Paediatric non-infectious granulomatous uveitis: a retrospective cohort study.

INTRODUCTION: Paediatric granulomatous uveitis (PGU) is rare. In addition, lack of awareness often leads to delayed diagnosis and poor visual outcome. Identifying the underlying cause and deciding how best to treat each patient is challenging. OBJECTIVE: The objective of this study was to evaluate the demographics, aetiologies, complications, treatments, and visual prognosis of paediatric non-infectious granulomatous uveitis. METHODS: A retrospective chart review of non-infectious PGU occurring in children before the age of 16 years (recruited from the Paediatric Rheumatology Unit, Bicêtre H…

Inflammatory Choroidal Neovascularization in Pediatric Uveitis: A Case Series.

BACKGROUND: The aim of our study is to describe clinical features, therapy, and outcome of children with inflammatory choroidal neovascularization (CNV). METHODS: Retrospective case series of children seen at the Jules-Gonin Eye Hospital between 2000 and 2024 with macular choroidal neovascularisation secondary to ocular inflammation. RESULTS: Four children (one male, three females) developed a retro-foveolar neovascular membrane after a median time of 2.5 months (range 1 - 16 months) following uveitis onset. The median age at presentation was 12 years (range 5 - 12 years). The CNV presentati…

Isolated Immunoglobulin G4-Related Uveitis Diagnosed by Choroidal Biopsy.

This report highlights a rare case of isolated immunoglobulin G4 (IgG4)-related uveitis in a 24-year-old woman, diagnosed by globe-sparing vitrectomy and choroidal biopsy. She presented with treatment-resistant posterior uveitis characterised by a white elevated subretinal lesion, which progressed to panuveitis and scleritis, causing intractable pain and reducing vision to light perception. After extensive investigations failed to identify a cause, a diagnostic vitrectomy with excisional choroidal biopsy was performed. Histological analysis confirmed IgG4-related disease, despite the absence o…

Vitamin D Supplementation and Remission from Chronic Anterior Uveitis.

PURPOSE: Chronic anterior uveitis (CAU) often requires suppressive therapy, which has potential side effects including cataract, ocular hypertension, and increased risk of infection. No remittive therapy is currently available; however, several studies have demonstrated an association between low 25-hydroxy Vitamin D (25OHD) levels and either uveitis incidence or uveitis disease activity. This study investigates the potential of Vitamin D supplementation as a remittive treatment for CAU. METHODS: We conducted a retrospective analysis using data from the Systemic Immunosuppressive Therapy for …

Drug Retention Time of Immunosuppressive Therapy in Behcet's Uveitis.

PURPOSE: Behcet's Disease is a chronic multisystem vasculitis associated with a blinding uveitis. Few comparative studies exist between conventional disease-modifying antirheumatic drugs (DMARDs) and biologic DMARDs in Behcet's uveitis (BU). We therefore used drug retention time (DRT), an accepted surrogate measure of pharmacological efficacy and tolerability, to compare these treatments in patients with BU. METHODS: Retrospective chart review of patients who met the revised International Criteria for Behcet's Disease (ICBD) treated at the Royal Victorian Eye and Ear Hospital, Australia, betw…

Association between polycystic ovary syndrome and non-infectious uveitis.

Polycystic ovary syndrome (PCOS) is a common endocrine disease in young women. It has been reported that increased proinflammatory cytokines can induce systemic inflammation. However, the association between PCOS and uveitis remains elusive. In this study, we investigate the possible association between PCOS and uveitis using Korean National Health Insurance Service-National Sample Cohort. The incidence of non-infectious uveitis was compared between patients with and without PCOS before and after propensity score matching. Hazard ratios were determined using univariate and multivariate Cox reg…

[Research progress of medication treatment of pediatric noninfectious uveitis].

Pediatric uveitis accounts for 5% to 10% of all uveitis, most of which are noninfectious. Most cases have an insidious onset and are accompanied by many complications, which may lead to poor prognosis and intractable treatment. At present, traditional drugs that are commonly employed to treat pediatric noninfectious uveitis include local and systemic corticosteroids, methotrexate and other immunosuppressants. In recent years, the application of various biological agents has provided new means for the treatment of this kind of disease. This article reviews the progress of medication treatment o…

IgG4-related uveitis. A French cohort and literature review.

INTRODUCTION: IgG4-related disease (IgG4-RD) is a fibro-inflammatory disorder that can affect almost any organ. IgG4-related ophthalmic disease is a protean condition involving the orbit and ocular adnexa. Although a few cases of uveitis have been reported, the exact pattern of IgG4-related intraocular manifestations remains unclear. Here, we report on a nationwide French multicenter cohort of patients with IgG4-RD and uveitis and conducted a literature review. METHODS: Patients with uveitis and a concomitant definite diagnosis of IgG4-RD (Revised Comprehensive Diagnostic criteria, American C…

Endoscopic Visualization for Atypical Uveitis Glaucoma Hyphema Syndrome Management.

We discuss how ophthalmic endoscopy was used in the management of 6 cases with atypical uveitis glaucoma hyphema syndrome. For case 1, the endoscope was used to remove a retained haptic foreign body after an intraocular lens (IOL) exchange with an iris-sutured IOL for a complete capsular bag-IOL complex dislocation. In case 2, the endoscope was key in identifying the presence and location of vascular lesions at the site of previous pars plana sclerotomies. In case 3, the endoscope enabled visualization of a large segmental Soemmering's Ring pushing a 3-piece IOL haptic into the posterior iris.…

Mechanism of Secondary Glaucoma Development in HTLV-1 Uveitis.

Human T-cell lymphotropic virus type 1 (HTLV-1) was the first retrovirus identified as the causative agent of human diseases, such as adult T-cell leukemia, HTLV-1-associated myelopathy, and HTLV-1 uveitis (HU). HU is one of the most frequent ocular inflammatory diseases in endemic areas, which has raised considerable public health concerns. Approximately 30% of HU patients develop secondary glaucoma, which is higher than the general uveitis incidence. We therefore investigated the mechanism underlying the high incidence of glaucoma secondary to HU in vitro. After contact with HTLV-1-producing…

Intraocular foreign body presenting as recalcitrant hypopyon anterior uveitis.

A 38-year-old male with a history of trauma presented to us with pain and decreased vision in the left eye (LE). Previously, the patient had been diagnosed with hypopyon uveitis and was prescribed topical steroids. We performed slit-lamp examination of the LE and found exudates in the anterior chamber (AC) extending from 6 to 9 o'clock, along with circumciliary congestion and pigmented keratic precipitates. We made a diagnosis of infectious anterior hypopyon uveitis; however, all of its causes were ruled out upon the necessary investigations, which returned normal results. The patient was star…
