Changes in fungal ultrastructure after short-course ciclopiroxolamine therapy in pityriasis versicolor.

The clinical outcome and the effects on morphogenesis and cell ultrastructure induced by a 1% ciclopiroxolamine solution in six patients with proven pityriasis versicolor were studied. Treatment regimens consisting of a once-daily application for 1 day, 2 days (Days 1 and 4) or 3 days (Days 1, 4 and 8). Clinical evaluation, scanning (SEM) and transmission electron microscopy (TEM) were performed on skin scrapings before treatment, and at 3, 7, 15 and 21 days after the start of therapy. SEM techniques have shown severe changes in the surface ultrastructure of yeasts and hyphae of Pityrosporum s…

Pityriasis rosea (Gibert): abnormal distribution pattern of antigen presenting cells in situ.

Pityriasis rosea is a skin disease which is obscure in its etiology and pathogenesis. We studied its immunopathology by immunophenotyping the inflammatory cells in situ using monoclonal antibodies that define leukocyte subsets. Findings as to T-cells and their major subsets did not reveal disease-specific data. Monocytes stained only rarely. Neither natural killer cells, B-cells nor plasma cells were ever found. An unexpected finding was the presence within the infiltrates and rarely within the epidermis of cells having the immunophenotype of interdigitating cells (RFD1+). Intense and dendriti…

Histopathologic diagnosis of pityriasis lichenoides et varioliformis acuta and its clinical correlation.

To assess the specificity of the histopathologic features in the diagnosis of pityriasis lichenoides et varioliformis acuta (PLEVA), we reviewed the clinical manifestations and courses of 42 patients for whom this diagnosis was suggested in the pathology report. The histologic diagnosis of PLEVA was clinically substantiated in 16 of these 42 cases. Of the 26 cases in which PLEVA was erroneously diagnosed histologically, the correct clinical diagnosis was suggested before biopsies were done in 21 instances. In the five remaining cases, both the prebiopsy clinical diagnosis and the pathologic di…

Persistent Acral Pityriasis Rosea in a Pediatric Patient: A Rare Clinical Variant.

Pityriasis rosea (PR) is a self-limited papulosquamous skin condition most commonly observed in children and young adults. Atypical variants of PR have been increasingly recognized, particularly those with unusual morphology, distribution, or disease course. Acral involvement is rare and may pose diagnostic challenges, especially in pediatric patients. We report the case of a 15-year-old previously healthy Saudi male patient who presented with a persistent erythematous and scaly eruption lasting for more than three months. The eruption involved the trunk and extremities and showed prominent ac…

Pityriasis amiantacea in erosive pustular dermatosis of the scalp: A report of three cases.

Erosive pustular dermatosis is an uncommon chronic inflammatory condition of the scalp characterized by crusted, erosive plaques and subsequent cicatricial alopecia. Management of erosive pustular dermatosis remains challenging, and various treatments have been described in the literature with inconsistent results. Herein, we report three cases of erosive pustular dermatosis with a pityriasis amiantacea-like presentation in the context of poor scalp hygiene. All patients presented with thick adherent crusts on the scalp that were refractory to multiple therapies. Significant clinical improveme…

Pityriasis rubra pilaris partially responsive to treatment with upadacitinib: A case report.

Pityriasis rubra pilaris is a rare inflammatory dermatosis characterized by orange-red confluent plaques, hyperkeratotic follicular papules, palmoplantar keratoderma, and, in some cases, erythroderma. The etiology of pityriasis rubra pilaris is unclear. This condition is often treated with oral retinoids and topical corticosteroids, and more recently, biological agents have become the mainstay of treatment. However, there is a paucity of high-quality evidence on the safety and effectiveness of these agents, and the disease often remains refractory to therapy. Herein, we present a case of pityr…

Serum Interleukin-36 in Pityriasis Rosea: A Potential Biomarker for Disease Severity.

OBJECTIVE: Pityriasis rosea (PR) is a self-limiting acute rash with unclear etiology and pathogenesis. The cytokine profile of PR is an infrequently investigated field of research. The aim of this study was to assess the level of IL-36 in sera of patients with PR and its possible interrelation with disease severity. METHODS: Forty patients with PR were included in this case-control study, and 40 comparable healthy control subjects. Severity was assessed using pityriasis rosea severity score (PRSS) and serum IL-36 was assessed using ELISA. RESULTS: Serum IL-36 was significantly higher in pati…
