Use of a Large Tenaculum Clamp as a Reduction Technique for Treatment of Distal Radius Fractures.

Distal radius fractures are common upper extremity injuries requiring surgical treatment. In the context of management with a volar locking plate (VLP), a number of described techniques assist with restoration of individual anatomical parameters such as radial length, volar tilt, and articular congruity. We present a surgical technique that utilizes a large tenaculum bone clamp to provide an efficacious reduction in several planes. With anteroposterior compression, the clamp enables volar translation of the distal fracture fragment. This compression also decreases the interval between the dist

Evidence-Based Medicine in Facial Trauma.

This article provides the reader with a comprehensive review of high-level evidence-based medicine in facial trauma and highlights areas devoid of high-level evidence. The article is organized in the order one might approach a clinical problem: starting with the workup, followed by treatment considerations, operative decisions, and postoperative treatments. Individual injuries are discussed within each section, with an overview of the available high-level clinical evidence. This article not only provides a quick reference for the facial traumatologist, but also allows the reader to identify ar

Capsulorrhaphy using suture anchors in open reduction of developmental dislocation of hip: technical note.

We describe a new technique of using titanium alloy anchors to perform internally rotating capsulorrhaphy during open reduction and stabilization procedure and obliteration of pseudoacetabulum when one exists. We routinely use anterior approach via bikini incision (modified Smith-Peterson approach) to perform open reduction and internally rotating capsulorrhaphy. We describe the technique of using barbed titanium alloy anchors preloaded with nonabsorbable sutures and two needles, to accomplish a secure and easy capsulorrhaphy, in selected cases. We have used this technique in 21 hips, since 20

Irreducible Phalangeal Fractures Due to Incarceration into the Extensor Tendon Apparatus - A Report of Two Patients.

Irreducible phalangeal fractures are uncommon, and if neglected can cause fracture non-union or functional deficits of the affected digit. We present two patients with irreducible phalangeal fractures - one in a child with a juxta-epiphyseal middle phalanx fracture incarcerated into the triangular ligament of the extensor apparatus and another in an adult following incarceration of a fractured proximal phalanx into the common intrinsic tendon on the radial side. Both required open reduction to extricate the incarcerated fracture ends, achieving good union and excellent functional outcomes. Lev

Treatment of missed Monteggia fracture with intact annular ligament after an interval of 9 years: a case report and literature review.

OBJECTIVE: Missed Monteggia fractures are commonly observed among children. Both the interval from the injury to surgery and the patient's age at operation are thought to be correlated directly with the success of the surgery and prognosis. The aim of the present work was to report one interesting adult case of a missed Monteggia fracture with an intact annular ligament 9 years after injury and the outcomes at a 7-year follow-up. CASE DESCRIPTION: One missed lesion with a 9-year delay for surgery occurred in a skeletally mature individual, and it was treated by open reduction and ulnar angula

Mandibular Body Fractures.

Fractures of the mandibular body most commonly occur after interpersonal violence or motorized vehicle accident but can occur in athletes. Mandibular body fractures are often associated with additional mandibular fractures. The treatment goal is to achieve preinjury occlusion and facial appearance, and this can be done via a closed reduction and maxillomandibular fixation or open reduction and fixation with or without maxillomandibular fixation. The authors present 3 cases in this article.

Revision Strategies for the Aseptic, Malaligned, Surgically Treated Ankle Fracture.

Although surgical fixation can take many forms depending on ankle fracture morphology, the goals of open reduction internal fixation are to restore fracture alignment, re-establish ankle stability, and achieve an anatomic mortise. A subset of patients may present postoperatively with evidence of suboptimal open reduction internal fixation. Increased contact pressures across the tibiotalar joint, increased talar shift, and pathologic joint loading leading to the development of post-traumatic arthritis are common sequela of the malaligned mortise. Treatment necessitates a comprehensive approach.

Bilateral acetabular fracture secondary to high-velocity trauma: A rare case report.

Bilateral acetabular fractures have long been described in seizure patients, osteoporotic acetabular insufficiency. Few cases have been reported in the context of high-velocity trauma. The bilateral traumatic acetabular fracture is very rare and complex due to the unique nature of the force and the fracture pattern. A 48-year-old female patient was admitted to our orthopedic ward for bilateral acetabular fracture after a road traffic accident. On admission, the patient presented with some associated complications susceptible to influence the prognosis. In conclusion, bilateral acetabular fract

Anterior Compartment Neurovascular Bundle Entrapment in a Minimally Displaced Distal Tibial Fracture: A Case Report.

CASE: A 29-year-old woman presented with a low-energy, minimally displaced pilon fracture with progressive pain and paresthesias in the affected foot, ultimately requiring open reduction and internal fixation. Intraoperatively, the deep peroneal nerve and anterior tibial artery and vein were entrapped within the fracture. After freeing the bundle and fixing the fracture, the paresthesias improved and ultimately resolved. CONCLUSION: Injury to the anterior leg compartment neurovascular structures should be considered in low-energy, minimally displaced pilon fractures. In this instance, progres

Hip Arthroscopic Reduction and Fixation for Adolescent Acetabular Posterior Wall Fracture: A Case Report.

BACKGROUND: Posterior wall fracture is the most common type of acetabular fracture, the traditional open reduction and fixation through the Kocher-Langenbeck approach required a large incision and extensive muscle and soft tissue dissection, resulting in more blood loss, more complications and delayed recovery after the operation. Hip arthroscopy has been widely used in clinical practice but rarely reported in acetabular fractures. CASE PRESENTATION: We present the case of a 14-year-old boy with acetabular posterior wall fracture who was treated with hip arthroscopy reduction and fixation usi

Complications of Low-Profile Plate Fixation of Phalanx Fractures.

Background: Phalanx fractures are common, and plate fixation can be used to treat difficult fractures. Major complications have been reported in up to 64% of phalanx fractures treated with plate fixation, with stiffness being the most common. Low-profile anatomic plates (LPAP) have been designed to decrease soft tissue irritation and postoperative stiffness. The objective of this study was to determine whether the use of LPAP has decreased complications in plate fixation of phalanx fractures. Methods: A retrospective chart review was performed of patients with phalanx fractures treated with op

Femoral Head Vascularity: Implications Following Trauma and Surgery About the Hip.

Traumatic injury and surgical intervention about the hip joint place the arterial supply to the femoral head (FH) at risk. Compromised perfusion may lead to FH ischemia, cell death, and osteonecrosis. Progression to FH collapse may lead to pain, functional impairment, and decreased quality of life, especially in younger patients. This review describes the arterial supply to the FH, analyzes the impact of femoral neck fractures on FH vascularity, and explores the vascular implications of various surgical interventions about the hip, offering specific techniques to minimize iatrogenic damage to

Mandibular Condylar Fractures in Children: Morphofunctional Results After Treatment With External Fixation.

Best treatment for paediatric patients with mandibular condylar fractures is still debated, and many approaches have been proposed. Closed treatment is the most used, but there is evidence showing long-term problems in patients with dislocated fractures. On the other hand, there are concerns on surgical treatment with open reduction internal fixation, particularly on the implanted hardware during growth. The aim of this study is to evaluate the long-term outcomes of paediatric patients treated surgically with external fixation. A total of 21 paediatric patients were treated. Diagnostic procedu
