[Penile metastasis from rectal carcinoma: a case report and literature review].

OBJECTIVE: To improve the diagnosis and treatment of penile metastasis from rectal carcinoma. METHODS: We reported a case of penile metastasis secondary to rectal adenocarcinoma, reviewed the relevant literature, and discussed the common origins, clinical features, pathogenic mechanisms, diagnosis and treatment of this disease. RESULTS: The patient was a 54-year-old male, with metastatic penile tumors secondary to rectal adenocarcinoma, with serious adhesion to the surrounding tissue and metastasis to the liver. As treatment, we performed colostomy to relieve voiding difficulty, followed by

Coarctation of aorta in a 52-year-old female.

Coarctation of the aorta is a complex vascular lesion that usually originates distal to the left subclavian artery. It accounts for 5-10% of all congenital cardiovascular malformations and carries a high morbidity and mortality risk. Symptoms depend on the severity of the disease and other coexisting anomalies. Early in life, diagnosis is usually based on significant symptoms and physical findings. Many patients remain asymptomatic until adulthood, with most being diagnosed in the 2nd or 3rd decade. In this case report, we present a late diagnosis of aortic coarctation with a bicuspid aortic v

Stressful life events, culture, and violence.

This study explored the contributions of stressful life events and their interactions with social support and cultural factors in predicting serious violence among American adolescent immigrants of Chinese and Southeast Asian origins. Youth differed in their exposure to stressors and how they responded to them. Cambodian and Laotian youth reported the highest levels of stressors, except for emotional abuse. Only physical abuse was an independent predictor of serious violence for all groups, except Chinese. Perceived social support buffered the effects of some stressors, whereas increased level

Anatomy of the cranial nerves in medieval Persian literature: Esmail Jorjani (AD 1042-1137) and The treasure of the Khwarazm shah.

Esmail Jorjani was an influential Persian physician and anatomist of the 12th century who did most of his writing after his seventh decade of life. Jorjani's comprehensive textbook of medicine, Zakhirey-e Khwarazmshahi (The Treasure of the Khwarazm Shah) was written in approximately AD 1112 and is considered to be the oldest medical encyclopedia written in Persian. This was an essential textbook for those studying medicine during this time. We describe the life and times of Jorjani and provide a translation and interpretations of his detailed descriptions of the cranial nerves, which were writ

Practical cystoscopy: care of the instruments and preparation for a cystoscopic examination.

Periodically, a look at the past helps health care professionals see how much practice has changed as well as how some concepts and practices remain similar to the original rationales and techniques. Two chapters from a 1911 textbook on cystoscopy have been reprinted that describe catheter and instrument care and preprocedure requirements for cystoscopy performed in the hospital, office, or home. Sterilization methods and instruments for cystoscopy have improved in the century since this book was written but the rationale for the procedure and concepts guiding the instrumentation have some sim

Association between duration of neonatal hospital stay and morbidity in the first month of life.

The morbidity of 506 healthy full-term newborns was studied in the first month of life in relation to the time they stayed in the hospital. The average time for the newborns who were born by vaginal delivery was 73.3+/-11.7 hours, while for those who were born by cesarean section it was 135+31.5 hours. Thirty-seven newborns presented health problems during the neonatal period (7.3%) and only 2% needed hospital readmission. The commonest problem in the newborns we studied was jaundice which appeared from the fourth to sixth day of life. During the second fortnight the commonest problems were in

Positive employment service - facilitating employment for people living with HIV.

As treatments and therapies develop and improve, enhancing life expectancy, Human Immunodeficiency Virus (HIV) has been described by some as a long term chronic condition rather than an acute life threatening illness. As this change occurs, new issues become relevant for the person living with HIV, employment being one of these. In light of this, the Positive Employment Service was developed and implemented to explore and address some of the issues related to employment for people living with HIV. The original concept for the service was primarily focused on vocational rehabilitation; however,

Mechanisms and clinical manifestations of supraventricular tachycardias.

Supraventricular tachycardia (SVT) is used as a general label for a variety of fast heart rhythms originating in any part of the conduction system above the ventricles. Supraventricular tachycardia encompasses subclasses of tachycardias such as: atrioventricular nodal reentrant tachycardia (AVNRT), atrioventricular reciprocating tachycardia (AVRT), atrial tachycardia, and atrial flutter. Nurses may encounter patients with these common types of dysrhythmias in inpatient or outpatient settings. Supraventricular tachycardia causes numerous symptoms which may make life difficult for these patients

Common sense and tumor treatment. A case of pilomatrical carcinoma in a 21-year-old patient with surprisingly rapid tumor progression.

Pilomatrical carcinoma is a rare tumor originating from skin appendages, usually occurring between the 5th and 7th decade of life. We present a case of an exceptionally young, 21-year-old patient with surprisingly rapid tumor progression and answer the question, what was the reason for such uncontrolled tumor growth. The main concern is the diagnostic challenge and a peculiar, one week race against time and tumor progression so that the least disfiguring surgery could be done.

Fearful birth? So what's new?

Significant fear of birth (tocophobia) appears to be increasing, coinciding with an increase in requests for non-medical indication caesarean section. It can arise from previous birth trauma, or be present in late childhood or early adulthood prior to pregnancy. This article explores some of its origins, presentations, effects and the importance of using a midwifery model of care with referral to specialist services as appropriate, to facilitate a positive birth experience which will enhance the woman's wellbeing and family life.

[Phaeochromocytoma revealed by a cardiogenic shock treated by extracorporeal life support].

Phaeochromocytoma is a catecholamine-secreting tumour that originates from the chromaffin cells in the adrenal medulla in 85% of the cases. Phaeochromocytoma typically presents with the classic signs and symptoms of paroxysmal hypertension, tachycardia, and episodic headache in young adults. However, it rarely may manifest as cardiogenic shock due to a catecholamine induced cardiomyopathy. We report the use of central extracorporeal life support (ECLS) in a young man admitted to our department because of cardiogenic shock caused by phaeochromocytoma.

Taussig-Bing anomaly: from original description to the current era.

Taussig-Bing anomaly is a rare congenital heart malformation that was first described in 1949 by Helen B. Taussig (1898-1986) and Richard J. Bing (1909-). Although substantial improvement has since been achieved in surgical results of the repair of the anomaly, management of the Taussig-Bing anomaly remains challenging. A history of the original description of the anomaly, the life stories of the individuals who first described it, and the current outcomes of its surgical management are reviewed herein.

Littre's hernia in newborn infants: report of two cases.

Littre's hernia was originally defined by Reinke in 1841 as "the presence of a Meckel's diverticulum in any hernial sac." It is generally difficult to differentiate from other types of hernia until complications arise. It is a rare and accidental finding at any age, but it is absolutely exceptional in neonates. To our knowledge, only one case under the age of 1 month has been reported in the literature. Herein two cases treated in the first 20 days of life are reported.

Mechanisms of premature closure of cranial sutures.

Craniosynostosis is defined as premature closure of the sutures of the skull, resulting in cranial deformity. Since Virchow's original paper describing the relationship between premature suture closure and skull morphology, we have learned much about the underlying mechanisms and consequences of premature suture closure. In this paper we will describe the biology of suture closure, the rules governing the resulting phenotypes seen clinically, and a prospective study of skull growth during the 1st year of life.

Upper limb Escherichia coli cellulitis in the immunocompromised.

The neutropenic state characteristic of acute lymphoblastic leukaemia (ALL) predisposes to infections involving Gram-negative bacilli. An Escherichia coli cellulitis originating in the first web space of the hand is described in a patient undergoing reinduction chemotherapy for ALL. Proximal extension of the infection progressed at a very rapid rate and required a forequarter amputation as a life saving measure. Due to the blunted inflammatory response in neutropenic patients, the need for close monitoring and quick intervention is stressed.

An Early and Unequal Decline: Life Course Trajectories of Cognitive Aging in the United States.

Objectives: Cognitive aging is a lifelong process with implications for Alzheimer's disease and dementia. This study aims to fill major gaps in research on the natural history of and social disparities in aging-related cognitive decline over the life span. Methods: We conducted integrative data analysis of four large U.S. population-based longitudinal studies of individuals aged 12 to 105 followed over two decades and modeled age trajectories of cognitive function in multiple domains. Results: We found evidence for the onset of cognitive decline in the 4th decade of life, varying gender differ

The puzzle of apparent life-threatening events in a healthy newborn.

Apneic neonatal seizures can present as apparent life-threatening events. We report a newborn with unexplained episodes of apnea associated with cyanosis and desaturation, starting on the first day postpartum. Biochemical tests were normal. Central nervous system infections as well as abnormalities of upper airways and cardiovascular system were excluded. Brain monitoring using amplitude-integrated electroencephalography (aEEG) was inconclusive. Continuous monitoring using video EEG revealed epileptic seizures originating from the left temporal region as the cause of the apneas. Magnetic reson

[The 'dizziness handicap inventory': initial psychometric evaluation of the German version].

BACKGROUND: Dizziness has a clear impact on quality of life of patients. Standardized instruments such as the "Dizziness Handicap Inventory" (DHI) help clinicians assess this impact systematically. The purpose of this study was to analyse the psychometric quality of a German version of the DHI. METHODS: One hundred and five patients with dizziness as their primary complaint have completed the DHI and undergone vestibular examination. The structure of the DHI was determined with a factor analysis, a principal component analysis with a Varimax rotation. To evaluate the reliability, internal con

The evolutionary expansion of the Sporozoa.

The sporozoans comprise a coherent group of protozoans, with characteristic and complex life cycles, containing 4-5000 species parasitic in invertebrates, particularly annelids and arthropods, and vertebrates. The group is a very successful one but neither its origins nor evolution are well understood. Considerations of traditional life cycles combined with newer methodologies have thrown some light on the evolutionary expansions of the main groups of sporozoans, the gregarines, coccidia, haemosporidians and piroplasms. The sporozoans of economic importance such as the coccidia, malaria parasi
