Augmentation enterocystoplasty for the management of voiding dysfunction in spinal cord injury patients.

A total of 12 spinal cord injury adults underwent augmentation enterocystoplasty for treatment of a high pressure neurogenic bladder. These patients suffered from urinary incontinence, recurrent urinary tract infection, upper tract deterioration and severe autonomic dysreflexia. A sigmoid colon segment fashioned into a cup-patch was used in 11 patients and detubularized cecum was used in 1. The artificial urinary sphincter was implanted in 3 patients at augmentation enterocystoplasty and in 1 after enterocystoplasty. After a mean followup of 15 months all patients were continent on clean inter

[Current principles of urological management of patients with injuries of the spine and spinal cord].

Urological management of patients with the neurogenic bladder resulting from the traumatic injury of the spinal cord differs according to the subsequent periods. During the spinal shock it includes: clean intermittent catheterization, prevention of the lithiasis and urinary tract infections with the use of antiseptics and acidifiers patients' acquaintance with their state and of necessity of the intensive co-operation in the process of the rehabilitation. At the transition period, one should attempt and efficient bladder voiding, prevent urinary infections, uro- and nephrolithiasis and establi

Inappropriate use of urinary catheters: a prospective observational study.

BACKGROUND: Despite the well-recognized role of urinary catheters in nosocomial urinary tract infections, data on risk factors associated with inappropriate urinary catheter use are scarce. METHODS: A prospective review of electronic medical records of 436 patients admitted to an adult medical-surgical unit between October and December 2007 was performed to examine the appropriateness of urinary catheter use. RESULTS: The use of 157 urinary catheters in 144 patients was observed. A total of 557 urinary catheter-days were recorded in these patients, of which 175 (31.4%) were found to be inapp

Catheter valves: are they useful in supporting patients in a trial without catheter?

A trial without catheter (TWOC) is a common urological procedure undertaken to remove an indwelling urinary catheter when no longer clinically indicated. An appropriately trained practitioner should undertake a TWOC in a controlled environment to ensure that a further urinary retention does not occur. Indwelling urinary catheters are commonly used with a free drainage system such as a leg bag, which continually empties the bladder. This article examines the potential benefits of using a catheter valve as an alternative to free drainage, prior to undertaking a TWOC, to optimise clinical outcome

Bladder deformities in patients with neurogenic bladder dysfunction.

231 patients with neurogenic bladder dysfunction were evaluated for bladder deformities. Bladder deformity implies bladder trabeculation and deformity of bladder shape such as pine tree shape. Bladder deformity was judged radiographically at maximum cystometric capacity and classified into grade 0 (none), grade I (mild), grade II (moderate) and grade III (severe). Upper urinary tract deterioration (hydronephrosis and/or reflux) was found in 2% of grade 0, 8% of grade I, 52% of grade II and 62% of grade III. I suggested that bladder deformity was one of the risk factors for upper tract deterior

The efficacy of super-lubricous Foley catheters in indwelling catheterization: a randomized controlled trial.

OBJECTIVE: This randomized controlled trial evaluated the clinical efficacy of super-lubricous Foley catheters in reducing urethral complications during indwelling catheterization. Focusing on addressing friction-induced injuries, the study assessed four outcomes: 1) urethral adverse reactions; 2) urethral trauma and inflammatory responses; 3) patient comfort; and 4) post-removal voiding symptoms. METHODS: A total of 114 patients from a tertiary hospital (December 2023-June 2024) were randomly allocated into two arms: 57 individuals in the experimental arm were treated with super-lubricous Fo

Impact of bladder management methods and other factors on sexual activity in women with chronic spinal cord injury/disease.

BACKGROUND: Spinal cord injury/disease (SCI/D) profoundly affects both sexuality and urinary function. Catheterization is often necessary to manage bladder voiding and it can interfere with sexual activity. AIM: We aim to investigate the effect of the bladder evacuation method on sexual activity in women with chronic SCI/D. METHODS: This cross-sectional study at a specialized SCI/D rehabilitation center recruited women ≥18 years with chronic SCI/D using intermittent or suprapubic catheterization. We assessed sexual activity and sexual function with the German version of the Sexual Behaviour

Efficacy and safety of preventing catheter-associated urinary tract infection by inhibiting catheter bacterial biofilm formation: a multicenter randomized controlled trial.

BACKGROUND: Catheter-associated urinary tract infection (CAUTI) remains the most significant challenge among hospital-acquired infections (HAIs), yet still unresolved. The present study aims to evaluate the preventive effectiveness of JUC Spray Dressing (name of U.S. FDA and CE certifications, while the medical device name in China is Long-acting Antimicrobial Material) alone for CAUTI without combining with antibiotics and to evaluate the impact of bacterial biofilm formation on CAUTI results on the inserted catheters of patients. METHODS: In this multicenter, randomized, double-blind study,

[Neurogenic bladder after operation of rectal cancer].

Of the patients who had had a urodynamic examination during the five year period from 1982 to 1987 in our clinic, 48 patients underwent operations for rectal cancer prior to the study. In 35 of them, the operation mode was known. If the pelvic nerve is damaged by operative modes for rectal cancer, urinary disturbances of severe kinds may occur. In spite of such disturbances, 71.4% of those who had had excision of the low anterior part and 51.9% of those with Miles' operation could be weaned from the clean intermittent self-catheterization and take up spontaneous urination. Even in patients who

Longterm indwelling urethral catheterization for neuropathic bladders--an audit.

An audit was performed on 17 patients with neuropathic bladders managed on longterm indwelling urethral catheters with a duration of at least eight years. The accumulated duration of continuous catheterization of this study group was at least 231 man years. The effects on the upper urinary tracts were studied by annual imaging alternating between limited intravenous urograms and ultrasonography, together with serum creatinine estimations. A grading system for upper tract changes was introduced. About a quarter of the patients on indwelling urethral catheter drainage developed early calyceal di
