The clinical usefulness of the vaginal smear.

The chief value of the vaginal smear is as a simple, rapid means of accelerating the diagnosis of cancer in those cases in which the presenting symptoms or lesions are not sufficiently alarming to provoke immediate surgical investigation. Carcinoma of the endocervix, carcinoma in situ, and carcinoma existing together with inflammatory and scarifying lesions are most likely to be discovered by the method. The very early ulcerative malignancy, before the fibrous proliferative reaction produces the classical raised edge and granular base, also has been detected on the smear taken just before the

High-dose-rate vaginal brachytherapy for treatment of recurrent high-grade vaginal intraepithelial neoplasia.

PURPOSE: High-grade vaginal intraepithelial neoplasia (HG VaIN) is a rare preinvasive lesion characterized by a high risk of recurrence. Management strategies encompass both surgical excision, ablative procedures, and topical treatments. Brachytherapy provides a less invasive option, particularly for recurrent HG VaIN cases where other therapies have been unsuccessful or are not feasible. The present study assessed effectiveness and safety of brachytherapy in treating HG VaIN, with a focus on long-term outcomes, including local disease control, progression to invasive carcinoma, and treatment-

Laparoscopic Assisted Neo-Cervico-Vaginal Anastomosis by Rail-Road Procedure In Cervico-Vaginal Atresia.

Cervico-vaginal atresia is a rare entity, difficult to diagnose and treat, with a higher rate of complications. It is classified as type 1b mullerian anomaly per ASRM and subclass C4 per ESHRE classification. The increased rate of re-stenosis and sepsis makes the surgery challenging. Also, the young adolescent age group makes compliance difficult, necessitating an empathetic yet efficient approach. We aim to compare and observe the two repair techniques: laparoscopic v/s open. A stepwise narrative protocol describing this modified technique, which uses a key-hole fundal incision to reach the e

Quiet residents of the vaginal epithelium: immunohistochemical study on the distribution and role of human vaginal Merkel cells.

BACKGROUND: Although Merkel cells (MCs) are well-established mechanoreceptors in human skin, their function within the vaginal epithelium remains undefined. The aim of the present histological study was to investigate whether MCs located in the stratified epithelium of the anterior wall of the human vagina exhibit similar mechanosensory functions to those observed in the skin. METHODS: Immunohistochemical analysis was performed on vaginal wall samples from eight women undergoing transabdominal or laparoscopic surgery. Immunohistochemical markers including cytokeratin 20 (CK20), neuron-specifi

Vaginal natural orifice transluminal endoscopic surgery (vNOTES): a new chapter in vaginal surgical innovation.

Vaginal Natural Orifice Transluminal Endoscopic Surgery (vNOTES) is a novel minimal invasive technique which employs the vaginal orifice for surgical access, after which endoscopic instruments are introduced, thus obviating the need for any external skin incisions. This approach has gained traction as it is associated with reduced morbidity, improved cosmesis, lower pain scores and decreased length of hospital stay, with no increase in adverse events, when compared with conventional laparoscopic surgery. Initially limited to hysterectomy and adnexectomy, its role has now expanded to include in

Use of an Ethinyl Estradiol/Etonogestrel Vaginal Ring Alters Vaginal Microbial Communities in Women With HIV.

BACKGROUND: Human immunodeficiency virus 1 (HIV-1) antiretroviral therapy (ART) alters hormonal contraceptive levels delivered via intravaginal ring (IVR) in a regimen-specific manner. We explored the role of the IVR on vaginal microbial communities, vaginal short chain fatty acids (SCFAs), vaginal HIV shedding, and the effect of vaginal microbes on hormone concentrations in cisgender women with HIV (WWH). METHODS: Vaginal microbes were assessed by 16S RNA sequencing of weekly vaginal swabs, vaginal SCFA by mass spectrometry, HIV-1 shedding by nucleic acid amplification on vaginal aspirates,

Vaginal fungi are associated with treatment-induced shifts in the vaginal microbiota and with a distinct genital immune profile.

Vaginal colonization by fungi may elicit genital inflammation and enhance the risk of adverse reproductive health outcomes, such as HIV acquisition. Cross-sectional studies have linked fungi with an absence of bacterial vaginosis (BV), but it is unclear whether shifts in vaginal bacteria alter the abundance of vaginal fungi. Vaginal swabs collected following topical metronidazole treatment for BV during the phase 2b, placebo-controlled trial of LACTIN-V, a Lactobacillus crispatus-based live biotherapeutic, were assayed with semi-quantitative PCR for the relative quantitation of fungi and key b

The Effect of Citrus Aurantium Vaginal Cream on Vaginal Atrophy in Postmenopausal Women: A Quasi-experimental Study.

BACKGROUND: Vulvovaginal atrophy (VVA) is a common condition and a silent epidemic affecting many postmenopausal women who suffer from it in silence. This study aimed to evaluate the effect of Citrus aurantium vaginal cream on vaginal atrophy in postmenopausal women. METHODS: This single-group pretest-posttest quasi-experimental study was conducted on 30 postmenopausal women who were referred to the Gynecology Clinic of Imam Khomeini Hospital in the city of Noor, Iran, from June to November 2020. Citrus aurantium vaginal cream was administered to women diagnosed with vaginal atrophy (based on

Vaginal Microflora in High Vaginal Swab in Prelabour Rupture of Membrane: A Descriptive Cross-sectional Study.

INTRODUCTION: Premature rupture of membrane (PROM) refers to the disruption of the fetal membrane before the beginning of labor, resulting in spontaneous leakage of amniotic fluid. Cervicovaginal infection is an important risk factor of PROM and can lead to complications to mother and the child. This study aimed to delineate the bacterial patterns found in PROM so that the ideal appropriate responsive antibiotics can be chosen. METHODS: A descriptive cross-sectional was done during the period of 6 months from December of 2023 to May 2023, to characterize the microorganisms in the vaginal flui

The impact of obesity on vaginal hysterectomy and laparoscopically-assisted vaginal hysterectomy outcomes: A randomised control trial.

OBJECTIVES: This prospective randomised control trial aimed to compare outcome measures of vaginal hysterectomy (VH) and laparoscopically-assisted vaginal hysterectomy (LAVH) in obese vs. non-obese women undergoing hysterectomy for benign uterine conditions with a non-prolapsed uterus. The primary objective of the study was to estimate operation time, uterine weight and blood loss amongst obese and non-obese patients undergoing VH and LAVH. The secondary objective was to determine any difference in hospital stay, the need for post-operative analgesia, intra- and immediate post-operative compli

VEGF overexpressed mesoangioblasts enhance urethral and vaginal recovery following simulated vaginal birth in rats.

Vaginal birth causes pelvic floor injury which may lead to urinary incontinence. Cell therapy has been proposed to assist in functional recovery. We aim to assess if intra-arterial injection of rat mesoangioblasts (MABs) and stable Vascular Endothelial Growth Factor (VEGF)-expressing MABs, improve recovery of urethral and vaginal function following simulated vaginal delivery (SVD). Female rats (n = 86) were assigned to either injection of saline (control), allogeneic-MABs (MABsallo), autologous-MABs (MABsauto) or allogeneic-MABs transduced to stably expressed VEGF (MABsallo-VEGF). One hour aft

Next generation sequencing to examine associations between vaginal washing and vaginal microbiota: A cohort study.

BACKGROUND: The association between vaginal washing and HIV risk may be mediated by vaginal washing-associated changes in vaginal microbiota. METHODS: Data from a cohort of HIV-negative US and Kenyan women enrolled in the Preventing Vaginal Infections trial were analyzed. Vaginal fluid samples and vaginal washing data were collected every 2 months for 12 months. Bacterial relative abundances were measured by broad-range 16S rRNA gene polymerase chain reaction with next generation sequencing. Generalized estimating equations were used to evaluate the association between vaginal washing and i)
