Radiation-associated valvular heart disease.

Therapeutic ionizing radiation, such as that used in the treatment of Hodgkin's lymphoma, can cause cardiac valvular damage that may take several years to manifest as radiation-associated valvular heart disease. Treatment can be complicated by comorbid radiation injury to other cardiac and mediastinal structures that lead to traditional surgical valve replacement or repair becoming high-risk. A representative case is presented that demonstrates the complexity of radiation-associated valvular heart disease and its successful treatment with percutaneous transcatheter valve replacement. The preva

Radiation-induced enzyme efflux from rat heart: sedentary animals.

Serum levels of lactate dehydrogenase, creatine kinase, and glutamate oxaloacetate transaminase show initial elevations within 12 hr of exposure to 2,000 rads of gamma-radiation to the thoracic region of rats. Significant decreases in heart muscle homogenate levels of these enzymes parallel initial elevations in the serum and may suggest that enhanced leakage of enzymes is a consequence of radiation injury to heart muscle. Insignificant alterations in mitochondrial glutamate oxaloacetate transaminase levels after exposure indicate that in vivo injury to the mitochondria from therapeutic levels

Multiple level injuries in pediatric spinal trauma.

BACKGROUND: The incidence of concomitant, particularly noncontiguous, spine injuries in the pediatric population has not been well described. There is a balance between limiting radiation exposure and not missing concomitant injuries; understanding of this risk of concomitant spine injuries in this population is important. We hypothesize that the rate of concomitant spinal injuries in children is similar to adults. METHODS: The trauma registry of a pediatric trauma center was queried for all patients who sustained spine injuries over a 10-year period. Patient demographics, presence of other i

Radiation-induced liver disease: beyond DNA damage.

Radiation-induced liver disease (RILD), also known as radiation hepatitis, is a serious side effect of radiotherapy (RT) for hepatocellular carcinoma. The therapeutic dose of RT can damage normal liver tissue, and the toxicity that accumulates around the irradiated liver tissue is related to numerous physiological and pathological processes. RILD may restrict treatment use or eventually deteriorate into liver fibrosis. However, the research on the mechanism of radiation-induced liver injury has seen little progress compared with that on radiation injury in other tissues, and no targeted clinic

[Thyroid disorders associated with external radiation in children and adolescents].

The effects of ionizing radiation on the thyroid have been studied for several decades, and nuclear accidents are the major source of information about the subject. There is an association of hypothyroidism, hyperthyroidism, thyroid nodules and cancer with radiation, but the threshold dose, mechanism of injury, and some risk factors have not been fully established. Children are more susceptible to thyroid injury caused by radiation and require prolonged follow-up after exposure. This issue is especially relevant nowadays, since a large number of people treated with radiation for childhood canc

Metabolomic changes in gastrointestinal tissues after whole body radiation in a murine model.

Exposure to ionizing radiation (IR) elicits a set of complex biological responses involving gene expression and protein turnover that ultimately manifest as dysregulation of metabolic processes representing the cellular phenotype. Although radiation biomarkers have been reported in urine and serum, they are not informative about IR mediated tissue or organ specific injury. In the present study we report IR induced metabolic changes in gastrointestinal (GI) tissue of CD2F1 mice using ultra-performance liquid chromatography (UPLC) coupled with electrospray time-of-flight mass spectrometry. Evaluation of a Novel Enhanced Radiation Protection Device in Interventional Neuroradiology: a Phantom Study.

INTRODUCTION: Scatter radiation during interventional neuroradiological (INR) procedures is potentially harmful to the operator and cath-lab staff. Enhanced radiation protection devices (ERPDs) aim to improve safety by minimizing scatter radiation in addition to standard and personal protective equipment (PPE). This study evaluated the efficacy of a novel ERPD in INR. METHODS: Radiation exposure of the main operator was estimated by measuring the local dose rate at the examination position. An anthropomorphic patient whole body phantom was used simulating a standard INR procedure. The relativ

Accidents of occupational exposure to ionizing radiation in radiology departments: risks, prevention, and management.

Occupational exposure to ionizing radiation in radiology departments is a rare but important concern, given the potential long-term health effects and the regulatory framework surrounding radioprotection. This review analyzes the main risks associated with exposure to X-rays, including cancer, cataracts, radiodermatitis, pregnancy risks, and reduced fertility, as well as the specific contexts in which accidents occur. The majority of significant exposure incidents are reported in interventional radiology, where proximity to the radiation source and prolonged fluoroscopy times increase operator

CURRENT ACHIEVEMENTS AND TOPICAL ISSUES IN RADIATION OPHTHALMOLOGY: POST-CHORNOBYL EXPERIENCE.

The Chornobyl disaster - a largescale nuclear accident that caused significant radiation exposure to large populations of people. The work of ophthalmologists who studied its consequences radically changed scientists' understanding of the effects of ionizing radiation on the organ of vision. Before the Chornobyl accident, it was widely believed that the organ of vision was relatively resistant to the effects of ionizing radiation. It was thought that the most likely effect of radiation exposure was radiation cataracts, which were considered a deterministic effect. The objective of this study is

ASSESSMENT ON THE INTERCHANGEABILITY OF PERSONAL EFFECTIVE DOSE ALGORITHMS IN FLUOROSCOPY-GUIDED INTERVENTIONS USING BLAND-ALTMAN ANALYSIS.

Long exposure to radiation from fluoroscopy-guided interventions (FGIs) can be detrimental to both patients and radiologists. The effective doses received by the interventional radiology staff after performing 230 FGIs in a year were assessed by using double dosimetry and five various algorithms. The Shapiro-Wilk test revealed normally-distributed data (p < 0.01), while the significant correlation coefficients between the effective doses ranged between 0.88 and 1.00. As for the Bland-Altman analysis, both Niklason and Boetticher algorithms strongly supported the absence of statistical signific

[Influence of radiation on survivability, behavior and neurochemical correlates of rats].

The aim of the work was to study the dependence of individual radiosensitivity of white and black rats on radiation. The rats under study were derived from a cross between black and white rats and called Ratus Ratus-Georgia. Comparative radiosensitivity of white (Wistar) and black rats was studied at a total exposure with sublethal and lethal doses (5, 7 and 9 Gy). Cumulative survival functions of rats, a spectrum of changes in some ethological parameters and content of serotonin and catecholamines in their brain structures were given as criteria of radiosensitivity. Survival rate of black and

Radiation-induced brachial plexopathy and hypofractionated regimens in adjuvant irradiation of patients with breast cancer--a review.

In order to increase the availability of adjuvant radiotherapy of breast cancer patients and make it more convenient and cheaper, in numerous cancer centres, the dose per fraction has been increased from 2 Gy to 2.25-2.75 Gy and the total dose has been decreased from 50 Gy to 40-45 Gy. The risk of developing any late complications after conventionally fractionated megavoltage radiotherapy is estimated to be below 1%. The aim of this review is to determine whether hypofractionated regimens increase the risk of damage to the brachial plexus. A review of the published literature shows that the us

Ionizing Radiation in Interventional Cardiology and Electrophysiology.

Fluoroscopy-guided procedures constitute a major part in the practice of cardiology. These procedures are also a source of human-made ionizing radiation. Although the benefits of performing the procedure surpass the radiogenic risks in most cases, the risks are not negligible. Exposure to ionizing radiation may lead to tissue injuries and potential increase in risk of cancer. Both patients and operating physicians are exposed to these risks in variable degrees. The institution of radiation safety practices alone significantly reduces radiation exposure. Beyond the interventional laboratory, in

Potential Radiation-Related Effects on Radiologists.

OBJECTIVE: The risk of injury associated with long-term occupational exposure to ionizing radiation is low for radiologists. The purpose of this article is to systematically review and inform radiologists about radiation-related effects to which they are potentially susceptible. CONCLUSION: Formal education and training on radiation safety and management, careful attention to good radiation protection habits, and continued emphasis on radiation management and the as low as reasonably achievable principle are recommended for all radiologists.

Oral administration amifostine-grafted chitosan nanodrug for intestinal radioprotection.

Ionizing radiation from accidental exposure or planned radiotherapy can lead to irreparable damage to the gastrointestinal tract, especially the small intestine. There has been an unmet clinical need to protect the intestine from radiation-induced injury till now. Herein, we develop a strategy for efficient intestinal radioprotection by an oral administration amifostine-grafted chitosan nanodrug. Specifically, amifostine is grafted onto carboxymethyl chitosan by a facile one-pot synthesis. The nanodrug not only preserves the pH-responsive behavior and excellent biocompatibility from chitosan,

Hyperbaric Oxygen Therapy for Suburethral Vaginal Mucosal Necrosis after Interstitial Irradiation for Recurrent Cervical Cancer.

Patients with gynecological malignancies can develop radiation injuries, such as cystitis, proctitis, and soft tissue necrosis which have approved indications for hyperbaric oxygen therapy (HBOT). A 76-year-old Japanese woman with vaginal recurrence of cervical cancer was treated with the high-dose rate interstitial brachytherapy. Twenty-one months after the irradiation, she developed radiation necrosis on the external urethral opening. Two cycles of HBOT were performed. HBOT consisted of delivering 100% oxygen for 60 minutes at 2.4 atmospheres absolute. Pressure exposure was performed once da…

Managing patient dose in interventional cardiology.

This paper introduces a methodology for managing the radiation delivered to individual patients, incidental to their fluoroscopically guided interventional procedures. Because rare, but devastating skin injuries have occurred, radiation should be monitored and managed as well as drugs or radiographic contrast media. Dose metrics such as the total dose delivered to a reference point (reference point dose) should be used for this purpose. Fluoroscopy time is a poor independent predictor of skin dose in an individual patient. The concept of a "significant dose" is introduced. This is a predetermi

AMPK phosphorylates NAMPT to regulate NAD+ homeostasis under ionizing radiation.

Radiation-induced oral mucositis is the most common complication for patients who receive head/neck radiotherapy. Nicotinamide adenine dinucleotide (NAD+) is vital for DNA damage repair under ionizing radiation, through functioning as either the substrate for protein poly(ADP-ribosyl)ation at DNA break sites or the cofactor for multiple DNA repair-related enzymes, which therefore can result in a significant consumption of cellular NAD+ during DNA repair. Mammalian cells produce NAD+ mainly by recycling nicotinamide via the salvage pathway, in which the rate-limiting step is governed by nicotin
