Predicting Critical Outcomes in Suspected Cardiopulmonary Emergencies Using Dispatch Narratives: Temporal Validation Study.

BACKGROUND: Early risk stratification in emergency medical services (EMS) is essential for patients presenting with acute cardiopulmonary symptoms, yet prehospital decision-making at the dispatch stage is often based on limited structured information. Free-text dispatch narratives may contain additional clinical signals, but their role in early risk assessment remains insufficiently characterized. OBJECTIVE: This study aims to develop and temporally validate a natural language processing-assisted machine learning framework for early risk stratification using free-text EMS dispatch narratives

Caregiver speaks: a randomized controlled trial of a storytelling intervention for hospice family caregivers of persons with dementia.

BACKGROUND AND OBJECTIVES: Family caregivers of persons living with dementia experience significant emotional distress during end-of-life care and bereavement. Interventions supporting meaning-making and social connection may help mitigate anxiety and depression, and support caregivers' adjustment during bereavement. RESEARCH DESIGN AND METHODS: This randomized controlled trial evaluated Caregiver Speaks, a storytelling intervention for family caregivers of persons living with dementia. Participants from 5 hospices in the Midwestern and Northeastern United States were randomized 1:1 to interv

Attenuated symptoms are associated with connectedness and emotional expression in narratives based on emotional pictures in a Brazilian clinical high-risk cohort.

Natural language processing tools have been applied to identify symptoms through the analysis of narratives. Here we use this strategy to map attenuated symptoms when applied to prodromal stages of psychosis. We assessed narratives in a population-based prodromal screen-positive Brazilian cohort of individuals confirmed through structured interview as at clinical high-risk for psychosis (CHR) (N = 42) or as controls (N = 29) to identify language markers predictive of transition to diagnosis or remission. Participants were identified by screening >4500 individuals with the Prodromal Questionnai

Differences in Narrative Discourse Ability in Traumatic Brain Injury and a Neurotypical Comparison Group: Influence of Age, Education, and Sex.

OBJECTIVE: To examine the influence of traumatic brain injury (TBI) on narrative discourse comprehension and production of individuals with TBI compared with a non-brain-injured (NBI) group. Furthermore, this work investigated the potential influence of sociodemographic factors (ie, age, education, sex) on narrative discourse performance. DESIGN: Case-control design. SETTING: General community. PARTICIPANTS: A total of 44 adults (N=44) were grouped into 2 age- and education-matched cohorts. The TBI group comprised 21 participants with mild-moderate cognitive impairment, and the NBI comprise

Narratives of Disconnection: A Life Course Perspective of Methamphetamine Use Among Sexual Minority Men Living with HIV.

BACKGROUND: Sexual minority men (SMM) are exposed to societal and structural stressors that translate into poor health outcomes. One such outcome is substance use, which research has long documented as a prominent disparity among SMM. Methamphetamine is a particularly deleterious substance for SMM because its use is often framed as a coping response to social and structural stressors. METHOD: Guided by stress and coping theory and a life course perspective, the purpose of this qualitative study is to assess the development of coping strategies in the context of prominent social and structural

The Changing Care of Older Adults With Bipolar Disorder: A Narrative Analysis.

Older adults with bipolar disorder experience distinct challenges compared to younger age groups with bipolar disorder. They potentially require adaptations to the care they receive. This study aimed to explore experiences of care and changing care needs in older adults with bipolar disorder. People with bipolar disorder (aged ≥60) were recruited through three NHS Trusts in the North West of England, charity organisations, a confidential university participant database, and social media. Participants completed single time-point biographical narrative interviews, which were analysed using narra

Contesting Cannabinoid Hyperemesis Syndrome: How Narratives of Cannabis Shape Diagnosis Contestation and Treatment Resistance.

Cannabinoid hyperemesis syndrome (CHS) is characterized by the onset of cyclic bouts of severe nausea and vomiting in chronic cannabis users. As the number of CHS diagnoses rises, it is important to understand how people experience the disease. Using a narrative framework, we explore how the symbolic meaning participants associated with cannabis shaped the way they experienced diagnosis and treatment of CHS. To do this, we relied on semi-structured interviews with 24 people who self-reported having CHS. Participants had very positive perceptions of cannabis, which contributed to them contestin

Narrative Force: How Science and Storytelling Impact Parental Concussion Beliefs and Intentions Through Transportation and Trust.

Although news and entertainment coverage of sports concussions has increased in recent years, many parents of adolescent athletes remain indifferent to or unaware of practices that would mitigate concussion harm. This experiment with U.S. parents of 10- to 17-year-olds (N = 502) explores how narratives and expository concussion science could be used together to increase concussion risk assessments, mitigation intentions, and support for sports concussion policy. Direct and indirect relationships through narrative transportation and trust are explored.

Automatic Assessment of Connected Speech and Its Relation to Memory in Alzheimer's Disease in Low Education.

INTRODUCTION: Discourse is one of the main linguistic aspects affected by Alzheimer's disease (AD), and its relationship with memory needs to be further studied, mainly in low education and low socioeconomic status groups. The present study aimed to investigate differences in the recall of short narratives between participants with mild AD and a control group (CG) of typical older adults (CG) with the use of automatic assessment. METHODS: Seventeen older adults diagnosed with AD (mean age 76.41, mean education 5.82) and 34 typical older adults (mean age 74.26, mean education 7.09) were asked

Silent voices of the midwives: factors that influence midwives' achievement of successful neonatal resuscitation in sub-Saharan Africa: a narrative inquiry.

BACKGROUND: In Tanzania, birth asphyxia is a leading cause of neonatal death. The aim of this study was to identify factors that influence successful neonatal resuscitation to inform clinical practice and reduce the incidence of very early neonatal death (death within 24 h of delivery). METHODS: This was a qualitative narrative inquiry study utilizing the 32 consolidated criteria for reporting qualitative research (COREQ). Audio-recorded, semistructured, individual interviews with midwives were conducted. Thematic analysis was applied to identify themes. RESULTS: Thematic analysis of the mid

Sharing the Story of the Cardiac Rehab Patient Experience: A QUALITATIVE STUDY.

PURPOSE: To provide a prototypical patient narrative of the cardiac rehabilitation (CR) experience for providers and prospective patients using narrative analysis. METHODS: Qualitative interviews with 17 CR patients from a previous study regarding their experiences, reasons, and motivations related to engagement in CR were analyzed using narrative inquiry. Interviews were previously analyzed and coded for recurring themes, and these themes were implemented in an exploratory narrative inquiry to craft a CR patient "story." A hypothetical composite character representing the varied experiences

Women's Experiences with Neuraxial Labor Analgesia in the Listening to Mothers II Survey: A Content Analysis of Open-Ended Responses.

BACKGROUND: Most women who give birth in United States hospitals receive neuraxial analgesia to manage pain during labor. In this analysis, we examined themes of the patient experience of neuraxial analgesia among a national sample of U.S. mothers. METHODS: Data are from the Listening to Mothers II survey, conducted among a national sample of women who delivered a singleton baby in a U.S. hospital in 2005 (N = 1,573). Our study population consisted of women who experienced labor, did not deliver by planned cesarean, and who reported neuraxial analgesia use (n = 914). We analyzed open-ended re

Narratives of continuity among older people with late stage chronic kidney disease who decline dialysis.

Chronic and life-threatening conditions are widely thought to shatter the lives of those affected. In this article, we examine the accounts of 19 older people diagnosed with late stage chronic kidney disease who declined dialysis. Accounts were collected through in-depth interview in the United Kingdom (March-November, 2010). Drawing on a phenomenological approach, we focus particularly on the embodied and lived experience of the condition and on how participants constructed treatment modalities and approached treatment choice. We look toward contemporary elaborations of the conceptual framewo

Family management of a chronic health condition: perspectives of adolescents.

A growing number of adolescents and their families are dealing with a chronic health condition that impacts their daily life. Research using the Family Management Style Framework (FMSF) has added much to our understanding of how the family integrates chronic condition management into family life. Less clear, however, is the adolescent perspective of the FMSF components. The purpose of this secondary qualitative analysis was to explore 25 interviews of adolescents with spina bifida to uncover the adolescent's perspective of the three major FMSF components: Definition of the Situation, Managemen

Will words ever harm me?: escalation from verbal to physical abuse in sixth-grade classrooms.

Based on in-depth qualitative interviews, this article gives the opportunity to 145 sixth graders to tell, in their own words, how they felt and reacted when verbally and emotionally abused by their classmates. Content analysis of interviews revealed gender differences in students' reactions to verbal aggression. Another interesting finding was the differentiation between verbally aggressive messages meant to be for fun from those that were not so meant based on (1) contextual cues, (2) other peers' reaction, and (3) the malleability of the personal features that were the topic of the attack.

A narrative approach to the evaluation of supportive housing: stories of homeless people who have experienced serious mental illness.

We present the findings of a narrative approach to the evaluation of supportive housing for formerly homeless people who have experienced serious mental illness. According to the accounts of 11 men and 9 women, their youth and adult years were filled with personal problems, troubled relationships, and a lack of adequate social resources. Since entering supportive housing, participants noted more stability in their lives and the beginning of journeys to recover positive personal identities, restore or develop new supportive relationships, and reclaim resources vital to leading lives with dignit
