Study of clinical and radiological profile and outcome in patients of intracranial hemorrhage.

BACKGROUND: Incidence of intracerebral hemorrhage (ICH) is twice as high as in Western countries. Prognostic factors for predicting function … ignificantly associated with worst clinical outcome. Mortality was high if the patient was comatose/stuporous compared to drowsy state (P < 0.0001). Mortality was found to be high when the size exceeded 30 cm3. High ICH score, low GCS score at the time of admission, presence of intraventricular hemorrhage, and midline shift were significantly associated …

Posterior Reversible Encephalopathy Syndrome in Henoch-Schonlein Purpura and Hemolytic Uremic Syndrome.

Posterior reversible encephalopathy syndrome (PRES) is a clinico-radiological syndrome, composed of symptoms such as headache, seizures, visual disturbances, lethargy, confusion, stupor, focal neurologic findings and radiological findings of bilateral gray and white matter abnormalities suggestive of edema in the posterior regions of the cerebral hemispheres. PRES is associated with significant morbidity and mortality if it is not expeditiously recognized. Magnetic resonance image (MRI) represents the most sensitive imaging technique for recognizing PRES. PRES has been seen in various clinical…

[Reversible changes on MR images in a patient with metronidazole-induced encephalopathy].

A 66-year-old woman was diagnosed with a brain abscess. The abscess was drained by sterotactic catheter insertion. She was administered metronidazole at a dose of 2 g/day. On the 30th day of treatment, she had nausea that gradually progressed. On the 45th day, she developed a disturbance of consciousness and was admitted to our department. She was in stuporous state, and had slight vestibular and cerebellar dysfunctions. Diffusion-weighted and FLAIR brain MR images showed bilateral symmetrical high signals in the splenium of the corpus callosum (SCC), cerebellar dentate nucleus, and inferior c…

The role of deprivation, abuse, and trauma in pediatric catatonia without a clear medical cause.

OBJECTIVE: Catatonia is considered a unique syndrome of motor signs, at times life-threatening when aggravated by autonomic dysfunction and … c depression, a condition found by Spitz in deprived institutionalized children, strongly resembles stuporous catatonia. Leonhard considered lack of communication with the mother or substitute mother as an important risk factor for childhood catatonia. Children including those with autism who experience emotional and physical trauma sometimes develop cat …

[Diagnosis of delirium].

Delirium is a clinical diagnosis, which is based on observed disturbances of consciousness and cognitive dysfunction. Associated neuropsychiatric and psychomotoric symptoms are common. Many elderly delirious patients are hypoactive, manifesting a passive demeanor, reduced activity and, in extreme cases, stupor and coma. Delirium has an acute onset and a fluctuating course. It is usually reversible. It is crucial to examine cognitive functions among elderly hospitalized patients, since advanced age and preexisting dementia are important risk factors for the development of delirium. In patients …

[Patients with alteration of consciousness in the emergency department].

A subject is conscious when he is awake and with an adequate awareness of him and the environment. The term alteration of consciousness requires specification as to whether it defines alteration of arousal--when the patient might be confused, in a stupor or in some degree of coma (light, deep)--or alteration of awareness--that is, confused (spatio-temporally disoriented, with difficulty in maintaining his attention), with or without delirious ideation. The coma, in the strict sense, originates from structural (neurological) or functional (metabolic) dysfunction of the ascending reticular activ…

[Sleep disorders in prion diseases].

Prion diseases are a group of encephalopathies with neurodegenerative changes caused by an altered protein named prion whose characteristic datum is transmissibility. In most cases they occur in a sporadic form although a group of them are familial associated with mutations in the gene of the prion protein. Genetic polymorphism seems to determine the different family variants. One of the most enigmatic and unusual is Fatal Familial Insomnia (FFI), a hereditary disorder characterised by loss of physiological sleep with oneiric stupor, autonomic and motor hyperactivity, and motor anomalies. The …

[Methylmalonic aciduria associated with myoclonic convulsions, psychomotor retardation and hypsarrhythmia].

INTRODUCTION: Organic acidurias have long been known to cause neurological problems, such as convulsions, stupor, coma, and psychomotor and mental retardation. The organic acidurias include propionic aciduria, methylmalonic aciduria (MMA), isovaleric acidemia, lactic acidemia and glutaric acidemia type I. However, the association of MMA with electrical activity of the brain characterised by a hypsarrhythmic pattern, refractory convulsions and psychomotor retardation is very rare. CASE REPORTS: Two patients, one male and one female, were seen to have psychomotor retardation, erratic attacks of…

The neurological involvement in Manson's Schistosomiasis is rarely reported in the literature. The authors describe a case of a 23 years old patient born in São Francisco-MG. She presented with headache, vomiting, fever and stupor. The neurological examination showed stiffness of the neck, paresis with bilateral muscular hypertonia in the upper members, and loss of motricity in inferior members. There was no sign of hepatosplenomegaly. Cerebrospinal fluid examination showed: a decrease in the glucose content (28 mg/dl), increase in the protein content (113 mg/dl) and an increase in cell number…

Spontaneous Reye's-like syndrome in BALB/cByJ mice.

In five spontaneous outbreaks, sixty-four BALB/cByJ mice developed Reye's-like syndrome 4 to 33 days after introduction into mouse rooms known to harbor a variety of indigenous murine viruses. The clinical course lasted 24 hours and consisted of progressively deteriorating consciousness and hyperventilation, usually leading to death. Mice killed and necropsied while stuporous or comatose had serum ammonia levels of 2524 +/- 179 versus 66 +/- 8 micrograms/dl for control BALB/cByJ mice. Necropsy findings included swollen, diffusely pale yellow livers with panlobular microvesicular fatty change, …

Traumatic spreading depression syndrome. Review of a particular type of head injury in 37 patients.

From 1476 hospital admissions with head injury, 37 patients were selected on the basis of distinctive clinical features which appeared to share a single and benign aetiology. In head injuries of this type, transient neurological disorders resulted from trivial or rather mild head injuries. These disorders, which included headache, nausea and vomiting, pallor, somnolence, irritability and restlessness, stupor, hemiparesis and aphasia, appeared after a lucid interval which was usually less than two hours; then the patient either recovered, or went on to develop convulsive attacks. Recovery witho…

A Subcircuit in the Suprachiasmatic Nucleus Generates Wakefulness.

The daily cycles of sleep and arousal are among the most prominent biological rhythms under circadian control. While the role of the suprach … emogenetic activation and inhibition of mWAKE+ cells lead to a marked increase in wakefulness and a stupor-like phenotype, respectively. Optogenetic activation specifically of mWAKE-expressing neurons in the SCN (SCNmWAKE neurons) promotes arousal, while genetic knockout of mWAKE or impairment of CLOCK function in these neurons enhances wakefulness selec …
