Chylous peritonitis in children: case report and literature review.

Sixty-one cases of acute chylous peritonitis have been reported in all age groups in the surgical literature. Only seven of these cases have been noted in children. We report an additional case of a 7-year-old child who presented with acute peritoneal irritation and, at laparotomy, was found to have chyle in the peritoneal cavity and retroperitoneum. Proposed etiologies and management of this surgical entity are reviewed.

Gangrenous and perforated appendicitis with peritonitis: treatment and bacteriology.

A comparison of single-agent antimicrobial therapy in the treatment of patients with perforated or gangrenous appendicitis and peritonitis was performed in a double-blind, randomized, prospective trial. Pathologic documentation of advanced appendicitis and positive intraoperative specimen cultures were required for inclusion in the study. Ceftizoxime (2 gm every 12 hours) and cefoxitin (2 gm every six hours) were compared. There were no significant differences between the treatment groups. Ninety-seven percent of patients treated with ceftizoxime and 89% of those treated with cefoxitin were cu…

Intense accumulation of indium-111 leukocytes in peritonitis carcinomatosa.

In order to detect the infectious foci in a case of terminal recurrent cancer of the sigmoid colon with intense inflammation, In-111 oxine leukocyte scintigraphy was performed. Leukocytes labeled with In-111 oxine quickly localized within the region of peritonitis carcinomatosa and could be imaged after 4 hours. With time, high activity appeared in this area. And 48 hours after injection, the large intestine was clearly seen. However, no activity was seen in the main recurrent tumor. This suggested that the labeled leukocytes had accumulated in regions of inflammation rather than in malignant …

[Granulomatous peritonitis due to starch. Apropos of a case].

The authors present a case of granulomatous peritonitis due to starch in a 47-year-old female. This is an infrequent, but clinically important, process. Contamination occurs mainly through rubber gloves. The symptomatology is varied and simulates a picture of acute abdomen. The diagnosis rests on examination of the liquid obtained by peritoneocentesis or peritoneal biopsy. The histopathologic picture is that of a granulomatous reaction to foreign matter, with giant cells that exhibit birefringent granules in the form of the Maltese cross. Medical management is based on the use of steroids and …

Diagnostic value of ascites adenosine deaminase in tuberculous peritonitis.

The value of ascitic fluid adenosine deaminase activity in distinguishing tuberculosis from other causes of ascites was examined in a retrospective study of 41 patients with bacteriologically confirmed tuberculous peritonitis and 41 control patients, matched for age and sex, with ascites of other causes (12 alcoholic cirrhosis, 5 cryptogenic cirrhosis, 12 malignant disorders, 3 pancreatitis, and 9 miscellaneous causes). The mean ascites adenosine deaminase activity was 99.8 (SD 49.1) in tuberculous patients and 14.8 (8.4) U/l in control patients (p less than 0.0001). A cutoff of 32.3 U/l had a…

[Peritonitis and infection in children with idiopathic nephrotic syndrome].

We analyzed episodes of peritonitis and/or sepsis associated with the idiopathic nephrotic syndrome (ns) in 23 children treated between 1975 and 1985 at our clinic. 37.5% of the children with infantile ns, 16% of those with steroidsensible ns, and 13.6% of those with steroidresistant ns developed peritonitis. Children with infantile ns and one girl with gram-negative secondary peritonitis presented with sepsis. 3 septic children died. Four patients developed peritonitis secondary to intestinal perforation. The most common bacterial pathogen in primary peritonitis was S. pneumoniae (7 patients)…

[Characteristics of peritonitis in cancer of the gastrointestinal tract].

Under analysis was material of treatment of 63 patients with gastric and colonic cancer complicated by acute peritonitis. An atypical asymptomatic course of the subreactive and a reactive forms of peritonitis is associated with pronounced inhibition of factors of antiinfection defense in elderly patients with the III-IV degrees of cancer. Complex stimulating therapy is indicated in the postoperative period because of immunodepression taking place after urgent operative interventions.

Antibiotic susceptibility of staphylococci from CAPD peritonitis in children.

Over a six year period to 1985, 64 episodes of bacterial peritonitis in children on CAPD yielded 59 Gram-positive and 15 Gram-negative isolates. The former included 27 strains of Staphylococcus epidermidis (45.8%) and 24 Staph. aureus (40.7%). Staph. epidermidis is now the commonest cause of infection in these patients and like Staph. aureus is spread by leaks and disconnections. It is also more resistant to antibiotics than Staph. aureus and this causes problems in the choice of antibiotics with failure to control peritonitis and subsequent complications such as diminished efficacy of therapy…

Meconium peritonitis: prenatal sonographic findings and their clinical significance.

Ultrasonographic (US) findings were correlated with clinical outcome in seven cases of meconium peritonitis detected with prenatal US during a 2-year period. Nineteen previously reported cases were also reviewed. US findings included intraabdominal calcifications (n = six cases), fetal ascites (n = 3), echogenic ascites without calcifications (n = 1), bowel dilatation (n = 2), and polyhydramnios (n = 5). Following delivery, six infants were still alive after a mean follow-up of 13 months (range, 6-26 months); the seventh died of hydrocephalus. Of the six, four required surgical correction of a…

[Means of reducing mortality in peritonitis of appendicular origin].

An experience with the treatment of 836 patients with appendicular peritonitis has shown that early diagnosis of the disease and exact determination of the degree of peritonitis could improve results of their treatment. The operative measures should be fulfilled under general anesthesia. The access should ensure good treatment of the abdominal cavity and drainage of the small intestine in paralytic obstruction. Special care is necessary in the postoperative period for timely recognition of intraabdominal abscesses.

Surgical management of diffuse peritonitis complicating obstetric/gynecologic infections.

A retrospective review of 176 women undergoing surgery for diffuse peritonitis secondary to pelvic infections from 1972 to 1976 was conducted. Mortality with septic abortion was 27.3% (12 of 44), with pelvic inflammatory disease 7.1% (eight of 113), and with puerperal sepsis 6.7% (one of 15). Overall mortality was 13.1% (23 of 176). Mortality was lower (P less than .05) with surgery within 24 hours (7.5%, six of 80) than after 24 hours (17.7%, 17 of 96). Mortality was lower (P less than .01) with specific antianaerobic antibiotics (zero of 36) than without (16.4%, 23 of 140). Mortality was 21.…

Consequences of intraperitoneal bile: bile ascites versus bile peritonitis.

Recent experience with patients with bile ascites and bile peritonitis prompted a review of other case histories in the medical literature of these conditions. The clinical courses of 24 patients with bile ascites and 34 with bile peritonitis were reviewed. Bile ascites occurred most often as a postoperative complication of biliary tract operations and also occasionally after trauma. Clinical signs were minimal except for abdominal distention, and operations were delayed for an average of 30 days. Peritoneal fluid was sterile in the 11 patients studied. In contrast, bile peritonitis occurred m…

[Prevention of dynamic intestinal obstruction in children with peritonitis].

In order to prevent the development of the dynamic intestinal obstruction the prolonged peridural blockade, intravenous and intraaortal injection of the solution of novocain were used with reference to the phase of the pathological process. Results of the examination of 200 patients has shown that these measures facilitate passage of flatus and stools, shortens the staying of the patients at the hospital and reduces the amount of postoperative complications and lethality.
