Adding Teres Major Transfer to Arthroscopic-Assisted Posterior Latissimus Dorsi Transfer Provides Superior Outcomes for Posterosuperior Irreparable Rotator Cuff Tears.

PURPOSE: To compare the clinical and radiologic outcomes of arthroscopic-assisted latissimus dorsi transfer (LDT) and arthroscopic-assisted posterior latissimus dorsi and teres major (pLDTM) transfer in patients with posterosuperior irreparable rotator cuff tears (PSIRCTs). METHODS: We retrospectively reviewed patients with PSIRCTs who underwent LDT or pLDTM transfer between January 2017 and December 2020 and were followed up for at least 2 years. Clinical outcomes were evaluated based on pain, patient-reported outcome scores, active range of motion, and active range-of-motion strength. Radio

Minimally Invasive Subscapularis Release: A Novel Technique and Results.

BACKGROUND: Shoulder imbalance secondary to residual brachial plexus birth palsy requires release of internal rotation contracture and tendon transfer. Subscapularis is considered as the prime element of internal rotation contracture and various methods have been described for subscapularis lengthening. It includes open subscapularis slide or lengthening and arthroscopic release. We hypothesized that subscapularis can be released through minimally invasive approach from the medial border of scapula and thus avoiding formal open procedures and risk of weakening the internal rotation strength. …

The composite groin fascial free flap.

Seven patients with extensive loss of soft tissue, extensor tendon, and exposure of bone and joint of dorsum of hand and foot were treated with free composite flaps from the groin area. To retard the adhesion by scarring, membrane interposition between tendon and bone was recommended. The groin flap, based on superficial circumflex iliac vessels, was combined with a well-vascularized external oblique aponeurosis. The groin skin flap provided skin coverage, and the aponeurosis was used simultaneously to create a gliding surface for reconstructed tendons in a single one-stage procedure. All comp

Semitendinosus Tendon Autograft for Reconstruction of Large Defects in Chronic Achilles Tendon Ruptures.

BACKGROUND: Chronic Achilles tendon ruptures are associated with considerable functional morbidity. When treated operatively, debridement of degenerated tendon ends may create large defects. Various procedures to reconstruct large defects have been described. We present a simple technique in which an autologous semitendinosus tendon graft is used to reconstruct defects larger than 5 cm in chronic Achilles tendon ruptures. The purpose of this study was to describe our operative technique and its functional outcome. METHODS: Achilles ruptures of more than 6 weeks duration were considered for th

However, when joints that are motored by the ruptured tendon are still functional, tendon transfer or grafting may be considered. Because of the progressive nature of the disease, dislocation and end-stage arthritis often require stabilization with bony procedures. [Evaluation of reconstruction of extensor pollicis function by transfer of extensor indicis].

OBJECTIVE: To assess the long-time results of reconstruction of the extensor pollicis longus (EPL) function by transfer of the extensor indicis (EI). METHODS: From August 1978 to March 2003, 46 cases of loss of the EPL function were treated by transfer of the extensor indicis. Of 46 cases, there were 32 males and 14 females, aged 16-51 years with an average of 36 years; there were 24 cases of old traumatic rupture and 22 cases of secondary rupture. The disease course was 2 days to 5 months, averaged 74 days. A specific EI-EPL evaluation method (SEEM) was used to measure the EPL function after

Surgical treatment of lateral ankle instability syndrome.

Lateral ankle instability syndrome is defined by pain and instability caused by ligament laxity. Anterior talofibular ligament laxity, with or without calcaneofibular laxity and other abnormalities, is often present in conjunction with peroneus brevis tendon tears, abnormal ligament placement, tibialis posterior tendon tears, osteochondritis dissecans, arthritis, synovitis, loose bodies, and tarsal coalition. Surgical reconstructions were performed on 43 ankles using a split peroneus brevis tendon graft routed through osseous tunnels in the talus, fibula, and calcaneus and resutured to the rec

Current Concepts in Elbow Extension Reconstruction for the Tetraplegic Patient.

Individuals with C5 or C6 spinal cord injury (SCI) have paralysis of the triceps brachii, and the subsequent loss of elbow extension makes it impossible to reliably use their hands above shoulder level because of the inability to hold the elbow extended against gravity. For persons with cervical SCI, elbow extension can be restored with both tendon and nerve transfers. Elbow extension is necessary for dressing, eating, wheelchair locomotion, pressure relief maneuvers, independent transfers, and reaching objects above shoulder level. Deltoid-to-triceps and biceps-to-triceps tendon transfers hav…

Interosseous Membrane Reconstructions for Longitudinal Radioulnar Instability: A Cadaveric Comparison.

BACKGROUND: Multiple options exist for reconstruction of the interosseous membrane (IOM) including biologic and nonbiologic materials. In this study, we present the biomechanical properties of reconstruction of the IOM using suture button suspension with and without pronator teres (PT) tendon rerouting. METHODS: Using 8 cadaveric specimens, radioulnar displacement was tested for the following successive conditions: intact specimen, IOM release with radial head (RH) resection, IOM reconstruction with PT tendon transfer alone, and PT transfer augmented with a suture button suspension construct …

Acute Flaccid Myelitis: A Single Pediatric Center Experience From 2014 to 2019.

BACKGROUND: Acute flaccid myelitis has emerged as the leading cause of acute flaccid paralysis in children. Acute flaccid myelitis leads to significant physical disability; hence, objective outcome measures to study disease severity and progression are desirable. In addition, nerve transfer to improve motor function in affected children needs further study. METHODS: Retrospective study of acute flaccid myelitis subjects managed at Children's Healthcare of Atlanta from August 2014 to December 2019. Clinical, electromyography and nerve conduction study, neuropsychological functional independenc

[Clinic applications of vascularized plantaris tendon grafting].

OBJECTIVE: To study a new method of the vascularized plantaris tendon transferring. METHODS: The vascularized plantaris tendon was elevated with the pedicle of the fascial perforating vessel or a small segment of the posterior tibial artery, which was transplanted to the wrist to repair the tendon defects of the flexor pollicis longus or the flexor digitorum muscle resulted from electric injury. According to the defects, the plantaris tendon can be used together with the skin, fascia, or both as a composite flap. 7 cases received the operation with complete survival of the flap. RESULTS: Pos…

Bipolar transfer of the pectoralis major muscle for restoration of elbow flexion in 29 cases.

BACKGROUND: This study evaluated the functional outcomes of bipolar pedicled pectoralis major (PM) transfer to restore elbow flexion. METHODS: We retrospectively reviewed 29 transfers in 28 patients with a mean age of 31.2 years (range, 5-65 years). The loss of elbow flexion was due to brachial plexus palsy in 24 patients, elbow flexors necrosis in 4, and poliomyelitis in 1. The entire PM muscle was mobilized and fixed proximally to the coracoid process. Intraoperative positioning and postoperative immobilization of the shoulder and the elbow flexed at 60° and 120°, respectively, allowed dire

Flexor Tendon Rupture After Distal Radius Malunion Associated With Electromyographic Abnormalities: A Case Report.

CASE: A 63-year-old woman with a history of a malunited distal radius fracture presented with left hand pain and inability to flex her thumb, index, and middle fingers. Anterior interosseous neuropathy was suggested because of abnormal electromyography findings. However, magnetic resonance imaging later revealed flexor tendon ruptures. A distal radius corrective osteotomy with autograft and volar fixation was performed with tendon transfers and carpal tunnel release. CONCLUSIONS: Attritional flexor tendon rupture after a nonoperatively managed distal radius fracture is rare but remains an imp…

A posterior interosseous nerve syndrome associated with spontaneous rupture of the extensor pollicis longus tendon.

Posterior interosseous nerve entrapment syndrome and spontaneous rupture of the extensor pollicis longus tendon are rare conditions. The authors describe the bizarre combination of a spontaneous rupture of the extensor pollicis longus tendon in a 82-year-old lady with a posterior interosseous nerve syndrome. As far as the authors know, this is the first description of such an association in the literature. Surgical exploration revealed compression of the posterior interosseous nerve at the proximal portion of the supinator muscle and at Henry's leash. The nerve was freed, and the tendon of the

[Reconstruction of quadriceps femoris muscle function with muscle transfer].

Femoral nerve palsy, mostly of iatrogen cause, leads to paresis of quadriceps muscle with complete loss of knee extension. Therapeutical options include neurolysis, nerve reconstruction or functional muscle transplantations. Another concept is the transfer of hamstring muscles as described in post polio surgery. We describe our experience of biceps femoris and semitendinosus muscle transfer for reconstruction of knee extension. From 2003 to 2007 seven patients (mean age 43) with complete loss of knee extension after femoral nerve lesion were treated. Nerve palsy was caused by direct lesion, tr
