[Laparoscopic treatment of common bile duct calculi].

Laparoscopic exploration of the common bile duct is now technically possible. This prospective study evaluates the feasibility of the differ … uccessful in 85 (94.4%). The failures were treated by 3 laparotomies and 2 postoperative endoscopic sphincterotomy. The total success rate was 92.8% (130/140). The 5 local complications were pain (1), liver injury (1), and wound abscess (2), bleeding from a trocar site. Heart failure (medical treatment) (2), gastrointestinal haemorrhage from intestinal angiomas, …

Choledocholithiasis in a 4-month-old infant.

Cholelithiasis is increasingly being diagnosed in newborns and infants because of the more frequent use of abdominal ultrasound. However, common bile duct stones causing obstruction or symptoms in infancy are exceedingly rare and are not often reported in the literature. We report the successful treatment of symptomatic choledocholithiasis in a 4-month-old patient with endoscopic retrograde cholangiopancreatography and endoscopic sphincterotomy.

Ampullary hamartoma: endoscopic diagnosis and treatment.

This study shows a patient who presented with intermittent biliary tract obstruction caused by ampullary hamartoma. Endoscopic retrograde cholangiopancreatography showed a large ulcerated papilla and dilated biliary ducts. Tissue diagnosis was established by a large particle biopsy obtained with a snare. The patient underwent a successful endoscopic sphincterotomy and has remained symptom free for 4 years.

Determining the Risk Factors of Complications Due to Endoscopic Retrograde Cholangiopancreatography.

Background and objective The effective use of endoscopic retrograde cholangiopancreatography (ERCP) has been on the rise in diagnosing and t … eatitis (7.2%), cholecystitis (5.5%), bleeding (3.9%), and perforation (1.1%). It was observed that sphincterotomy was associated with an increase in all complications. The incidence of cholangitis decreased in the presence of dilated bile ducts unrelated to obstruction. The increased incidence of pancreatitis was associated with the female gender, the use of sp …

[Acute pancreatitis in children caused by gallstones].

A 10 year-old girl presented with acute abdominal pain. Serum-amylase was 3959 U/l. Ultrasonography showed dilatation of both the common bile duct and the pancreatic duct and a high density area near the ampulla. Endoscopic retrograde cholangiography (ERC) verified a stone near the ampulla, which was removed after sphincterotomy. The girl recovered fully.

Portal biliopathy in a 13-year-old Asian girl: a case report and review of literature.

Portal biliopathy (PB) is a term used to describe biliary ductal and gallbladder wall abnormalities seen in patients with portal hypertension. The pathogenesis of PB is not well known. It has been postulated that external pressure of portal cavernoma and/ or ischemia may play a role. We report a case of a patient with PB presenting with obstructive jaundice that involved endoscopic sphincterotomy and stenting.

Successful treatment of postoperative external biliary fistula by selective nasobiliary drainage.

A 25-year old man presented with a high output external biliary fistula after an operation for a giant hydatid cyst of the liver. Endoscopic sphincterotomy was inadequate to close the fistula. A nasobiliary tube was selectively inserted into the leaking hepatic duct and bile was continuously aspirated. The fistula and the residual cavity healed completely. Details of the patients' management using this alternative technique, are discussed.

Duodenal pseudotumour: a silent impacted common bile duct calculus.

We report the case of a man with mild dyspeptic symptoms referred for barium meal, who was initially thought to have a duodenal tumour. Subsequent investigation showed this to be a pseudotumour caused by impaction of a gallstone in the distal common bile duct, and endoscopic sphincterotomy effected a cure. This readily treatable condition should be remembered when filling defects are demonstrated in the duodenal loop.

Anterior ectopic anus: a common cause of constipation in children.

Anterior ectopic anus is increasingly being recognized as a cause of constipation in infancy. Experience is reported with surgical treatment of 20 patients with anterior ectopic anus. This form of anal ectopia is associated with a syndrome of constipation that begins at birth, straining on defecation, and anterior displacement of an otherwise normal anus and sphincter. It is seen almost exclusively in females. Posterior anoplasty with sphincterotomy is curative.

Current status of endoscopic papillary balloon dilation for the treatment of bile duct stones.

BACKGROUND/PURPOSE: While endoscopic sphincterotomy (EST) is performed worldwide for the removal of common bile duct stones, many biliary endoscopists hesitate to regard endoscopic papillary balloon dilation (EPBD) as a standard procedure for treatment. Therefore, the aim of this review is to re-evaluate the status of EPBD for the treatment of common bile duct stones. RESULTS: A major benefit of EPBD is preservation of papillary function, which is not complete but may be greater than that after EST. The disadvantages of EPBD compared with EST are that EPBD is difficult to use for the removal …

[Suprapapilar puncture for biliary access to advanced cancer of the papilla and severe coagulopathy].

Biliary cannulation to perform endoscopic retrograde cholangiopancreatography (ERCP) may be difficult in patients with advanced papillary cancer, due to anatomical and technical reasons. Sphincterotomy may be contraindicated in severe coagulopathy. We report a recently described technique of suprapapillary puncture for biliary access with use of an Artifon's catheter for biliary access in a high-risk patient with coagulopathy and periampullary neoplasm.

[Intra-operative identification of the ostium of Wirsung's pancreatic duct after papillosphincterotomy].

PURPOSE: The aim of this study is to suggest a feasible method to find the ostium of the Wirsung's duct during sphincteroplasty of the Vater's papilla, in order to avoid post-operative complications such as acute pancreatitis. PATIENTS AND METHODS: A total of 27 patients were submitted to sphincteroplasty for choledocolithiasis with or without Odditis. After therapeutic papillotomy and sphincterotomy through the duodenun, the location of the ostium of the Wirsung's duct was determined and studied. After papillotomy, the Vater's papilla becomes an isosceles triangle and its measurements were m…

[Benign stenosis of the papilla (author's transl)].

The diagnostics of benign stenosis of the papilla is based on the preoperative endoscopic retrograde cholangiopancreatography (ERCP) and preoperative cholangiography and debimetry with diagnostic dilatation of the papilla. The high rate of positive results achieved with these difficult procedures necessitate their use. The therapy of reversible stenosis consists of the eradication of the responsible inflammatory focus (cholecystolithiasis and choledocholithiasis). Irreversible stenoses of the papilla are without exception successfully treated with sphincterotomy or papillotomy.

[Therapeutic problems in neurological bladder in the child (author's transl)].

On the basis of a retrospective review of 116 cases of neurological bladder in children, consisting essentially of patients with spina bifida and meningomyelocoele, the authors consider their present attitude to this problem. They take a simple neurological classification as a basis, and the various forms of treatment (cutaneous ureteric transplantation, physiotherapy, divulsion, sphincterotomy, anti-reflux plastic procedures, treatment of incontinence) are reviewed in terms of method, indications and results.

Common bile duct calculi. 2. Nonsurgical therapy.

Several nonsurgical methods of therapy are available for treatment of retained common bile duct calculi. These include percutaneous extraction, endoscopic extraction, dissolution, and endoscopic sphincterotomy. The method chosen depends on location and size of calculi, size of sinus tract, patient age, surgical risks, and other factors. In most cases, procedures can be carried out safely and successfully with few or no complications.

[Out-patient treatment of periproctitic abscesses (author's transl)].

A total of 237 patients were treated over a period of five years for anorectal abscesses. Wide circular opening with removal of all purulent foci and necroses from outside had been preferred, because almost only "late forms" of anorectal inflammatory processes (perianal and ischiorectal abscesses) were seen. Sphincterotomy was possible in only a few patients. Postoperative fistulae occurred in 14% of the patients.

Rectal foreign body of a cosmetic bottle treated successfully by transanal retrieval: A case report.

RATIONALE: Retained rectal foreign bodies (RFBs) are unusual clinical presentations whose management is challenging for emergency physicians … hy revealed a foreign body stuck in the rectum. INTERVENTIONS AND OUTCOMES: After lateral internal sphincterotomy, the cosmetic bottle's plastic cap was pinched using a towel clamp and rotated slowly. The patient had an uneventful recovery period; the difficult defecation and anal bulging were relieved. LESSONS: This case proves that lateral internal sphincter …
