BACKGROUND: Sacrococcygeal teratomas (SCT), although rare, are the most common teratomas found in neonates. Anesthetic management of neonates undergoing SCT resection surgery is challenging, given the risk of massive hemorrhage and high mortality rate.
AIMS: The primary aim of this single center retrospective study was to analyze neonates undergoing SCT resection over the last decade and report on perioperative outcomes, including blood product transfusion practices. The secondary aim was to describe patient and tumor characteristics that might place neonates undergoing SCT resection surgery

BACKGROUND: Providing safe sedation in pediatric patients remains challenging because of age-dependent physiological variations. This study aimed to identify the age-stratified 95% effective dose (ED95) of remimazolam tosilate for achieving moderate sedation in children.
METHODS: This study enrolled children aged 1-12 years with ASA physical status I-II scheduled for elective surgery. Participants were stratified into three age groups: toddlers (1-3 years), preschoolers (3-6 years), and school-aged children (6-12 years), with 40 patients in each group. The initial dose of remimazolam tosilate

Extracorporeal membrane oxygenation (ECMO) is widely used in paediatric cardiac surgery units. Over recent years, interest in ECMO has grown among anaesthesiologists working in paediatric intensive care units. The indication for ECMO therapy is respiratory or cardiopulmonary failure in which, despite high inspired oxygen concentrations, advanced ventilator strategies and optimisation of the patient's condition, persistent hypoxaemia and hypercapnia carry a risk of further deterioration and death. This consensus statement of Paediatric Anaesthesiology and Intensive Therapy Section of the Polish

BACKGROUND: When cricoid pressure was introduced in pediatric anesthesia, the cricoid force was not defined, leading pediatric anesthesiologists to question its necessity, effectiveness, and safety. We hypothesized that airway complications encountered in clinical practice may have resulted from the exertion of excessive cricoid force.
METHODS: Using a novel instrument, we measured cricoid force during Glidescope® videolaryngoscopy in three groups of 40 anesthetized children: Group 1 (3-5 years), Group 2 (6-8 years), and Group 3 (9-14 years). A biased-coin up-and-down design was employed to e…

OBJECTIVE: We evaluated the need for vigorous resuscitation (bag-and-mask ventilation, tracheal intubation, and cardiopulmonary resuscitation) in certain common cesarean deliveries at term to evaluate the need for pediatrician attendance on behalf of the fetus.
METHODS: Records of singleton cesarean deliveries (repeat, nonprogressive labor, fetal malposition, fetal heart rate abnormality) at term over 2 years were reviewed for the following: need for vigorous resuscitation, Apgar scores, anesthesia used, and the need for newborn intensive care. The next consecutive, uncomplicated singleton va

Situs inversus totalis (SIT) with congenitally corrected transposition of the great arteries (ccTGA) represents a rare and complex congenital combination that significantly complicates anaesthetic management. We report the anaesthetic management of an early childhood (2-5 years), 13 kg child diagnosed with SIT and ccTGA undergoing elective adenotonsillectomy. Individualised perioperative strategies were implemented, including modified ECG monitoring, arrhythmia preparedness and explicit mg/kg drug dosing. Intraoperative management was uneventful, and the patient had a stable recovery. This cas

BACKGROUND: Preoperative sedation in children is necessary as it is a common source of distress, potentially leading to difficult induction. Intranasal dexmedetomidine bypasses hepatic metabolism and has good availability. It is non invasive, easy with minimal respiratory depression. This study aimed to observe safety, efficacy, sedation level and parental separation score following a fixed dose of 1.5 mcg/kg Methods: A cross sectional study design was conducted among 43 children posted for elective surgeries. After informed consent and assent were obtained, intranasal DEX 1.5mcg/kg was admini

BACKGROUND: Preoperative sedation is critical to alleviate anxiety in children undergoing neurosurgery, but the role of remimazolam is still uncertain such as its unestablished dosing and safety. This study was aimed to determine the 90% effective dose (ED90) of remimazolam for moderate sedation in this pediatric population.
METHODS: This dose-finding study enrolled children aged 3 months to 6 years scheduled for neurosurgery. The up-and-down method (k-in-a-row, k= 6) was employed to investigate the ED90 of remimazolam for moderate sedation. According to the k-in-a-row rule, one patient recei

PURPOSE OF REVIEW: The U.S. Federal Drug Administration (FDA) recently released a warning stating that 'repeated or lengthy use of general anesthetic and sedation drugs during surgeries or procedures in children younger than 3 years or in pregnant women during their third trimester may affect the development of children's brains' (www.fda.gov/ucm582356.htm). The goal of this article is to review the most recent clinical studies which provide evidence that these concerns may be overstated for the majority of healthy young children who require surgery and anesthesia.

Perioperative respiratory adverse events pose a significant risk in pediatric anesthesia, and identifying these risks is vital. Traditionally, this is assessed using history and examination. However, the perioperative risk is multifactorial, and children with complex medical backgrounds such as chronic lung disease or obesity may benefit from additional objective preoperative pulmonary function tests. This article summarizes the utility of available pulmonary function assessment tools as preoperative tests in improving post-anesthetic outcomes. Currently, there is no evidence to support or dis…

Prior to anesthesia with propofol for gastrointestinal endoscopy, sets of pictures were presented to 20 children and adolescents (M age = 12 years). Word pairs (e.g., "hiking-woods") were presented via earphones after the children were anesthetized. Upon regaining consciousness, the children were tested for explicit memory of both the picture sets and word pairs by free recall, cued recall, and yes/no recognition. Implicit memory was tested by free association to category cues for the pictures and by word association for the word pairs. Postoperative testing revealed retrograde memory for mate

Conjoined twins are rare and present significant challenges for surgical and anesthetic teams. Pygopagus twins, fused at the pelvis and lower spine, require meticulous planning, and multidisciplinary collaboration. This report describes the elective separation of pygopagus twin girls, involving advanced imaging, 3D anatomical modeling, and simulation-based preparation. A color-coded system differentiated anesthesia teams, equipment, and monitors, ensuring clarity during the 27-h procedure. Anesthesia was maintained with sevoflurane and remifentanil, with individualized fluid and hemodynamic ma

The objectives of the study were to investigate the characteristics of ears with dehiscence of the fallopian canal at the time of cholesteatoma surgery and the relationship between dehiscence and age, and to consider the reasons why the fallopian canal tends to be preserved in pediatric patients. This study included 37 ears with cholesteatoma in pediatric patients (mean age 9.2 years, age range 4-14 years) and 273 ears with cholesteatoma in non-pediatric patients (mean age 45 years, age range 15-84 years). Patients were treated between January 2006 and April 2012. All patients had undergone pr

Endoscopy for a hospitalized patient with acute severe ulcerative colitis should be limited to flexible sigmoidoscopy. If, during pregnancy, inflammatory bowel disease (IBD) is active or if a new diagnosis of IBD is suspected, endoscopy may be needed to guide medical decision-making. The determination to proceed with endoscopy during pregnancy is nuanced and requires the ability to discern appropriate indications, knowledge regarding safety of bowel preparation and anesthesia. Pediatric patients with IBD represent a unique subset of IBD with distinctive diagnostic, therapeutic, and monitoring

Possible modes of transmission of the human papilloma virus (HPV) in children include perinatal transmission, sexual transmission, or extragenital contact. Conventional treatment options with chemical and physical destruction methods can be difficult and painful and often require general anesthesia. Imiquimod is a topically active immunomodulatory agent that has been shown to successfully treat pediatric anogenital warts. We report on a case of extensive anogenital warts in a 18-month-old girl who was successfully treated with topical 5% imiquimod cream.

Moyamoya disease is a condition that results from bilateral stenosis or obstruction of the intracranial arteries at the base of the brain. Patients exhibit ischemic symptoms, and vascular reconstruction is the therapy of choice. Surgical treatment for Moyamoya disease is often complicated by cerebral ischemia, so the goal in perioperative management is to maintain the balance between oxygen supply and demand in the brain. This report presents three cases of Moyamoya disease in patients under 3 years of age, and discusses anesthesia management issues for pediatric patients with this condition.
