Current Transition Practice for Primary Immunodeficiencies and Autoinflammatory Diseases in Europe: a RITA-ERN Survey.

BACKGROUND: Due to the absence of curative treatments for inborn errors of immunity (IEI), children born with IEI require long-term follow-up for disease manifestations and related complications that occur over the lifespan. Effective transition from pediatric to adult services is known to significantly improve adherence to treatment and long-term outcomes. It is currently not known what transition services are available for young people with IEI in Europe. OBJECTIVE: To understand the prevalence and practice of transition services in Europe for young people with IEI, encompassing both primar

Pain management in a patient with intestinal failure in the palliative care setting: case report.

BACKGROUND: Managing pain in patients with incurable intestinal failure requires a multidisciplinary approach that addresses complex pharmacological and systemic challenges while navigating prognostic uncertainties. CASE PRESENTATION: This case report of gastrointestinal hypoganglionosis demonstrates the use of subcutaneous ketamine when conventional opioids fail due to intestinal malabsorption and dysmotility. Effective care required interdisciplinary collaboration, yet persistent discordance between the patient’s curative expectations and her life-limiting prognosis hindered timely advance c

Modelling collaboration of primary and secondary care for children with complex care needs: long-term ventilation as an example.

Children dependent on long-term ventilation need the planning, provision and monitoring of complex services generally provided at home by professionals belonging to different care settings. The collaboration among professionals improves the efficiency and the continuity of care especially when treating children with complex care needs. In this paper, the Unified Modelling Language (UML) has been adopted to detect the variety of the patterns of collaboration as well as to represent and compare the different processes of care across the 30 EU/EEA countries of the MOCHA project. Conclusion: Half o

Practical Considerations for Managing Transitions from Parenteral Prostacyclins to Oral Selexipag in Pulmonary Arterial Hypertension.

The prostacyclin pathway represents a foundational pathway in the treatment of patients with pulmonary arterial hypertension (PAH). Parenteral prostacyclin pathway agents (PPAs) are often used for treatment of severe PAH but their broader use is limited by administration difficulties, including a need for continuous infusion, administration side effects, and complexities of a chronic indwelling catheter. Administration of the prostacyclin receptor agonist selexipag, an oral therapy with more than 10 years' clinical experience, may be more favorable for some patients. However, treatment protoco

Development and validation of the HIV adolescent readiness for transition scale (HARTS) in South Africa.

INTRODUCTION: Adolescents living with perinatally acquired HIV have low rates of retention in care and viral suppression after the transition from paediatric to adult care. In this study, we developed and validated a tool to identify adolescent transition readiness. METHODS: We developed the HIV Adolescent Readiness for Transition Scale (HARTS) from June 2016 to May 2019 by iteratively adapting existing transition readiness scales for other chronic illnesses by conducting focus groups with 11 healthcare providers and 20 adolescents in South Africa. We administered a preliminary questionnaire

Differences in Perspectives and Policies Regarding End-of-Life Care Between Hematologists and Gastroenterologists.

BackgroundPatients with hematologic malignancies often receive aggressive treatment until the terminal phase and transition to end-of-life (EOL) care less frequently than those with solid tumors. A 2015 survey compared hematologists' and solid tumor-oncologists' attitudes toward EOL care, revealing differences. A decade later, no similar reports have appeared despite therapeutic advances. Therefore, we repeated the survey with additional questions to examine whether perspectives on EOL care have changed.MethodsA 50-item questionnaire was administered to hematologists and gastroenterologists. [Ecological transition process in healthcare: certification as a push to progress].

The healthcare system accounts for 5 to 8% of greenhouse gas emissions. Since the 2010s, several institutions have been calling on healthcare systems to reduce their carbon footprint without compromising the quality of care. This article describes an ecological transition process in healthcare at the Geneva University Hospitals, in the form of a certification program for "eco-care" units, driven by frontline professionals' commitment to human and planetary health, comprising both an educational and an operational component. The certification process mobilizes units around a shared transition p

Janus at the Crossroads: Perspectives on Long-term Care Trajectories for Older Women With Dementia in a Canadian Context.

PURPOSE OF THE STUDY: Janus, the two-faced, Roman god of beginnings and transitions, is used as a metaphor to explore our personal narratives and our quantitative research on the experiences of older women with dementia in long-term care (LTC). Two research questions are addressed: (a) How do our quantitative data map onto our mothers' experiences? (b) What lessons do our mothers' experiences offer for the care of older women with dementia? DESIGN AND METHODS: Informed by a life-course perspective, we triangulate administrative data on 3,717 women with dementia receiving LTC in British Columb

Multidisciplinary management of Duchenne muscular dystrophy from childhood to adulthood.

Duchenne muscular dystrophy (DMD) is a severe, progressive neuromuscular disorder characterized by muscle degeneration and weakness, leading to significant physical disability and multisystem complications. The management of DMD requires a coordinated, multidisciplinary approach to address the evolving needs of patients from childhood through adulthood, with a focus on patient quality of life. Early diagnosis and proactive interventions are critical to optimizing outcomes and may include physical therapy, respiratory support, cardiac monitoring, and orthopedic care. In children, the focus is o

Executive Functioning Predicts Adaptive Functioning and Self-Care Independence in Pediatric Sickle Cell Disease.

OBJECTIVE: This study examined the impact of executive functioning (EF) on adaptive functioning and self-care independence in pre-transition youth with sickle cell disease (SCD). METHODS: Thirty-four youth with SCD (ages 12-18 years) completed a brief neuropsychological battery and their caregivers completed questionnaires assessing adaptive functioning, EF behaviors, and independence in completing self-care tasks, including disease management. Hierarchical linear regression analyses were utilized to investigate if EF predicted significantly more variability in adaptive functioning and self-c

Longitudinal association between panic disorder and health care costs in older adults.

BACKGROUND: To analyze whether probable panic disorder (PD) is associated with health care costs in older age over time. METHODS: Data regarding individuals aged 65 and over were derived from two waves of the ESTHER cohort study (nt1  = 2,348, nt2  = 2,090). Probable PD was assessed using the panic screening module from the Patient Health Questionnaire. Health care costs were obtained through monetary valuation of self-reported health care use data. Fixed effects regressions analyzed the association between transitions in probable PD status and change in health care costs, while adjusting for

Association of continuity of care with treatment adherence and quality of life in patients with chronic kidney disease and hyperkalemia: A retrospective study.

This retrospective study evaluated whether a continuity-of-care nursing model was associated with improved adherence and quality of life in patients with chronic kidney disease complicated by hyperkalemia. Medical records of patients hospitalized between January 2021 and December 2023 were reviewed. Based on documented discharge planning and postdischarge follow-up intensity, patients were categorized into a control group receiving routine education and standard follow-up, and a experimental group receiving enhanced transitional and postdischarge nursing. The continuity-of-care approach incorp

The longitudinal relationship between the use of long-term care and depressive symptoms in older adults.

PURPOSE: The aim of this study was to estimate the longitudinal relationship between transitions in the use of long-term care and older adults' depressive symptoms and to investigate whether this relationship could be explained by markers of older adults' underlying health, or other variables including demographics, personality, and partner status. DESIGN AND METHODS: Data were from the Longitudinal Aging Study Amsterdam, which consists of a random, community-based sample of 3,107 older Dutch people (55-85 years of age) stratified by age and gender. The use of informal care, professional home

Filling in the helper-gap: the intentions of frail older widows.

Despite marked interest in continuity of care and transitions experienced by older persons, there is little information available about the intentions of older women regarding changes that occur in their support networks. This article reports the findings of a descriptive phenomenological study of older widows' experience of home care and describes the experiences of 10 women who lost a key helper during the 3-year study. Compared with theories of continuity of care or transition, the findings are in keeping with the focus of nonequilibrium systems theory: bringing order out of disorder. Findi

Impact of a phased transition model on Advanced HIV Disease outcomes: A pre- and post-implementation evaluation study in Malawi.

To promote sustainability and strengthen national ownership of Advanced HIV Disease (AHD) services, a transition was implemented across 22 health facilities in Central Malawi. This transition involved shifting responsibility for key AHD program elements from partner-led implementation to full Ministry of Health (MoH) leadership. This evaluation assessed the impact of this transition on diagnostic coverage, TB preventive therapy (TPT) uptake, and 12-month survival outcomes. A retrospective cohort study included all children and adults enrolled in AHD care during the pre-MoH transition (January

Survivorship preparedness and activation among survivors of lymphoma.

OBJECTIVES: Taking an active role in managing post-treatment care has emerged as a key aspect of promoting a successful transition into survivorship and is associated with better patient outcomes. In this study, we focus on two key aspects of active self-management, activation and preparedness. Activation was defined as understanding one's role in the care process and having the knowledge, skill, and confidence to take on a role in managing self-care. Preparedness was defined as the extent to which individuals perceived they had sufficient information about what to expect after cancer treatmen
