The effect of vitamin D and calcium supplementation in pediatric steroid-sensitive nephrotic syndrome.

BACKGROUND: Low serum levels of total 25-hydroxycholecalciferol (25(OH)D) occur in nephrotic syndrome (NS). We aimed to assess the effects of vitamin D3 and calcium supplementation on 25(OH)D levels, bone mineralization, and NS relapse rate in children with steroid-sensitive NS. METHODS: A randomized controlled trial (RCT) was performed in children with steroid-sensitive NS. The treatment group received vitamin D3 (60,000 IU orally, weekly for 4 weeks) and calcium supplements (500 to 1,000 mg/day for 3 months) after achieving NS remission. Blood samples for bone biochemistry were taken during

Diagnostic and therapeutic approaches in patients with secondary hyperoxaluria.

Secondary hyperoxaluria is due either to increased intestinal oxalate absorption or to excessive dietary oxalate intake. Certain intestinal diseases like short bowel syndrome, chronic inflammatory bowel disease or cystic fibrosis and other malabsorption syndromes are known to increase the risk of secondary hyperoxaluria. Although the urinary oxalate excretion is usually lower than in primary hyperoxaluria, it may still lead to significant morbidity by recurrent urolithiasis or progressive nephrocalcinosis. A clear distinction between primary and secondary hyperoxalurias is important. As correc

The European distal renal tubular acidosis registry: a 5-year analysis.

BACKGROUND: Distal renal tubular acidosis (dRTA) is characterized by metabolic acidosis, growth failure, nephrocalcinosis, nephrolithiasis and chronic kidney disease (CKD). Previous retrospective data suggested improved outcome with adequate metabolic control, but confirmation from prospective data is lacking. Since 2019, the European dRTA registry has collected prospective data on patients with dRTA. Herein we present results of the first data analysis. METHODS: The registry is hosted as a subregistry of the European Rare Kidney Disease Network (https://www.erknet.org/). In addition to the s

Chest radiographic findings suggestive of possible sarcoidosis in patients with nephrolithiasis.

Renal involvement in sarcoidosis may manifest as hypercalcemia, hypercalciuria, nephrolithiasis, nephrocalcinosis, and other abnormalities, but is often underrecognized. This study investigated chest radiographic findings suggestive of possible sarcoidosis in patients with nephrolithiasis and compared available laboratory parameters between patients with and without such findings. This retrospective analytical cross-sectional study included patients undergoing PCNL or TUL for urinary stones at Razi Hospital (Guilan, Iran) from 2016 to 2021. Routine preoperative chest X-rays were reviewed; 1297

Thiazide therapy in chronic hypoparathyroidism: effects on hypercalciuria and renal function-a systematic review and exploratory meta-analysis.

PURPOSE: Chronic hypoparathyroidism is associated with impaired renal calcium reabsorption, resulting in persistent hypercalciuria and increased risk of nephrolithiasis, nephrocalcinosis, and chronic kidney disease. Thiazide diuretics are commonly used to reduce urinary calcium excretion; however, evidence regarding their efficacy and renal safety remains limited. This study aimed to evaluate the effects of thiazide therapy on hypercalciuria and renal outcomes in patients with chronic hypoparathyroidism. METHODS: A systematic search of PubMed/MEDLINE, Embase, Scopus, and Cochrane CENTRAL was

Recurrent Infection-Triggered Immune Thrombocytopenia Flares in Medullary Sponge Kidney: A Case Report.

Medullary sponge kidney (MSK) is a congenital collecting duct abnormality associated with nephrocalcinosis, recurrent nephrolithiasis, hematuria, urinary tract infections, and obstructive urological complications. Immune thrombocytopenia (ITP) is an acquired autoimmune disorder characterized by isolated thrombocytopenia and bleeding risk. We report a case illustrating how these two chronic conditions can interact indirectly to create a recurrent management dilemma. A 45-year-old woman with bilateral MSK and chronic ITP controlled with weekly romiplostim had a baseline platelet count of approxi

Human glyoxylate metabolism revisited: New insights pointing to multi-organ involvement with implications for siRNA-based therapies in primary hyperoxaluria.

Glyoxylate is a toxic metabolite because of its rapid conversion into oxalate, as catalyzed by the ubiquitous enzyme lactate dehydrogenase. This requires the presence of efficient glyoxylate detoxification systems in multiple subcellular compartments, as glyoxylate is produced in peroxisomes, mitochondria, and the cytosol. Alanine glyoxylate aminotransferase (AGT) and glyoxylate reductase/hydroxypyruvate reductase (GRHPR) are the key enzymes involved in glyoxylate detoxification. Bi-allelic mutations in the genes coding for these enzymes cause primary hyperoxaluria type 1 (PH1) and 2 (PH2), re

Long-term outcome of Bartter syndrome in 54 patients: A multicenter study in Korea.

INTRODUCTION: Bartter syndrome (BS) is a rare salt-wasting tubulopathy caused by mutations in genes encoding sodium, potassium, or chloride transporters of the thick ascending limb of the loop of Henle and/or the distal convoluted tubule of the kidney. BS is characterized by polyuria, failure to thrive, hypokalemia, metabolic alkalosis, hyperreninemia, and hyperaldosteronism. Potassium and/or sodium supplements, potassium-sparing diuretics, and nonsteroidal anti-inflammatory drugs can be used to treat BS. While its symptoms and initial management are relatively well known, long-term outcomes a

Recombinant human parathyroid hormone (1-84) replacement therapy in a child with hypoparathyroidism.

First line conventional therapy of hypoparathyroidism comprises oral calcium and active vitamin D analogues. This approach may fail to correct hypocalcemia and hyperphosphatemia caused by the absence of parathyroid hormone and carries the risk of long-term complications including ectopic calcifications and renal damage. Full-length recombinant human parathyroid hormone (rhPTH[1-84]) is approved for the treatment of hypoparathyroidism in adults refractory to conventional therapy. To date, there is no data in children. Here, we report the successful use of rhPTH(1-84) in a 5-year old girl with h

Assessment of Urine Proteomics in Type 1 Primary Hyperoxaluria.

BACKGROUND: Primary hyperoxaluria type 1 (PH1) and idiopathic hypercalciuria (IHC) are stone-forming diseases that may result in the formation of calcium (Ca) oxalate (Ox) stones, nephrocalcinosis, and progressive chronic kidney disease (CKD). Poorer clinical outcome in PH1 is segregated by the highest urine (Ur)-Ox (UrOx), while IHC outcomes are not predictable by UrCa. We hypothesized that differences would be found in selected Ur-protein (PRO) patterns in PH1 and IHC, compared to healthy intra-familial sibling controls (C) of PH1 patients. We also hypothesized that the PRO patterns associat
