Wnt-β Catenin Signaling Pathway: A Major Player in the Injury Induced Fibrosis and Dysfunction of the External Anal Sphincter.

Wnt-β catenin is an important signaling pathway in the genesis of fibrosis in many organ systems. Our goal was to examine the role of Wnt pathway in the external anal sphincter (EAS) injury-related fibrosis and muscle dysfunction. New Zealand White female rabbits were subjected to surgical EAS myotomy and administered local injections of either a Wnt antagonist (sFRP-2; daily for 7 days) or saline. Anal canal pressure and EAS length-tension (L-T) were measured for 15 weeks after which the animals were sacrificed. Anal canal was harvested and processed for histochemical studies (Masson trichrom…

Pharyngeal dysphagia in inflammatory muscle diseases resulting from impaired suprahyoid musculature.

Dysphagia has previously been reported in the inflammatory myopathies (IMs): inclusion body myositis (IBM), dermatomyositis (DM), and polymyositis (PM). Patients report coughing, choking, and bolus sticking in the pharynx. Myotomy has been the treatment of choice, with variable success reported. We sought to determine underlying causes of dysphagia in IM patients using instrumental evaluation. Eighteen subjects participated in the study: four with DM, six with PM, and eight with IBM. They underwent simultaneous videofluoroscopy and manometry, yielding 214 swallows for analysis regarding functi…

Endoscopic management of spontaneous esophageal and postoperative motility disorders.

Considerable progress has been made in therapeutic endoscopy over recent years and new techniques now offer access to a new space, the submucosal space, thus paving the way for new therapeutic perspectives, in particular, for functional endoscopy. The first of these applications, POEM (Per-oral endoscopic myotomy), has become, since the beginning of the 2010's, an endoscopic alternative to Heller myotomy for the treatment of achalasia. The results of this technique have been fully documented, and the technique has replaced pneumatic dilatation and become the standard treatment for this indicat…

POEM, GPOEM, and ZPOEM.

Our tripartite narrative review discusses Peroral Endoscopic Myotomy (POEM), gastric POEM (GPOEM) and POEM for Zenker's diverticula (ZPOEM). POEM is the prototypical procedure that launched the novel "3rd space endoscopy" field of advanced endoscopy. It revolutionized achalasia therapy by offering a much less invasive version of the prior gold standard, the laparoscopic Heller myotomy (HM). We review in detail indications, outcomes, technique variations and comparative data between POEM and HM particularly with regard to the hotly debated issue of GERD. We then proceed to discuss two less illu…

Management of primary achalasia: The role of endoscopy.

Achalasia is an oesophageal motor disorder which leads to the functional obstruction of the lower oesophageal sphincter (LES) and is currently incurable. The main objective of all existing therapies is to achieve a reduction in the obstruction of the distal oesophagus in order to improve oesophageal transit, relieve the symptomatology, and prevent long-term complications. The most common treatments used are pneumatic dilation (PD) and laparoscopic Heller myotomy, which involves partial fundoplication with comparable short-term success rates. The most economic non-surgical therapy is PD, with b…

Surgery for Killian-Jamieson diverticulum: a report of two cases.

BACKGROUND: Killian-Jamieson diverticulum (KJD) is a rare diverticulum arising from a muscular gap in the anterolateral wall of the proximal … radiographic findings. She underwent diverticulectomy with cricopharyngeal and proximal esophageal myotomy. Staple line leakage developed at 1 month after surgery and was successfully treated conservatively. At 5 months after surgery, she was asymptomatic. Case 2 involved a 77-year-old woman presenting dysphagia for the past 2 years. She had a history of …

Esophageal motility disorder achalasia.

PURPOSE OF REVIEW: Although not a common disease, the last several years have had exciting breakthroughs in better defining the pathophysiology of achalasia, classifying the disease with high-resolution manometry and potentially exciting new treatments. RECENT FINDINGS: The introduction of high-resolution manometry with detailed assessment of lower esophageal sphincter function and peristalsis has made it possible to classify achalasia into three subtypes. This becomes clinically important as patients with type II achalasia do best with both pneumatic dilation and surgical myotomy, whereas ty…

[Esophagocardiomyotomy or modified Heller technique, through the thorax, for the treatment of achalasia].

BACKGROUND: Since 1913 surgical myotomy has been an effective method for patients with esophageal achalasia and until today there is controversy between pneumatic dilatation or surgery and between the abdominal or thoracic approach for Heller's procedure. AIMS: To find out the long-term results with thoracic short esophagomyotomy without an antireflux procedure for esophageal achalasia. METHODS: The charts of 31 patients with achalasia operated of thoracic Heller's myotomy between 1986 and 1995 were reviewed in retrospective fashion. RESULTS: Demographic and clinical data were similar to th…

Laparoscopic esophagomyotomy for achalasia.

Results of an ongoing clinical study treating achalasia patients with a transabdominal laparoscopic Heller myotomy and Toupet partial fundoplication are presented. Twelve patients underwent surgery between January 1992 and October 1993. All patients had barium esophagograms, preoperative endoscopy, esophageal manometry, 24-h pH studies, and extensive GI history preoperatively. Surgical complications included two perforations of the mucosa at the gastroesophageal junction repaired laparoscopically. There were no surgical mortalities and the average hospital stay was 39 h. Postoperatively all pa…

Laparoscopic Heller cardiomyotomy with intraoperative manometry in the management of oesophageal achalasia.

Results of an ongoing clinical study treating achalasia patients with laparoscopic Heller myotomy and Dor anterior fundoplication are presented. 18 patients underwent surgery between August 1991 and July 1995. Completeness of myotomies and calibration of fundoplications were measured using intraoperative manometry. Only one intraoperative complication, a perforation of the mucosa sutured laparoscopically, was encountered. There were no surgical mortalities nor postoperative morbidities. Mean hospital stay was 3.4 days. Complete relief of dysphagia and modifications of radiological and manometr…

Thoracoscopic Heller's cardiomyotomy: a new approach for achalasia.

The authors performed Heller's myotomy using an endothoracoscopic technique on two patients with swallowing difficulties that were clinically demonstrated to be oesophageal achalasia. At 12 and 18 months' followup, respectively, both patients were asymptomatic. Extramucosal cardiomyotomy, as reported by Heller in 1913, is the most common approach to repair achalasia of the oesophagus. The advantages of the laparoscopic technique (less pain, small scars, reduced hospital stay) are given, as well as the disadvantages (possible mobilization of left lateral liver segment, possible splenic damage, …

To minimize invasion - Is it possible to treat early esophageal cancer and achalasia within one operation?

BACKGROUND AND AIM: Patients with achalasia face an increased risk of dysplasia and squamous cell carcinoma due to chronic inflammation. We demonstrated the feasibility of performing endoscopic submucosal dissection (ESD) and well-designed peroral endoscopic myotomy (POEM) within a single operation, aiming to reduce trauma and enhance recovery. METHODS: A 65-year-old male with progressive dysphagia was diagnosed with type II achalasia and suspected esophageal lesions. Our treatment plan involved addressing both achalasia and the lesions using POEM and ESD during one operation. POEM was initia…

Treatment of Achalasia and Epiphrenic Diverticulum.

BACKGROUND: Epiphrenic diverticulum (ED) is a pulsion pseudodiverticulum found in the distal 10 cm of the esophagus. Motility disorders are … atients. Therapeutic options include laparoscopic, thoracoscopic and endoscopic procedures. While a myotomy must always be performed, diverticulectomy is not always necessary. CONCLUSIONS: Epiphrenic diverticulum is a rare condition whose pathophysiology involves an underlying motility disorder-achalasia in most cases. Symptoms usually include dysphagia, …

Zenker's diverticulum: exploring treatment options.

Zenker's diverticulum is an acquired sac-like outpouching of the mucosa and submucosa layers located dorsally at the pharyngoesophageal junction through Killian's dehiscence. It is the most common type of oesophageal diverticula with a reported prevalence ranging between 0.01 to 0.11% and typically occurs in middle-aged and elderly patients. Predominant symptoms are dysphagia and regurgitation. Treatment is recommended for symptomatic patients and considering the aetiopathogenesis of the disease demands myotomy of the cricopharyngeal muscle. Myotomy may be pursued through either open surgical …

[Evaluation of esophageal function in surgically-treated patients].

The authors report on their experience acquired in the surgical treatment of functional esophageal disease (achalasia, diffuse esophageal spasm, diverticula and gastroesophageal reflux). The authors affirm that a correct and meticulous functional study of the esophagus is fundamental for the adoption of precise surgical plotting, in order to circumvent all of the complications and failures described in Literature. Such an objective can be achieved intraoperatively thanks to myotomy and fundoplication with the aid of intraoperative manometry (IEM) that, when performed in the course of myotomy, …

Ergonomic laparoscopic spreader for infantile hypertrophic pyloric stenosis.

BACKGROUND: Laparoscopic pyloromyotomy (LP) for Infantile Hypertrophic Pyloric Stenosis (IHPS) is associated with an increased risk of mucosal tears or incomplete myotomy when compared with open myotomy. Keeping these risks in mind, a new ergonomically designed spreader for LP was developed and investigated in IHPS. METHODS: The new Pyloric Spreader was designed as a 3.5 mm instrument specifically for minimal access application taking into consideration the specific needs of LP. RESULTS: LP was performed using this spreader in 25 infants with IHPS during a 5-year period from 2006-2012. After…

Surgical treatment of a severe, massive, symptomatic Zenker's diverticulum in a very elderly patient.

Zenker's diverticula commonly occur in the elderly, and quality of life is often impaired by typical symptoms such as dysphagia, regurgitation, halitosis, cough and aspiration pneumonia, malnutrition and weight loss. The "gold standard" treatment for pharyngo-oesophageal diverticula is the resection of the sac via left lateral cervicotomy and cricopharingeal myotomy. In the last decade, with the fast development of minimally invasive techniques, an endoscopic stapled approach has been proposed. This procedure rapidly encountered the favour of gastroenterologists because patients with Zenker's …
