Can the results 6 months after anterior cervical decompression and fusion identify patients who will have remaining deficit at long-term?

PURPOSE: There is no knowledge if short-term outcome in patients after anterior cervical decompression and fusion (ACDF) can be used to identify which patients have remaining deficit in long term. This study investigates if 6-month outcome with a broad assessment after ACDF with a cervical intervertebral fusion cage can be a guide for the 3-years outcome. METHOD: A prospective study. Questions about background data, pain, numbness, neck specific disability, distress, sick leave, health, symptom satisfaction and effect of and satisfaction with surgery were asked 28 patients 3 years after ACDF.…

Factors predicting pain reduction in chronic back and neck pain after multimodal treatment.

OBJECTIVES: To determine whether treatment related pain reduction on the short- and long-term is predicted by different baseline variables, … els of pain severity, and high affective distress were important predictors. Variables such as sex, sick leave history, working status, accident, pain duration, and depressive symptoms demonstrated no predictive value. Short- and long-term pain outcome was equally predictable and predicted by almost the same variables. CONCLUSIONS: Patients who reported unc …

How do we offer clinical relief to patients with gastro-oesophageal reflux disease?

In gastro-oesophageal reflux disease (GORD), the majority of patients are endoscopy negative. However, symptoms can affect the quality of li … -line therapy. If one considers not only the direct cost of drugs, but also the indirect ones (e.g. sick leave days), it is reasonable to think that PPIs may be more cost-effective than H2RAs or prokinetics, at least when the disease is of moderate or severe intensity. In conclusion, with modern PPIs like lansoprazole, rapid and full resolution of symptoms i …

Change in physical workload and the risk of work disability due to musculoskeletal disorders: a register-based study of 1 703 935 workers in Sweden.

OBJECTIVE: This register-based study examined whether changes over 14 years in occupational physical workload (PWL) affected the risk of registered work disability due to a musculoskeletal disorder (MSD). METHODS: This study included 1 703 935 workers in Sweden in 2005. PWL exposure was measured using job-exposure matrices linked to registered occupations in 1990 and 2005; change in PWL was defined between categories (low, medium, high) in either direction. The outcome was cumulative incidence of 'work disability' (either long-term sickness of at least 2 years or disability pension) due to MS

The Cost-Utility of a Workplace Mental Health Program for Employees with Common Mental Disorders (FRIAA Project).

BACKGROUND: Common mental disorders (CMDs) are a leading cause of sickness absence and early retirement due to health problems. Early identification and treatment of CMDs may have a positive impact on prognosis and associated economic costs, including healthcare expenditure, lost productivity, and the societal burden. The effectiveness of workplace psychotherapeutic interventions has been studied, but information on cost-effectiveness in this population is lacking. AIMS OF THE STUDY: This study evaluates the cost-effectiveness of the FRIAA intervention for employees with CMDs compared to care

COVID-19-related sickness absence among 4,721 NHS staff in England and its relation with long COVID symptoms: findings from NHS CHECK.

BACKGROUND: Long COVID (LC) occurs when COVID-19 symptoms continue for 12 + weeks after the onset of infection. LC may have several impacts, including sickness absence. This study explored number of days off work due to COVID-19 infection among healthcare workers (HCWs) and predictors of long-term sickness absence (i.e., 4 + consecutive weeks off work). METHODS: Data were from the NHS CHECK survey and included 4,721 HCWs (19.6% of the cohort) who reported COVID-19-related symptoms and sickness absence at two follow-up periods, approximately 12 and 32 months post-baseline (baseline collected be

Work participation trajectories among 1,098,748 Finns: reasons for premature labour market exit and the incidence of sickness absence due to mental disorders and musculoskeletal diseases.

BACKGROUND: Early exit from paid employment is a notable public health and societal challenge. Previous research has largely focused on the relationships among variables instead of the relationships among individuals with different work participation history. Person-oriented methods enable to identify latent groups of individuals who are likely to follow similar development in their work participation over time. We thus aimed to identify work participation trajectories during early and midlife careers and their social determinants using large nationally representative data comprising over 1 mi

Can group-based reassuring information alter low back pain behavior? A cluster-randomized controlled trial.

BACKGROUND: Low back pain (LBP) is common in the population and multifactorial in nature, often involving negative consequences. Reassuring information to improve coping is recommended for reducing the negative consequences of LBP. Adding a simple non-threatening explanation for the pain (temporary muscular dysfunction) has been successful at altering beliefs and behavior when delivered with other intervention elements. This study investigates the isolated effect of this specific information on future occupational behavior outcomes when delivered to the workforce. DESIGN: A cluster-randomized

Association of olfactory dysfunction in chronic rhinosinusitis with economic productivity and medication usage.

BACKGROUND: Chronic rhinosinusitis (CRS) has significant impacts upon productivity, economic metrics, and medication usage; however, factors that are associated with these economic outcomes are unknown. METHODS: We evaluated olfactory dysfunction in 221 patients with CRS using the Questionnaire of Olfactory Disorders-Negative Statements (QOD-NS) and the 40-item Smell Identification Test (SIT) and assessed whether an association existed between these olfactory metrics and healthcare utilization, productivity, and medication usage over the preceding 90 days. RESULTS: After adjusting for CRS-as

Quality of life and work status in patients treated for neurogenic thoracic outlet syndrome via the microsurgical supraclavicular approach - an observational study.

OBJECTIVE: Neurogenic thoracic outlet syndrome (nTOS) is a diagnostic and therapeutic challenge. Recently, consensus statements were develop … median values of 3/3/4/3/3 before surgery and of 2/2/2/2/1 at FU. Patient absence from work due to sick leave decreased from 44% to 18% at FU (p = 0.02). CONCLUSION: Consensus-based patient management in nTOS led to a significant benefit in terms of pain relief and individual health-related QoL. After surgery, return to work increased significantly.

Integrative Music Therapy for Depression-related Disorders (iMTDep) - study protocol for a randomised controlled trial.

BACKGROUND: Depression, anxiety and work-related stress affect a significant number of adults disrupting their productivity, ability to function and attend work. These highly comorbid disorders tend to be difficult to treat effectively, and more information about integrative treatment approaches is needed. This study investigates the effectiveness of music therapy in treating depression-related disorders. METHODS/DESIGN: The intervention is targeted at people of 18-65 years of age who are in employment, in their studies, temporarily unemployed, on short-term sick leave or rehabilitation allow…
