Muscle formation in Ehlers-Danlos syndrome.

Ehlers-Danlos Syndrome (EDS) is a familial connective tissue disorder characterized by marked joint hypermobility, skin hyperdistensibility and fragility. To determine the influence of connective tissue hyperdistensibility on muscle function, a young woman with classic EDS underwent extensive muscle function testing which included a controlled strengthening exercise program. Quantitative muscle strength testing revealed generalized muscle weakness characteristic of this disease. The strength-length relationship curve for quadriceps was shifted, implying an increased series elastic component of

Effects of overweight and obesity on lower limb walking characteristics from joint kinematics to muscle activations.

BACKGROUND: Obesity is a crucial factor that increases the risk of initiating and advancing knee osteoarthritis. However, it remains unclear how obesity directly impacts the biomechanical experience of the lower limb joints, potentially triggering or exacerbating joint degeneration. This study investigated the interactive effects of BMI augmentation on lower limb kinematics, kinetics, and muscle activations during walking. METHODOLOGY: A group of 60 participants underwent a three-dimensional gait analysis. These individuals were categorized into three groups based on their body mass index (BM

Do individual differences in the distribution of activation between synergist muscles reflect individual strategies?

Individual differences in the distribution of activation between synergist muscles have been reported during a wide variety of tasks. Whether these differences represent actual individual strategies is unknown. The aims of this study were to: (i) test the between-day reliability of the distribution of activation between synergist muscles, (ii) to determine the robustness of these strategies between tasks, and to (iii) describe the inter-individual variability of activation strategies in a large sample size. Eighty-five volunteers performed a series of single-joint isometric tasks with their do

Compensatory load shifting in response to combined muscle weakness and neural delay during uneven-terrain walking: A predictive simulation study.

BACKGROUND: Age-related falls are often associated with muscle weakness (MW) and neural response delays (ND), but their combined influence on gait stability on complex terrain is not well understood. We examined how MW and ND jointly shape stability and compensatory motor strategies in a predictive neuromusculoskeletal model. METHODS: We used predictive simulations of a 2D sagittal-plane walker on uneven terrain. A factorial design (8 MW × 6 ND levels) varied quadriceps weakness (0-70%) and reflex pathway delays (0-100%). The objective used a mechanics-driven task formulation that prioritized

Vastus lateralis muscle force and architecture were unchanged after an eccentric training cycle in elite athletes.

The ability to generate force and power is conditioned by muscle properties, which can be improved through eccentric training. While vastus lateralis (VL) reported to be a major contributor of sprinting performance, its muscle architecture and subsequent impact on strength performance following an eccentric training cycle in elite sprinters remain unknown. Seven elite sprinters (2 females) were involved in the study. Before and after a 3-week eccentric training cycle, during which in addition to their four habitual sprint training sessions, athletes performed two eccentric strength and conditi

Technical Report on the New Ultrasound Lateral Mid-Shaft Approach to the Sciatic Nerve: A Never-Ending Story.

The anatomy of the sciatic nerve allows it to be blocked at different levels using various anesthetic approaches. However, for several reasons, performing these approaches may be challenging or disadvantageous in specific categories of patients, particularly in obese patients. The objective of this brief technical report is to describe a new technical approach to sciatic nerve block, designed to simplify the procedure for certain categories of patients and less experienced practitioners. Since 2010, more than 5000 procedures have been performed by both experienced anesthesiologists and novice

Skeletal muscle vulnerability in a child with Pitt-Hopkins syndrome.

BACKGROUND: TCF4 acts as a transcription factor that binds to the immunoglobulin enhancer Mu-E5/KE5 motif. Dominant variants in TCF4 are associated with the manifestation of Pitt-Hopkins syndrome, a rare disease characterized by severe mental retardation, certain features of facial dysmorphism and, in many cases, with abnormalities in respiratory rhythm (episodes of paroxysmal tachypnea and hyperventilation, followed by apnea and cyanosis). Frequently, patients also develop epilepsy, microcephaly, and postnatal short stature. Although TCF4 is expressed in skeletal muscle and TCF4 seems to play

A Comparison of Squat Depth and Sex on Knee Kinematics and Muscle Activation.

The squat is an essential exercise for strengthening lower body musculature. Although squats are frequently employed to improve lower extremity strength and neuromuscular control, differences between sexes and slight modifications, such as squat depth, can dramatically alter muscle recruitment and thus the foci of the exercise. The purpose of this study was to assess the effect of sex and squat depth on lower extremity coactivation and kinematics. Twenty recreationally active (female = 10) participants were recruited. The first visit consisted of one repetition maximum testing. For the second

The influence of physiologic and atmospheric variables on spasticity after spinal cord injury.

BACKGROUND: A number of physiological and atmospheric variables are believed to increase spasticity in persons with spinal cord injury (SCI) based on self-reported measures, however, there is limited objective evidence about the influence of these variables on spasticity. OBJECTIVE: We investigated the relationship between physiological/ atmospheric variables and level of spasticity in individuals with SCI. METHODS: In 53 participants with motor-incomplete SCI, we assessed the influence of age, time since injury, sex, injury severity, neurological level of injury, ability to walk, antispasmo

Echo intensity and muscle thickness as predictors Of athleticism and isometric strength in middle-school boys.

INTRODUCTION: We examined correlations among echo intensity and muscle thickness versus measures of athleticism and isometric strength in healthy children. METHODS: B-mode ultrasonography was used to examine the vastus lateralis (VL) and rectus femoris (RF) in 28 boys (mean age = 12 years). Tests of athleticism included jump height and peak velocity, sprint speed, and agility. Peak torque and the rate of torque development (RTD) at 100 and 200 ms from torque onset were assessed. RESULTS: Several significant correlations existed after controlling for age and mass. RF thickness showed the stro

Computational biomechanics of human knee joint in maximum voluntary isometric extension with focus on the role of joint center positioning.

Maximum voluntary isometric contraction (MVIC) extension assesses knee quadriceps function, strength, neuromuscular recovery and rehabilitation programs. We use a musculoskeletal (MS) model of the lower limb that incorporates a finite element (FE) of a cadaver knee. Muscle/ligament/contact forces, tissue stresses/strains and passive reaction moments are computed while simulating MVIC extension in seated posture at three knee flexion angles (KFA) (30°, 60°, 90°). Three input parameters of MVIC extension moment, lever arm of the posteriorly-directed external force, and coactivation in knee flexo

Acute femoral neuropathy secondary to an iliacus muscle hematoma.

We present a patient with a spontaneous iliacus muscle hematoma, appearing immediately after a minor physical maneuver, presenting with pain and femoral neuropathy initially evidenced by massive quadriceps muscle fasciculations. A magnetic resonance imaging (MRI) study of the pelvic area confirmed the diagnosis, showing a hematoma secondary to a partial muscle tear. The patient was managed conservatively, and the continuous muscle activity ceased in 3 days, with progressive improvement of the pain and weakness. The recovery was complete. Femoral neuropathy is uncommon and usually due to compre…

Predictive neuromuscular simulations of slip-perturbed sit-to-walk reveal the combined effects of quadriceps weakness and sensorimotor delay.

BACKGROUND: Sit-to-walk (STW) is an unstable transition in which age-related quadriceps weakness and slowed sensorimotor processing may combine to elevate slip-related fall risk. This study quantified how vastus weakness and neural transmission delay, alone and together, affect stability and recovery mechanics during slip-perturbed STW. METHODS: We used predictive neuromuscular simulations of a sagittal-plane musculoskeletal model to impose a standardized slip-like disturbance (20 N anterior force to both heels for 0.1 s at t = 1.0 s) across a 5 × 5 factorial grid: vastus strength scaled to 1
