Management of shoulder dystocia: trends in incidence and maternal and neonatal morbidity.

OBJECTIVE: To investigate trends in the incidence of shoulder dystocia, methods used to overcome the obstruction, and rates of maternal and neonatal morbidity. METHODS: Cases of shoulder dystocia and of neonatal brachial plexus injury occurring from 1991 to 2005 in our unit were identified. The obstetric notes of cases were examined, and the management of the shoulder dystocia was recorded. Demographic data, labor management with outcome, and neonatal outcome were also recorded for all vaginal deliveries over the same period. Incidence rates of shoulder dystocia and associated morbidity relat

Relationship between Shoulder Pain, Trunk and Lower Limb Pain in Overhead Athletes: A Systematic Review with Meta-analysis.

BACKGROUND: Forty-nine percent of overhead athletes suffer from shoulder pain. Throwing movements require the participation of all components of the kinetic chain to reduce risk for shoulder overuse. Thus, limited lower limb range of motion or weakness has been identified as a risk factor for shoulder pain in overhead athletes. PURPOSE: This systematic review aims to evaluate the association between shoulder, trunk, and lower limb pain in overhead athletes. STUDY DESIGN: Systematic Review and Meta-analysis. METHODS: A systematic review was conducted in the PubMed/MEDLINE, Science Direct and

Tennis injuries: prevention and treatment. A review.

When players are engaged in the sport of tennis, injuries may occur to the eyes, in the neck, to the shoulder and back, arm and elbow, wrist and hand, and feet. The key to prevention and treatment of these injuries is good coaching and a formal stretching and strengthening program. The drooped "tennis shoulder" of professionals and senior tennis players is a natural response to heavy use. Shoulder elevating exercises are useful when soreness is associated. The treatment of tennis elbow includes wrist extensor stretching, isometrics, and light weightlifting. When a player follows this program,

Infant shoulder sonography: technique, anatomy, and pathology.

BACKGROUND: In the infant, shoulder injury due to birth trauma and infection in joint and bone may be difficult to detect radiographically. Shoulder ultrasound provides a dynamic, noninvasive method of evaluation. OBJECTIVE: To develop a technique to evaluate the infant shoulder and to successfully demonstrate pathology. MATERIALS AND METHODS: Using a multiplanar, dynamic technique, 24 normal and 12 symptomatic infants (age range, 3 days to 9 months) were examined (49 shoulders). RESULTS: Sonographic findings were normal in 41 shoulders and abnormal in 8 shoulders. Abnormalities included su

Traumatic Asymmetric Bilateral Shoulder Dislocation: The First Case Report in Mexico.

INTRODUCTION: Asymmetric bilateral shoulder dislocation is an exceptionally rare pattern of glenohumeral injury characterized by simultaneous dislocation of both shoulders in different directions. Since its first description in 1985, only a limited number of cases have been reported worldwide. Most cases are associated with seizures or electrical injuries, while traumatic mechanisms are uncommon. Reporting additional cases is important to improve recognition and management of this unusual injury pattern. CASE REPORT: We report the case of a 39-year-old Hispanic male who presented to the emerg

Acromioclavicular Joint Reconstruction Using an Artificial Ligament With Palmaris Longus Autograft Augmentation.

Acromioclavicular joint disruption is one of the commonest shoulder injuries, particularly in the young male athletic population. There are several techniques described in literature to treat acute and chronic injuries, although there is no gold standard. In this Technical Note, we describe an open technique to treat high-grade acute and chronic acromioclavicular joint disruption using an artificial ligament along with palmaris longus graft augmentation to replicate the conoid and trapezoid ligaments.

Complications of shoulder dystocia.

Complications of shoulder dystocia are divided into fetal and maternal. Fetal brachial plexus injury (BPI) is the most common fetal complication occurring in 4-40% of cases. BPI has also been reported in abdominal deliveries and in deliveries not complicated by shoulder dystocia. Fractures of the fetal humerus and clavicle occur in about 10.6% of cases of shoulder dystocia and usually heal with no sequel. Hypoxic ischemic brain injury is reported in 0.5-23% of cases of shoulder dystocia. The risk correlates with the duration of head-to-body delivery and is especially increased when the duratio

Has the management of shoulder dislocation changed over time?

Anterior shoulder dislocation is a disabling injury affecting all ages, young and old alike. Recently, the treatment of traumatic shoulder dislocation has included immobilisation for varying periods of time followed by physiotherapy. This study is the first in this country to address the demographic data and recurrence rates of shoulder dislocation. Three hundred and eight patients (170 men and 138 women) were followed up for an average of 5.9 years. The most frequent mechanism of injury was a fall (65.66% of cases), and in 92.1% of the patients, the shoulder was reduced in the Emergency Depar

Management of anterior shoulder instability in patients aged 40 and older: a systematic review.

PURPOSE: Anterior shoulder instability (ASI) is a common shoulder condition that primarily affects adolescents and young adults. In older adults this represents a distinct condition with a distinct pathomechanism and complication profile. This review synthesizes the literature on ASI in patients 40 years or older, focusing on injury characteristics, treatment modalities, and clinical outcomes. METHODS: Three databases (PubMed, MEDLINE, Embase) were searched on October 18, 2025. Primary studies investigating operative or non-operative management of ASI in patients with onset of instability at

Batter's Shoulder: Clinical Outcomes and Return to Sport.

Background Batter's shoulder has been defined as an acute posterior subluxation of the lead shoulder during a baseball swing causing a traumatic tear of the posterior labrum. There are limited data correlating repair techniques with return-to-play information but none utilizing standardized outcome measures. The purpose of this study is to examine a case series of patients for postoperative return-to-play and obtain follow-up using standardized outcome measures. Methods We retrospectively identified 10 patients with a batter's shoulder injury. Patients were included if they met the criteria fo

Long-Term Outcomes of Glenohumeral Arthrodesis.

BACKGROUND: Glenohumeral arthrodesis is associated with a high rate of complications. Although patients experience reasonable pain relief and shoulder stability, they experience marked limitations in their upper-extremity function. The purpose of this study was to examine the clinical outcomes of glenohumeral arthrodesis. METHODS: Twenty-nine patients with 29 affected shoulders underwent primary glenohumeral arthrodesis between 1992 and 2009. Surgical indications included rotator cuff arthropathy and pseudoparalysis (n = 7), neurologic injuries (n = 12), chronic infection (n = 3), recurrent d

Simultaneous bilateral fractures of the hip following a grand mal seizure. An unusual complication.

Pathological fractures as a consequence of convulsive seizures without direct trauma occur in 0.3% of cases. Some lesions such as bilateral posterior dislocation of the shoulder, fracture-dislocation of the shoulder or fracture and/or dislocation of the hip strongly suggest a seizure as the aetiology. We report on a patient with simultaneous central dislocation of one hip with fracture of the contralateral femoral neck provoked by spontaneous seizures. Unrecognized associated injuries following a convulsion may result in long-term functional disability and legal consequences.

Infraspinatus muscle atrophy from suprascapular nerve compression.

Muscle weakness without pain may signal a nerve compression injury. Because these injuries should be identified and treated early to prevent permanent muscle weakness and atrophy, providers should consider suprascapular nerve compression in patients with shoulder muscle weakness.

The painful shoulder: part I. Clinical evaluation.

Family physicians need to understand diagnostic and treatment strategies for common causes of shoulder pain. We review key elements of the history and physical examination and describe maneuvers that can be used to reach an appropriate diagnosis. Examination of the shoulder should include inspection, palpation, evaluation of range of motion and provocative testing. In addition, a thorough sensorimotor examination of the upper extremity should be performed, and the neck and elbow should be evaluated.
