Association of Visual Health With Depressive Symptoms and Brain Imaging Phenotypes Among Middle-Aged and Older Adults.

IMPORTANCE: Vision loss and depression are common conditions with major health implications. However, mechanisms of the association of visual health (across the full acuity spectrum) with depression remain unclear. OBJECTIVE: To characterize the association between visual health and depression and investigate the association between depression and brain microstructure and macrostructure in subgroups divided by visual acuity. DESIGN, SETTING, AND PARTICIPANTS: In the UK Biobank Study cohort, 114 583 volunteers were included at baseline from March to June 2006 to July 2010. Habitual distance v

Cataract extraction after vitrectomy.

Cataract extractions were performed in 25 patients who had previously undergone pars plana vitrectomy. Posterior chamber IOLs were implanted in 20 of 26 eyes. Modification of the usual surgical technique was often necessary because the zonules and/or posterior capsule were unusually mobile. Lens opacities were nuclear sclerotic in 17 eyes (65%) and posterior subcapsular in nine eyes (35%). Visual acuity improved an average of five lines, and final visual acuity was 20/50 or better in 13 eyes (50%). Visual acuity after cataract extraction closely paralleled the best visual acuity after vitreous

Rehabilitation and education are underutilized for mild stroke and TIA sufferers.

PURPOSE: Transient ischemic attack (TIA) and mild stroke represent a large proportion of cerebrovascular events, at high risk of being followed by recurrent, serious events. The importance of early education addressing risk management, secondary prevention and lifestyle modifications is the centerpiece of further stroke prevention. However, delivering education and rehabilitation to this population can be complex and challenging. METHODS: Via synthesis of a narrative review and clinical experience, we explore the unique and inherent complexities of rehabilitation management and education prov

A case report of spontaneously resolving Descemet's detachment after cataract surgery.

PURPOSE: To report a case of large central Descemet's membrane detachment (DMD) following cataract surgery and discuss the potential for spontaneous resolution as an alternative to surgical intervention. OBSERVATIONS: A 79-year-old male with a history of macular degeneration and ocular hypertension developed a large central DMD following cataract surgery complicated by posterior capsular rupture. The patient initially presented with severe corneal edema and an uncorrected visual acuity (UCVA) of 20/200. Despite medical treatment, the DMD persisted, and surgical intervention was considered at

Rare corneal complication following selective laser trabeculoplasty.

PURPOSE: This case report describes two glaucoma patients who underwent selective laser trabeculoplasty (SLT) and developed a rare post-procedure corneal complication and subsequent permanent corneal distortions and reduced acuity. OBSERVATIONS: Both patients experienced early post-procedure corneal edema, reduced visual acuity, and pain. Each patient was treated with topical steroids with resolution of the edema, but each patient had irregular corneal astigmatism which was not present pre-operatively and did not resolve. CONCLUSIONS AND IMPORTANCE: There are very few reported cases of post-

Successful treatment of central retinal artery occlusion using hyperbaric oxygen therapy.

Central retinal artery occlusion (CRAO) is considered an ophthalmologic emergency. The prognosis of this disease is very poor. Currently, there is no generally effective therapy available to treat CRAO. Hyperbaric oxygen therapy (HBOT) can increase the volume of oxygen delivered to the ischemic retinal tissue until spontaneous or assisted reperfusion occurs. We report the case of a patient who experienced sudden visual loss due to CRAO that was treated with HBOT. The patient was an 81-year-old woman who presented with CRAO in her right eye (OD). She exhibited "hand motion" visual acuity before

[Morgagnian cataract].

Cataract is a total or partial clouding of the lens of the eye. It can be congenital or acquired. Cataract associated with the aging process (senile) is the most common type. Common symptoms include visual impairment which varies based on the type and the density of lens opacity. Cataracts etiology is varied and treatment is based on surgery. In patients with morgagnian cataract cortex liquefies and lens core hardens and becomes cloudy. We here report the case of a 51-year old patient with no notable medical history presenting with progressive decreased visual acuity in the left eye with no pa

Voriconazole-refractory fungal infection of phacoemulsification tunnel.

A 44-year-old man presented 28 days after cataract surgery (phacoemulsification) in right eye with multiple pinpoint infiltrates in posterior stroma at cataract surgery wound site. Visual acuity was 20/60. Corneal scraping from the floor of the corneal tunnel revealed fungus which was later identified to be Aspergillus flavus. The patient was started on oral voriconazole 200 mg twice daily and topical voriconazole 1% every hour. Two intracameral injections of voriconazole (50 micrograms/ 0.1 ml) were given 72 h apart, five days after starting initial therapy. Infiltrates increased in size and
