PURPOSE: This database study aims to investigate the incidence and prevalence of optic neuritis (ON) among patients with different types of uveitis.
METHODS: A retrospective analysis of the Truven Health MarketScan® database from 2000 to 2014 was conducted. Patients with uveitis were followed until diagnosis of ON or until they were censored. Diagnosis of uveitis and demyelinating diseases of the central nervous system (DD) including ON were based on ICD-9 codes. Patients with a diagnosis of DD at or before the index date of diagnosis of uveitis were excluded from incidence calculation. In th…

PURPOSE: To determine the concentration of microelements in the blood of patients with a history of retrobulbar optic neuritis (ON).
MATERIALS AND METHODS: We examined 36 patients (18-63 years of age) - 11 with demyelinating ON (Group 1) and 25 with isolated ON (Group 2) - all of whom had been treated for retrobulbar ON. The control group comprised 38 healthy volunteers. Using atomic absorption spectroscopy, blood levels of zinc (Zn), iron (Fe), copper (Cu) and cadmium (Cd) were evaluated.
RESULTS: Compared with the control group, concentrations of Cd were elevated in Groups 1 and 2 (p = 0.0…

Here we report a case of optic neuritis in the setting of herpes zoster ophthalmicus (HZO) in a child. A six-year-old girl presented with HZO in the right eye. During the hospitalization, her visual acuity decreased. Fluorescein angiography (FAG) and optical coherence tomography revealed optic neuritis in the affected eye. Visual acuity improved with one month of treatment with acyclovir and steroids. FAG analysis showed no evidence of leakage at the optic disc. At one year post treatment, the patient's fundus exam and vision were normal. Therapy with antivirals and steroids may be effective i…

BACKGROUND AND AIM: : Infliximab is a chimeric human-murine monoclonal antibody with a high affinity and specificity for tumor necrosis factor α, which is nowadays widely used in Crohn's disease. An exacerbation of demyelinating disease as an adverse effect of this treatment is rare. The aim of this study was to discuss the relationship between infliximab and optic neuritis.
CASE REPORT: : We describe a 55-year old man with Crohn's disease who developed retrobulbar optic neuritis of the left eye after the infusion of infliximab. The outcome was favorable after systemic steroid treatment.

OBJECTIVE: 1) To evaluate the management of acute isolated optic neuritis (ON) by ophthalmologists and neurologists; 2) to evaluate the impact of clinical trials; 3) to compare these practices among 7 countries.
METHODS: A survey on diagnosis and treatment of acute isolated ON was sent to 5,443 neurologists and 6,099 ophthalmologists in the southeast-USA, Canada, Australia/New Zealand, Denmark, France, and Thailand. USA data were compared to those of other countries.
RESULTS: We collected 3,142 surveys (1,449 neurologists/1,693 ophthalmologists) (29.8% response rate). In all countries, ON pa…

BACKGROUND: A 38-year-old woman with systemic lupus erythematosus presented with headaches and bilateral hearing loss. Brain MRI was initially suggestive of small-vessel disease developing in the context of neuropsychiatric systemic lupus erythematosus. Several months later, the patient developed optic neuritis, followed by recurrent attacks of myelitis.
INVESTIGATIONS: MRI of the spine revealed multifocal regions of myelitis affecting the cervical spine. Serological evaluation revealed the presence of neuromyelitis optica-IgG antibodies. MRI of the brain was nondiagnostic for multiple sclero…

OBJECTIVE: To investigate the relation between retinal nerve fiber layer (RNFL) thickness and latency and amplitude of multifocal visual-evoked potentials (mfVEPs) in the postacute stage of optic neuritis in patients with early or possible multiple sclerosis.
METHOD: Thirty-two patients with clinical diagnosis of unilateral optic neuritis and magnetic resonance imaging lesions typical of demyelination and 25 control subjects underwent mfVEP and optical coherence tomography imaging.
RESULTS: Although there was significant reduction of RNFL thickness in the affected eyes (18.7%), a considerabl…

The recovery of visual performance and pupil responses were investigated in patients with demyelinating optic neuritis (ON) and multiple sclerosis (MS). The pupil constriction amplitude and the time delay (latency) of the pupil response were measured in 14 patients with a history of unilateral ON in response to either achromatic (luminance) or chromatic (isoluminant) stimulus modulation. Five of these subjects were diagnosed later with MS. In addition, we measured detection thresholds for achromatic stimuli using standard visual field perimetry and chromatic thresholds using a new colour asses…

OBJECTIVE: The pupil response to light flux increments is abnormal in severe optic neuritis, but little is known about the effects of this condition on the pupil colour response. The aim of this study was to examine how optic neuritis affects pupil responses to light flux and colour modulation and the extent to which such pupil responses mirror the loss and recovery of vision.
METHODS: A new pupil examination technique that makes use of sinusoidal modulation of either luminance contrast or chromatic saturation was employed. This technique enables the automatic extraction of both pupil respons…

OBJECTIVE: To determine whether the Optic Neuritis Treatment Trial (ONTT) results have altered the practice patterns of ophthalmologists and neurologists.
DESIGN: Mail survey.
PARTICIPANTS: A random sample of 987 ophthalmologists and 900 neurologists practicing in the United States were mailed a questionnaire that inquired into decision-making with regard to management of optic neuritis before and after the publication of the ONTT results.
MAIN OUTCOME MEASURES: Responses received from 202 ophthalmologists and 244 neurologists, a response rate of 47%.
RESULTS: Following the ONTT reports, n…

One hundred and thirty cases of optic neuritis seen between 1953 and 1994 were prospectively studied. Seventy-nine (61%) developed definite multiple sclerosis. There were 19 cases of recurrent optic neuritis which, during the follow up, did not develop other signs of multiple sclerosis. Copyright 1999 Harcourt Publishers Ltd.

Acute unilateral monosymptomatic optic neuritis (ON) is a common first manifestation of MS if associated with multiple MS-like lesions on brain MRI and oligoclonal IgG bands (OB) in the CSF, whereas ON patients lacking these laboratory abnormalities are considered to have a good prognosis regarding future MS development. Several cytokines involved in immune regulation are upregulated in blood and even more noticeable in CSF in MS. To study a possible relation between cytokine profiles and presence versus absence of MS-like brain MRI lesions and CSF OB, we used in situ hybridization to examine …

Optic neuritis may occur as an isolated entity or as a manifestation of multiple sclerosis (MS), a widespread central nervous system demyelinating disease. Clinical, electrophysiological, magnetic resonance imaging and neuropathologic data support the hypothesis that "idiopathic" optic neuritis represents a restricted form of MS. The current evidence for viral, cell mediated, and antibody-induced etiologies of MS and optic neuritis are reviewed.

The flicker test, introduced by Aulhorn is a new simple method to detect acute optic neuritis. We examined 24 patients suffering from acute optic neuritis. 23 patients showed a pathological result. In 179 subjects (patients with other ophthalmological diseases and healthy persons) the flicker test always showed normal results. After restitution of acute optic neuritis the results of the flicker test return to normal while the visual evoked responses remain pathological in many cases. Thus the flicker test can be an efficient tool in the diagnosis and differential diagnosis of acute optic neuri…
