Trauma ICU Prevalence Project: the diversity of surgical critical care.

BACKGROUND: Surgical critical care is crucial to the care of trauma and surgical patients. This study was designed to provide a contemporary assessment of patient types, injuries, and conditions in intensive care units (ICU) caring for trauma patients. METHODS: This was a multicenter prevalence study of the American Association for the Surgery of Trauma; data were collected on all patients present in participating centers' trauma ICU (TICU) on November 2, 2017 and April 10, 2018. RESULTS: Forty-nine centers submitted data on 1416 patients. Median age was 58 years (IQR 41-70). Patient types i

Mortality rates following trauma: The difference is night and day.

BACKGROUND: Although most medical centers are equipped for 24-h care, some "middle of the night" services may not be as robust as they are during daylight hours. This would have potential impact upon certain outcome measurements in trauma patients. The purpose of this paper was to assess the effect of patient arrival time at hospital emergency departments on in-hospital survival following trauma. MATERIALS AND METHODS: Data of patients, 18 years of age or older, with no evidence that they were transferred to or from that center were obtained from the National Trauma Data Bank Version 7.0. Variations in rates of tracheostomy in the critically ill trauma patient.

OBJECTIVE: The utility of tracheostomy to expedite weaning and prevent complications in patients with acute respiratory failure is actively debated, with many physicians holding strong opinions regarding the value and timing of this intervention. We postulated that these opinions would be reflected in significant variation in tracheostomy rates across centers. Thus, we set out explore the extent and potential sources of this variation among injured patients cared for in trauma centers in the United States. DESIGN: This is a retrospective cohort study. We used stratification and hierarchical m…

Evaluating the Regional Differences in Pediatric Injury Patterns During the COVID-19 Pandemic.

INTRODUCTION: Reports of pediatric injury patterns during the COVID-19 pandemic are conflicting and lack the granularity to explore differences across regions. We hypothesized there would be considerable variation in injury patterns across pediatric trauma centers in the United States. MATERIALS AND METHODS: A multicenter, retrospective study evaluating patients <18 y old with traumatic injuries meeting National Trauma Data Bank criteria was performed. Patients injured after stay-at-home orders through September 2020 ("COVID" cohort) were compared to "Historical" controls from an averaged per…

Protection from traumatic brain injury in hormonally active women vs men of a similar age: a retrospective international study.

BACKGROUND: It has been suggested that women with traumatic brain injury have more favorable outcomes than do men because of higher levels of circulating estrogen and progesterone that may reduce brain edema. OBJECTIVES: To determine whether there is any association between sex and mortality in TBI patients and whether there is any association between sex and brain edema. DESIGN: Retrospective cohort study using data from 2001 to 2007 collected from a trauma registry in Hong Kong and the Victorian State Trauma Registry. SETTING: Two regional trauma centers in Hong Kong and 2 adult major tra…

Learning gain of an ATLS®-based interprofessional and multidisciplinary in-situ simulation training of trauma resuscitation.

PURPOSE: Team performance in polytrauma management determines patient outcome and is crucially shaped by Crew Resource Management (CRM). This study explored patterns of self-perceived learning following an interdisciplinary, interprofessional, in-situ, simulation- and ATLS®-based trauma team training with a focus on CRM principles. METHODS: We conducted a retrospective analysis of routine educational data collected immediately after 36 training sessions (03/2022–11/2023) including 238 participants at a single Level I trauma center. Participants completed a retrospective pre-test/post-test self

Injury patterns and epidemiology of orthopedic trauma in polytrauma ICU patients: A 10-year retrospective analysis at major trauma hospital.

BACKGROUND: Polytrauma requiring intensive care remains a leading cause of morbidity and mortality, particularly among patients with orthopedic injuries. Despite advances in trauma systems and surgical management, outcomes for this cohort are influenced by complex interactions between injury burden, physiological status, and pre-existing comorbidities. There remains limited evidence focused specifically on predictors of outcome within orthopedic trauma patients admitted to the intensive care unit (ICU). METHODS: This retrospective cohort study analyzed all adults (≥18 years) admitted with ort

A Multicenter Analysis on the Effect of the US-Mexico Border Wall Extension on Neurological Trauma.

BACKGROUND AND OBJECTIVES: The California-Mexico border wall height extension has been associated with increased rates of traumatic injuries. We sought to determine whether the US-Mexico border wall fortifications are associated with increased rates and severity of neurological injuries after falls and increased hospital resource utilization across the Southwestern border. METHODS: Patients who presented to UC San Diego, Banner University of Arizona, or South Texas Health System trauma center after a fall from the US-Mexico border wall between 2016 and 2022 were identified and categorized as

Results of an adapted whole-body MRI in the primary care of children and adolescents in the trauma room.

BACKGROUND: In diagnostic imaging as part of pediatric polytrauma management radiation exposure and diagnostic benefit of a whole-body computed tomography must be weighed up against each other. Performing an adapted polytrauma magnetic resonance imaging (MRI) could be a sensible alternative in some cases. The aim of this study was to show what findings are made in the MRI and if this has consequences for further diagnosis and therapy. METHODS: We performed a retrospective evaluation of the adapted polytrauma MRI examination as part of the primary survey in trauma room care of children and ado

Prehospital Time Following Traumatic Injury Is Independently Associated With the Need for In-Hospital Blood and Early Mortality for Specific Injury Types.

OBJECTIVE: Treating traumatic hemorrhage is time sensitive. Prehospital care and transport modes (eg, helicopter and ground) may influence in-hospital events. We hypothesized that prehospital time (on-scene time [OST] and total prehospital time [TPT]) and transport mode are associated with same-day transfusion and mortality. Furthermore, we sought to identify regions of anatomic injury that modify the relationship between prehospital time and outcomes in strata corresponding to transport types. METHODS: We obtained prehospital, in-hospital, and trauma registry data from an 8-center cohort of

The effect of prehospital tranexamic acid on outcome in polytrauma patients with associated severe brain injury.

INTRODUCTION: Tranexamic acid (TXA) has shown to be beneficial in selected patients with hemorrhagic shock. Recently, TXA has gained interest in isolated traumatic brain injury (TBI) patients with variable results. There are limited data on TXA in polytrauma with associated TBI. This study investigated the role of TXA in severely injured patients with associated severe TBI. METHODS: A 7.5-year prospective cohort study was performed to investigate the relation between prehospital TXA and mortality in consecutive trauma patients with associated severe TBI (Abbreviated Injury Scale (AIS)head ≥ 3

Management of complex perineal soft-tissue injuries.

Debridement, fecal diversion, and rectal washout have been proposed as the primary therapy for complex perineal lacerations, but, in most series, survivors have a pelvic sepsis rate of 40-80%. In a retrospective study, six of 18 patients sustaining severe perineal lacerations died within the first few hours of injury due to exsanguination from pelvic injuries. The remaining 12 patients underwent sigmoidoscopy, diversion of the fecal stream with irrigation of the distal rectal stump, and radical initial debridement of necrotic soft tissue. Enteral access was obtained in two patients. In the pat

Documentation of Extended Focused Assessment with Sonography in Trauma (eFAST) Is Frequently Incomplete: A Prospective Observational Study.

INTRODUCTION: The extended focused assessment with sonography in trauma (eFAST) is a point-of-care ultrasound protocol that identifies life-threatening thoracoabdominal trauma. Clinical documentation of the eFAST is essential to convey medical decisions, but the extent of documentation in clinical practice is unknown. This study describes the proportion of eFAST exams that are documented in the medical record, the type of documentation (free text vs procedure note), and clinical factors associated with documentation. METHODS: This prospective, single-center study evaluated the documentation o

Extended focused assessment with sonography in trauma injury pattern associations in blunt versus penetrating trauma.

BACKGROUND: Extended focused assessment with ultrasound for trauma (EFAST) is a key component in the assessment of trauma patients; however, there is a paucity of research regarding how injury patterns manifest in EFAST and how associations might differ in blunt versus penetrating trauma. METHODS: A retrospective review was conducted of EFAST studies at a level 1 trauma center, capturing 1356 EFAST exams. Chi-squared analysis was used to evaluate associations within trauma groups and to compare injury prevalence between trauma subgroups. RESULTS: Among all ultrasound exams, 34 (2.5%) had pne

Evaluation of geriatric head and neck trauma cases in a German highest-level trauma centre from 2018 to 2024.

BACKGROUND: Geriatric populations face heightened vulnerability due to aging, socioeconomic challenges, and marginalization. In 2023, 18.1% of Germans over 65 were at risk of poverty, with women and those aged 80 + disproportionately affected. The SARS-CoV-2 pandemic (2020-2022) exacerbated these issues, restricting healthcare access. This study investigates geriatric head and neck trauma trends (2018-2024) across pre-pandemic, pandemic, and post-pandemic periods. METHODS: Conducted at a level 1 trauma centre, data were sourced from the Dortmund Maxillofacial Trauma Registry (14,500 + cases,
