The indirect lesion of the brachial plexus by a traumatic false aneurysm of the axillary artery is a rare event. Clinical signs, diagnosis and follow-up of two patients are reported. Characteristic symptoms are persistent shoulder pain and progredient neurologic deficits with delayed onset. Examination for a jet phenomenon in the axillary region may confirm the suspected diagnosis. Between 1982 and 1991 we performed eight operations on seven patients with carotid artery aneurysms. Their mean age was 52.8 years (range: 20-67 years). Five aneurysms were atherosclerotic, one was associated with Marfan's syndrome, and two were pseudoaneurysms, one occurring after Dacron patch angioplasty and the other due to tuberculosis. Since 1972, 14 patients with subclavian steal and four with primary vascular insufficiency of the arm have been operated on using the axillo-axillary bypass graft. Two grafts have been replaced, one after nine months for threatened erosion, and the other after three years for thrombosis due to disease progression in the donor artery. We have performed 82 operations on 79 individuals with occlusive disease of the subclavian or innominate arteries during the past 16 years. The left subclavian was occluded in 63, the right in nine, and the innominate in seven. Presenting symptomatology was neurologic in 29, arm ischemia in 24, and combined in 23. We report a case of successful reoperation for aortic arch dissection with use of the "arch-first" technique in a patient who had Marfan syndrome. Extracorporeal circulation was initiated via right subclavian artery cannulation, and the chest was entered through a clamshell incision for the best exposure. We report on our clinical experience with a knitted Dacron prosthesis: thin-walled, spirally reinforced InterGard prosthesis. Between 1993 and 1996, 81 prostheses were implanted in 72 patients with an infrainguinal connection. We describe the special features of the prosthesis. The entire group of patients was analyzed.
