In a prospective investigation of 19 patients with traumatic (n = 11) and septic (n = 8) shock at risk of developing the adult respiratory distress syndrome (ARDS), serial chest films were monitored. Eight patients developed ARDS, 2 following traumatic shock and 6 following septic shock. Only 2 of these 8 patients exhibited radiographic abnormalities consisting in bilateral widespread infiltrates of the alveolar type. In the 2 patients ARDS had already developed before artificial ventilation with positive end expiratory pressure (PEEP) was instituted. In the other 6 patients, in whom ventilato

Endogenous opioid peptides are released in response to stressful situations, such as circulatory shock, both as hormones and as central and peripheral neurotransmitters. Naloxone, an opiate antagonist, improves cardiovascular function in a variety of animal models of shock caused by endotoxemia, hemorrhage, anaphylaxis, or spinal trauma. Administration of thyrotropin-releasing hormone (TRH) in supraphysiologic doses also has pressor effects in these shock models. Given acutely after injury, TRH improves recovery in models of spinal trauma; however, the experimental effects of TRH do not involv

Shock describes a group of circulatory syndromes, all of which result in generalized cellular hypoxia. This leads to the depletion of adenos … y (cardiogenic shock), afterload (septic shock) or combinations of these (for example, anaphylactic shock, traumatic shock and neurogenic shock). During shock, important physiological changes occur in the nervous, respiratory, renal and gastrointestinal systems, as well as in intermediary metabolism. Hypotension is not synonymous with shock, and emphasis should be …

Hypertonic-hyperoncotic solutions are a supplementary possibility in the acute treatment of hypovolemia. The main mechanism of action of this solution is, induced by the tonicity of the infusion (approximately 2400 mOsm/l), the transcapillary fluid shift from the intracellular and interstitial to the intravascular compartment. Additionally the fluidity of blood is improved (due to an increase in the diameter of the capillaries), the adherence of leucocytes to the endothelial wall is reduced, and the arteriolar vasomotion is restarted. Due to the available prehospital and clinical results it ca

The potential to be successfully resuscitation from severe traumatic hemorrhagic shock is not only limited by the "golden 1 hr", but also by the "brass (or platinum) 10 mins" for combat casualties and civilian trauma victims with traumatic exsanguination. One research challenge is to determine how best to prevent cardiac arrest during severe hemorrhage, before control of bleeding is possible. Another research challenge is to determine the critical limits of, and optimal treatments for, protracted hemorrhagic hypotension, in order to prevent "delayed" multiple organ failure after hemostasis and

In 69 patients, treated in the intensive care unit of the department of traumatology we have examined the blood amylase activity. We found, that traumatic shock produced an increase in serum-amylase. Hyperamylasaemia is strongly related to the duration of shock and is due to an increase in S-type amylase. The increase is probably caused by an increased production and not by decreased renal excretion. The mechanism of hyperamylasaemia and the source of it are discussed.
