For functional treatment of acute ankle sprain after surgical or nonsurgical management various special shoes and braces are in use. For a comparison under functional conditions we applied controlled inversion movements of 20 and 30 degrees to 13 subjects. This study investigated the effect of anesthesia of the peroneal communis nerve on stress diagnostics in acute ankle ligament rupture. Using a special holding device, the anterior drawer sign was measured under loading of 4 to 15 kg with and without a peroneal nerve block. In 65 patients, we found a highly significant difference between these two conditions. The effect of low-power laser therapy on acute ankle sprains was evaluated in a double-blind randomised clinical study consisting of 40 patients from the casualty ward. All patients received treatment until their ankle joint was painless. No statistically significant differences regarding discolouring, pain, oedema, and use of analgetics were observed between patients treated with laser and placebo. The patients treated with active laser had a significantly longer sick leave.

Acute posttraumatic (Sudeck's) atrophy of the upper extremity is well known. The symptoms and signs are out of proportion to what is to be expected from the inciting event. Typically, bone atrophy is characterized by a focal, patchy, periarticular demineralization pattern, which is usually transient. Remineralization occurs after successful treatment. This report describes a typical case of the clinical syndrome after a wrist sprain but with carpal dissolution and collapse before remineralization.

We studied the late outcome of 40 ankles (from a consecutive series of 42) treated by a modified Evans procedure. The peroneus brevis tendon was used to fashion a static tenodesis. All the patients had suffered from persistent lateral instability following an ankle sprain. The follow-up period was between nine and 12 years. In an open multicentre study, 410 patients presenting with traumatic joint distortions, predominantly involving the ankle, were recruited for treatment with 50 mg diclofenac potassium given orally three times daily for 7 days. In a double-blind between-patient study the efficacy of diclofenac potassium, a non-steroidal anti-inflammatory drug, was assessed in 93 patients with mild to severe sprained ankles; patients with more severe sports injuries were excluded. A diagnosis of wrist sprain is no longer appropriate. With a proper history and careful physical examination, a specific problem can be suggested. Aids, including motion radiographs, special views, arthrography, technetium-99m scans, and/or tomography, can usually confirm the diagnosis. Only if specific injuries are identified can proper understanding and treatment of the injury be achieved.

A review of charts and x-ray films of individuals who sustained ankle sprains was carried out to determine the presence of calcaneonavicular coalition. Coalitions were present in 63% of 215 patients with 223 ankle sprains. In 130 ankle sprains recorded as occurring directly in athletics, 65% of the x-ray films showed some degree of calcaneal navicular abnormality. The trends in skiing injuries showed a decrease during a study conducted over six seasons, 1972 to 1978, at a Vermont ski resort. Over 11,000 interviews were conducted and nearly 1,000 control skiers were evaluated. The injuries totaled 1,711 in 407,600 skier days. This is an anomaly of the peroneus brevis in a 51-year-old woman that seems not to have been reported previously. Significant functional disability followed a seemingly innocuous ligamentous sprain of the ankle. A preclinical and clinical study of ankle arthrography for chronic ankle instability was performed. In the first study, ankle arthrograms were done on 50 cadaver ankles to demonstrate the limits of the normal ankle joint, and it was demonstrated that contrast in tendon sheaths is not a sign of a torn ankle ligament.
