can we Learn from Patients' Ethical Thinking about the right 'not to know' in Genomics? Lessons from Cancer Genetic Testing for Genetic Counselling.

This article is based on a qualitative empirical project about a distinct kinship group who were among the first identified internationally as having a genetic susceptibility to cancer (Lynch Syndrome). 50 were invited to participate (42 were tested; eight declined genetic testing). 15, who had all accepted testing, were interviewed. They form a unique case study. This study aimed to explore interviewees' experiences of genetic testing and how these influenced their family relationships. A key finding was that participants framed the decision to be tested as 'common sense'; the idea of choice

Attitudes towards end-of-life decisions and the subjective concepts of consciousness: an empirical analysis.

BACKGROUND: People have fought for their civil rights, primarily the right to live in dignity. At present, the development of technology in medicine and healthcare led to an apparent paradox: many people are fighting for the right to die. This study was aimed at testing whether different moral principles are associated with different attitudes towards end-of-life decisions for patients with a severe brain damage. METHODOLOGY: We focused on the ethical decisions about withdrawing life-sustaining treatments in patients with severe brain damage. 202 undergraduate students at the University of Pa

Paid organ procurement: pragmatic and ethical viewpoints.

None of the familiar arguments against paid organ procurement really works. Most arguments spring from revulsion that arises from the abuses of the practice rather than the act itself. This has not stopped official condemnation. However, the evidence we have at present is that such condemnation, aimed at correcting a perceived moral wrong rather than at abuses of a practice (the correction of which might actually bring some moral good), has utterly failed, and many more countries including the United States now harbor paid organ procurement. We can try to develop and articulate a truly compell

[Psychological and ethical determinants of professional burnout in healthcare workers: a systematic review].

INTRODUCTION: Professional burnout of medical personnel is recognized as a systemic factor that threatens patient safety and human resource sustainability of healthcare. The modern scientific paradigm shifts the focus from individual stress tolerance of a specialist to organizational and ethical determinants, including the concept of moral trauma. GOAL: To analyze the psychological and ethical factors of burnout among medical professionals based on the synthesis of relevant empirical data. MATERIALS AND METHODS: A systematic review of the literature for the period 2019-2026 has been carried

Trust tangles: Who do kids trust? Unraveling young children's attitudes in the gossip mill.

OBJECTIVE: As an important channel of social-information transmission, gossip enables individuals to acquire reputational information indirectly and to guide partner selection. This study systematically investigates the cognitive development from social evaluation to trust-based decision-making in children. Through two experiments, we examine how young children evaluate different roles in gossip scenarios and how those evaluations influence subsequent trust decisions. METHODS: Study 1 examined how 141 children (aged 5-7) integrated factors such as gossip valence, emotional cues, and informati

Clinical decision fatigue as a reversible state in the continuum of professional exhaustion.

Concerns about clinician fatigue typically focus on burnout and workload, yet less attention has been paid to how the process of making repeated, emotionally and ethically challenging decisions may transiently affect decision quality. The concept of clinical decision fatigue (CDF) remains debated, partly because empirical findings differ across clinical contexts. We propose that CDF is best understood as a short-term, reversible state arising from sustained self-regulatory demands during emotionally and morally salient clinical decision-making. Drawing on contemporary process models of self-re

Navigating Conflicts of Duty in Contexts of Compassionate Care: Utilitarian Reasoning as a Practice-Oriented Framework.

Compassionate nursing practice frequently requires nurses to navigate competing obligations, conflicting patient needs and constrained resources. While compassion motivates responsiveness to suffering, it does not always provide sufficient guidance for determining how such conflicts should be resolved. We argue that contemporary forms of utilitarianism may offer a useful framework for structuring compassionate practice in nursing within ethically pluralistic clinical environments. Drawing on contemporary utilitarian thought, particularly preference utilitarianism, the paper examines how attent

Factors associated with gerontological nursing practice among nurses in comprehensive nursing care service units: A cross-sectional descriptive study.

PURPOSE: In comprehensive nursing care service units (CNCSUs), where family caregivers are rarely present, nurses are primarily responsible for both medical treatment and daily care, making professional gerontological nursing practice essential. This study investigated factors associated with gerontological nursing practice among nurses in CNCSUs. METHODS: A cross-sectional descriptive study was conducted. The participants included 153 nurses at two general hospitals in Korea. A structured self-reported questionnaire was administered to assess general characteristics, perceived patient safety

Emotional conscience in individuals with affective disorders.

OBJECTIVES: The aim of the study was to analyse the functioning of emotional conscience in individuals with mood disorders, taking into account the differences between patients with recurrent depression and patients with bipolar disorder. The dependence of the susceptibility to experiencing particular moral feelings on the symptoms of the illness - both on their type and severity - was also a subject of consideration. METHODS: Results from 131 subjects were analysed, including 44 patients with bipolar disorder, 33 patients with recurrent depression and 54 individuals without a diagnosis of me

Bioethics Artificial Intelligence Advisory (BAIA): An Agentic Artificial Intelligence (AI) Framework for Bioethical Clinical Decision Support.

Healthcare professionals face complex ethical dilemmas in clinical settings in cases involving end-of-life care, informed consent, and surrogate decision-making. These nuanced situations often lead to moral distress among care providers. This paper introduces the Bioethics Artificial Intelligence Advisory (BAIA) framework, a novel and innovative approach that leverages artificial intelligence (AI) to support clinical ethical decision-making. The BAIA framework integrates multiple bioethical approaches, including principlism, casuistry, and narrative ethics, with advanced AI capabilities to pro

The relationship between obsessions and the self: Feared and actual self-descriptions in a clinical obsessive-compulsive disorder sample.

Cognitive models of obsessive-compulsive disorder (OCD) posit the relevance of the self in OCD, although the nature of this association is still unclear. We aimed to explore actual and feared selves and its association with obsessions and intrusions in a group of OCD patients. A group of 58 patients with OCD identified their most upsetting obsession and intrusion (non-clinical obsession) experienced in the past 3 months. These cognitions were classified as either moral-based or autogenous (obsessions n = 32; intrusions n = 26) or non-moral-based or reactive, depending on their content. Next, p

That's not fair: Children's neural computations of fairness and their impact on resource allocation behaviors and judgments.

The ability to distinguish between mere equality in resource distributions and fairness based on a broader range of contextual factors is of paramount importance in social decision making and is a critical component of morality. Children's developmental shift from viewing inequality as a dichotomous moral issue toward a more nuanced understanding of partial inequality has been well documented across middle childhood and is attributed to a host of potential theoretical underpinnings, including developing number concept, increased regard for one's social status, and a maturing concept of fairnes

Efficacy of legal judgments for defendants with traumatic brain injury.

BACKGROUND: Literature has compared the frequency of aggressive behaviors of the TBI population and the non-TBI population, suggesting that the TBI population is predisposed to aggressive tendencies because the injury enables impulsivity, loss of self-control, and the inability to modify behaviors. These behavior changes have consequently, been found to lead to criminal involvement. In fact, the majority of the prison population has sustained at least one TBI in their lifetime compared to the prevalence of brain injuries in the general population. However, there is little research investigatin

Patient and impatient punishers of free-riders.

Costly punishment of cheaters who contribute little or nothing to a cooperating group has been extensively studied, as an effective means to enforce cooperation. The prevailing view is that individuals use punishment to retaliate against transgressions of moral standards such as fairness or equity. However, there is much debate regarding the psychological underpinnings of costly punishment. Some authors suggest that costly punishment must be a product of humans' capacity for reasoning, self-control and long-term planning, whereas others argue that it is the result of an impulsive, present-orie

Competence as a predictor of sexual and reproductive health outcomes for youth: a systematic review.

To examine the association between "competence" and adolescent sexual and reproductive health (ASRH) outcomes. Competence refers to the development of skills to perform tasks successfully in four areas including social and behavioral, cognitive, emotional, and moral competence. We conducted a systematic review of research published from 1985 through 2007. Inclusion criteria included use of multivariate analyses, a sample size of >or=100, publication in a peer-reviewed journal, and measurement of an ASRH outcome. We coded findings as protective, risk, or no association and as longitudinal or cr

Ethical challenges in the care of older people and risk of being burned out among male nurses.

AIM: The aim of this study was to illuminate the meaning of being in ethically difficult situations when caring for older people, as experienced by male nurses. BACKGROUND: Nurses and physicians are frequently faced with ethical issues and challenges in their work with older people in hospitals. Male nurses are a minority group in the nursing profession, thus it is important to listen to their lived experiences of the ethical challenges they are faced with in their work. METHOD: The study is part of a comprehensive investigation into the narratives of female and male physicians and nurses, c

The ethical inadequacy of uninformed surrogate consent: advancing respect for persons in clinical research.

In clinical research, decision-making capacity is often equated with unspecified conceptions of autonomy, and autonomy is often equated with personhood. On this view, the loss of decision-making capacity is seen as a loss of autonomy, and the loss of autonomy subsumes a loss of personhood. An ethical concern arises at the intersection of those philosophical considerations with the legal considerations in informed consent. Because persons with inadequate decision-making capacity cannot provide legally effective consent, enrollment in research can occur only if a surrogate gives permission on th
