The effect of a muscle wrap on nerve healing in a rat model.

Sixty Sprague-Dawley rats were studied to determine if peripheral nerve healing could be enhanced by translocation of an injured segment of sciatic nerve from an interfascial to an intramuscular environment. Group 1 (n = 12) were unoperated controls; Group 2 (n = 12) were sham-operated; Group 3 (n = 12) sustained an acute, double-crush injury to the sciatic nerve; and Group 4 (n = 12) sustained the same acute, double-crush injury as in Group 3, but the injured segment of nerve was transposed intramuscularly. All animals had sciatic function indices (SFI) computed at weekly intervals. At 2 week

A kinematic theory of rapid human movements. Part I. Movement representation and generation.

This paper proposes a kinematic theory that can be used to study and analyze rapid human movements. It describes a synergy in terms of the agonist and antagonist neuromuscular systems involved in the production of these movements. It is shown that these systems have a log-normal impulse response that results from the limiting behavior of a large number of interdependent neuromuscular networks, as predicted by the central limit theorem. The delta log-normal law that follows from this model is very general and can reproduce almost perfectly the complete velocity patterns of an end-effector. The

Site of autonomic dysfunction in a patient with Ross' syndrome and postganglionic Horner's syndrome.

Ross' syndrome is a rare peripheral nervous system disorder defined by Adie's tonic pupil, hyporeflexia, and segmental anhidrosis. Injury to postganglionic cholinergic fibers is believed to account for the tonic pupil and sweating disturbance. We report a 47-year-old man found to have Ross' syndrome in combination with a complete postganglionic Horner's syndrome. Pharmacologic and sudomotor tests in this unique patient provide further evidence that Ross' syndrome results from injury to sympathetic and parasympathetic ganglion cells or to their postganglionic projections.

Paradoxical clinical consequences of peripheral nerve injury: conduction of nerve impulses does not occur across the site of injury immediately following nerve division and repair.

The occasional apparent clinical phenomenon of the immediate although transient return of peripheral nerve function after nerve division and primary repair has been previously reported. Electrophysiological findings from the sciatic nerve of the rabbit have been previously presented in support of the concept of the transmission of nerve impulses across a freshly divided and repaired peripheral nerve for a short period until the onset of Wallerian degeneration. This paper presents experimental evidence to show that these findings were misinterpreted and that a compound action potential cannot b

High prevalence of undetected carpal tunnel syndrome in patients with fibromyalgia syndrome.

OBJECTIVE: To assess the prevalence of carpal tunnel syndrome (CTS) in patients with fibromyalgia syndrome (FM). METHODS: A series of 206 consecutive patients with FM was evaluated for CTS in an unblinded, uncontrolled study. Sixty patients with dermatomal paresthesia in the fingers enervated by the median nerve were evaluated with electromyogram and nerve conduction velocity studies (EMG/NCV). RESULTS: Thirty-three patients showed EMG/NCV findings diagnostic for CTS (16% of the 206 cases, 55% of the 60 EMG studies). Only 2 of the 33 patients with CTS had been diagnosed before rheumatologic

Charcot-Marie-Tooth disease.

OBJECTIVE: To discuss the similarities and differences in the clinical presentation of Charcot-Marie-Tooth Disease, an inherited peripheral neuropathy, and acquired lumbar spinal stenosis. CLINICAL FEATURES: Patients with lumbar spinal stenosis causing nerve root entrapment often have leg pain and weakness during such activities as walking or standing. Additional symptoms of poor balance, foot deformity and signs of cerebellar and sensory ataxia suggest a diagnosis of peripheral neuropathy rather than nerve root entrapment. INTERVENTION: Electrodiagnostic testing and nerve conduction studies

Vibrameter testing facilitates the diagnosis of uremic and alcoholic polyneuropathy.

The diagnostic sensitivity of Vibrameter and tuning fork examination towards uremic and alcoholic neuropathy was tested in 75 patients. In 40 uremic and 35 alcoholic patients, we compared the sensitivity of neurological examination, nerve conduction studies (NCS) and vibration thresholds assessed at the malleoli by means of Vibrameter and scaled tuning fork. Vibrameter results were correlated with NCS. Polyneuropathy was diagnosed in 52 patients, but in 16 patients diagnosis depended upon inclusion of Vibrameter testing in the examination protocol. In uremic patients, Vibrameter (47.5%) showed

Fundamental aspects of neural-immune signaling.

Noradrenergic and peptidergic nerve fibers abundantly innervate the parenchyma of both primary (bone marrow) and secondary (spleen, lymph nodes) lymphoid organs. Nerve fibers distribute within the parenchyma of these organs, as well as along smooth muscle compartments. Both noradrenaline and peptides such as substance P have been shown to fulfill the basic criteria for neurotransmission with lymphocytes, macrophages, and other immunocytes as targets. Denervation or pharmacological manipulation of these neurotransmitters can profoundly alter immunological reactivity at the individual cellular l

Polyneuropathy associated with IgA monoclonal gammopathy of undetermined significance.

Although polyneuropathies associated with IgM and IgG monoclonal gammopathies have been well described, polyneuropathy with IgA monoclonal gammopathy of undetermined significance (MGUS) is less commonly seen and has not been well studied. We reviewed the clinical and electrodiagnostic features of 5 such patients, and the sural nerve biopsy findings in 4 of them. One patient was diabetic, while 4 were free of other diagnoses commonly associated with neuropathy. Clinical presentations were varied. Electrodiagnostic and histological studies ranged from primary demyelination to primary axon loss t

Peripheral nerve elongation with tissue expansion techniques.

Elongation of peripheral nerves would facilitate the repair of peripheral nerve injuries. The purpose of this study was to determine the efficacy of nerve expansion and the effect of different levels of intraluminal expander pressure upon nerve conduction. A 2-mL Silastic expander was placed under one sciatic nerve of adult male rats. The other nerve served as a control. Electroneuromyography (ENMG) was done at the time of initial expansion in seven animals. At 40 mm Hg expansion pressure a significant change in latency was found with an increase of 17% +/- 5%. Expansion was carried out for 14

Effects of acute graded strain on efferent conduction properties in the rabbit tibial nerve.

This study examines effects of acute stretch on electrophysiologic properties of rabbit tibial nerve. Compound motor action potentials (CMAP) were measured in 30 rabbits, during and after two hours of acute, graded strain. The rabbits were assigned to one of three groups defined by the degree of stretch (0%, 8%, or 15%) beyond original resting length. These groups were subdivided according to ankle position during nerve strain (ankle dorsiflexion or ankle plantar flexion). Two hours of 8% strain did not significantly decrease CMAP amplitude. Fifteen percent strain with the ankle in dorsiflexio

[A case of intercostal neuropathy (Th 2-9) due to sarcoidosis].

Neurosarcoidosis is a disorder which presents in 5 percent of cases of general sarcoidosis, in only 10% of which the peripheral nerves are involved. A patient who complained of numbness of the abdomen and back associated with paralytic motion of intestines was admitted. The patient was diagnosed as having neurosarcoidosis of the intercostal nerves (Th2-9) on the basis of abnormality of somatosensory evoked potentials (SEP) and reduced conduction velocity, and evident disease of intrathoracic lesions confirmed by biopsy. Corticosteroid was administered, with relief of symptoms and active respon

[On the mechanisms and diagnosis of conduction disturbances due to demyelination with special reference to multifocal demyelinating neuropathy (Lewis-Sumner)].

Multifocal demyelinating neuropathy with persistent conduction block can mimic motor neuron disease, but is potentially reversible. Its diagnosis rests upon electrophysiological demonstration of focal conduction block at multiple sites. Conduction block is the most important mechanism causing clinical symptoms in peripheral nerve demyelination. On the other hand, conduction slowing is not always associated with clinical symptoms. In 2 out of 9 patients with multifocal demyelinating motor neuropathy, MRI showed focal swelling of the nerve at the site of conduction block. Both of them had elevat

Lower extremity motor evoked potentials in multiple sclerosis.

Transcranial magnetic stimulation was performed on 25 patients with definite multiple sclerosis. Motor evoked potentials were recorded from the anterior tibial muscle. Central motor conduction time was calculated using the equation (F + M-1)/2 by stimulation of the common peroneal nerve. Motor evoked potentials were capable of detecting subclinical pyramidal tract lesions in multiple sclerosis. In patients with multiple sclerosis, the incidence of abnormality of motor and somatosensory evoked potentials was similar. Central motor conduction time was correlated with overall and pyramidal tract

Invited review: peripheral neuropathy in Sjogren's syndrome.

Our experience and review of the literature reveal that Sjogren's syndrome (SS) is an important, poorly recognized cause of peripheral neuropathy. Several forms of peripheral nerve dysfunction occur in SS including trigeminal sensory neuropathy, mononeuropathy multiplex, distal sensory neuropathy, distal sensorimotor peripheral neuropathy and a pure sensory neuronopathy syndrome. Rarely, chronic relapsing inflammatory polyneuropathy and multiple cranial neuropathies appear. Clinical evidence of glandular involvement is often minimal or absent when patients with SS develop peripheral neuropathy

[Amyloid neuropathy].

Primary amyloidosis and myeloma associated amyloidosis causes neuropathy in 10% of the cases, and hemodialysis associated amyloidosis causes carpal tunnel syndrome. However, most severe amyloid neuropathy is observed in familial amyloidotic polyneuropathy (FAP). FAP type I is an autosomal dominant systemic amyloidosis characterized by sensory dominant systemic amyloidosis characterized by dissociated sensory disturbance and autonomic dysfunction. Amyloid deposition is noted universally in endoneurium of peripheral nerve, especially prominent in sural, sciatic nerve, dorsal root ganglia and sym

A case of left hypoglossal neurapraxia following G exposure in a centrifuge.

Isolated hypoglossal (XIIth cranial nerve) neurapraxia is a rare clinical presentation. We present a case of an isolated hypoglossal neurapraxia following exposure to +7.2 Gz in a human centrifuge. Although a variety of cervical spine injuries have been reported as a result of G exposure in an aircraft, no cranial nerve injuries have been reported. A review of the literature of isolated hypoglossal neurapraxia is presented with discussion of the probable cause of this nerve injury.

Neuralgic amyotrophy with involvement of cranial nerves IX, X, XI and XII.

We report a patient who presented with involvement of multiple cranial nerves associated with otherwise typical neuralgic amyotrophy. This syndrome of unknown etiology is not limited to the brachial plexus. Simultaneous involvement of cranial nerves IX, X, XI, and XII is a unique presentation. The electrophysiological data indicate the presence of a multifocal neuropathy.
