BACKGROUND: A congenital extrahepatic portosystemic shunt (CEPS) is a rare condition. Currently, CEPS is primarily diagnosed in the postnatal period. Most cases are detected using imaging techniques such as computed tomography (CT), digital subtraction angiography (DSA), and magnetic resonance imaging (MRI), typically after severe complications arise. Due to the effect of abdominal intestinal gas, the diagnosis of CEPS via postnatal ultrasound is relatively challenging. Conversely, prenatal ultrasound offers a distinct advantage in the diagnosis of CEPS. However, a lack of understanding of thi

Arthroscopy is an accepted technique for evaluation of intra-articular pathology and treatment of a variety of disorders even in the wrist joint. Dedicated miniaturized instrumentation is needed along with a specific traction system. The external distraction alone (dry technique) allows for complete joint exploration and several type of arthroscopic surgery, avoiding annoying leaking in the subcutaneous tissues, though further distension of the articular pouches can be achieved by saline infusion (fluid distension or wet technique). Knowledge of surface anatomic landmarks and careful surgical

Phacomatosis pigmentovascularis is a rare complex nevus accompanied with various types of developmental abnormalities. We experienced a case of phacomatosis pigmentovascularis with esophageal varices due to hypoplasia of the portal veins. Although computed tomography demonstrated marked atrophy of the right hepatic lobe and compensatory hypertrophy of the left hepatic lobe, laparoscopy revealed no signs of chronic parenchymal liver disease on the liver surface. In addition, no microscopically identifiable pathological findings were observed in the needle-biopsied liver specimens. Angiographic

BACKGROUND: Congenital portosystemic shunt (CPSS) is a rare vascular malformation which results in anomalous communication between the portal venous system and the systemic vein. The objective of this review is to describe the prenatal ultrasonic characteristics and postnatal outcomes of CPSS diagnosed prenatally, along with providing some suggestions for perinatal monitoring.
MATERIALS AND METHODS: A systematic literature search was conducted in PubMed and Ovid electronic databases in a period from January 2012 to May 2024, using the terms: "congenital portosystemic shunt", "ductus venosus",

BACKGROUND: Metabolic dysfunction associated steatohepatitis (MASH) is a leading cause of advanced liver fibrosis globally. Current histological scoring (NAS-CRN) excludes portal-based features, despite emerging evidence suggesting their relevance. Aim: To study the prevalence of portal tract changes and their correlation with fibrosis in liver biopsies from patients with MASH.
METHODS: In this retrospective and prospective cross-sectional study, 505 liver biopsies were analysed. Portal inflammation, interface hepatitis, ductular reaction, bile duct damage, and plasma cells were semi-quantita

OBJECTIVES: The gastric mucosa of patients with portal hypertension frequently manifests changes in its appearance that are readily identifiable by endoscopy. Many of these can be sources of bleeding, and some imply the presence of systemic disease. Although portal hypertension is critical in development of portal hypertensive gastropathy (PHG), the role that other factors might play in its pathogenesis is uncertain.
METHODS: Four groups of subjects were studied prospectively: 37 with portal hypertension due to cirrhosis, 26 noncirrhotic subjects with portal hypertension due to extrahepatic p

Portal hypertension is one cause and a part of a dynamic process triggered by chronic liver disease, mostly induced by alcohol or incorrect nutrition and less often by viral infections and autoimmune or genetic disease. Adequate staging - continuously modified by current knowledge - should guide the prevention and treatment of portal hypertension with defined endpoints. The main goals are interruption of etiology and prevention of complications followed, if necessary, by treatment of these. For the past few decades, shunts, mostly as intrahepatic stent bypass between portal and hepatic vein br

In recent years, defined progress has been made in understanding the mechanisms of hemodynamic disturbances occurring in liver cirrhosis, which are based on portal hypertension. In addition to pathophysiological disorders related to endothelial dysfunction, it was revealed: There is the restructuring of the vasculature, which includes vascular remodeling and angiogenesis. In spite of the fact that these changes are the compensatory-adaptive response to the deteriorating conditions of blood circulation, taken together, they contribute to the development and progression of portal hypertension ca

The congenital absence of the portal vein (CAPV) is a rare venous malformation in which mesenteric venous blood drains directly into the systemic circulation. Liver transplantation (OLT) may be indicated for patients with symptomatic CAPV refractory to medical treatment, especially due to hyperammonemia, portosystemic encephalopathy, hepatopulmonary syndrome, or hepatic tumors. Because portal hypertension and collateral circulation do not occur with CAPV, significant splanchnic congestion may occur when the portocaval shunt is totally clamped during portal vein (PV) reconstruction in OLT. This

A 47-year-old man was referred for evaluation and treatment of gastrointestinal variceal bleeding and possible transjugular intrahepatic portal-systemic shunting. Intrahepatic manometry disclosed a normal portal pressure, but selective mesenteric arteriography revealed occlusion of the superior mesenteric, splenic, and inferior mesenteric veins. Duodenal and gastric varices were noted, but no esophageal varices were seen. The portal vein was clearly patent. At surgery, a 2 cm mass was found at the superior mesenteric vein-splenic vein juncture, and subsequent pathologic examination confirmed t

BACKGROUND & AIMS: Sympathetic nervous system over-activity is associated with liver disease progression and development of portal hypertension, influencing systemic inflammation. This study examined the role of the sympathetic alpha-2A adrenergic receptor (ADRA2a) and its antagonism in hepatic stellate cell (HSC) activation, a key event in fibrosis progression, in experimental metabolic dysfunction-associated steatohepatitis (MASH) and in portal pressure elevation in a cirrhotic-rat model.
METHODS: An established bile duct ligation (BDL)-induced cirrhosis model (n = 14) was used to assess AD

Purpose: Portal hypertension (PHT) leads to complications such as variceal bleeding, hepatic remodeling and thrombosis, driven by altered hemodynamics. This study aims to elucidate flow structure, shear stress and helicity changes under PHT, and their potential roles in promoting thrombosis and vascular remodeling. Methods: A patient-specific portal venous system model was reconstructed from CT images. Computational fluid dynamics (CFD) simulations were conducted to evaluate flow velocity, wall shear stress (WSS), oscillatory shear index (OSI), relative residence time (RRT) and helicity. Resul

The purpose of the article is to present the differential diagnostic criteria between pneumobilia (air in the biliary system) and portal vein gas on abdominal x-ray. Differential diagnosis is essential because of its influence on patient management. Two patients are presented, one with pneumobilia and the other with portal vein gas on abdominal x-ray, with review of the relevant literature. Pneumobilia is often iatrogenic and even in cases of cholecystitis it is never a sole indication for emergency surgery. Patients with pneumobilia on abdominal x-ray can always be investigated further. On th

Schistosoma mansoni infection invariably results in liver fibrosis of the host. This fibrosis may be represented by small focal areas of chronic inflammation and excess extracellular matrix deposited in periovular granulomas, distributed in variable numbers at the periphery of the portal vein system. This is the outcome of 90% of the infected population in endemic areas. Conversely, a minority of infected individuals develop extensive disease with numerous granulomas along the entire extension of the portal spaces. This latter situation is mainly dependent on special hemodynamic changes create

About 10% of children with CF develop hepatic cirrhosis and progressive portal hypertension. As the portal hypertension worsens these children are likely to develop serious variceal bleeding and other complications including malnutrition and a decline in respiratory function. Indices of lung function may fall as much as 50% in a year and chest infections may require frequent admissions to hospital. The respiratory symptoms are often attributed to CF related lung disease and affected children may therefore be considered unsuitable for liver transplantation. We propose a simple scoring system wh

Hepatic radionuclide angiography was performed with Gamma camera, after bolus injection of 740 MBq 99m-Tc-pertechnetate. Hepatic and portal phases (slopes) of the liver time-activity curve were separated at the moment of the maximal left kidney activity, and hepatic perfusion index (HPI) was determined. Decreased (p < 0.01) HPI values were observed in patients with liver cirrhosis with or without oesophageal varices (X + SD) (0.32 +/- 0.19 vs. 0.49 +/- 0.13 respectively) in comparison to the controls (0.68 +/- 0.06). HPI values in these two groups of patients also differ between themselves (p
