Spontaneous otoacoustic emissions (SOAEs) were measured in the ear canal of adult humans prior to, during, and following presentation of tonal and broadband stimuli to the contralateral ear. Tones were presented at a fixed level at ten frequencies relative to the SOAE. Broadband noise was presented at eight levels, from 6 to 76 dB SPL. Shifts in SOAE frequency and amplitude were observed for some subjects, for some tone conditions. Frequency shifts were always positive, whereas amplitude shifts were variable. No apparent pattern of tuning was seen, such that tones with a particular frequency r

The results from 100 patients with 103 acoustic neurinomas who were operated on between February 1979 and April 1992 are presented. All patients were placed in the half-sitting position and operated on by the same surgeon using the suboccipital-trans-meatal approach. Postoperative facial nerve function was good in 56%, moderate in 20% and poor in 24% of our patients. Preservation of useful hearing was achieved in 21%: the smaller the tumor, the better the functional result. However, the early symptoms of acoustic neuromas (tinnitus, dizziness, feeling of unsteadiness) were only slightly improv

Visualization of the posterior fossa during neonatal cranial sonography through the anterior fontanelle can at times be limited. To improve visualization of the posterior fossa, the foramen magnum was used as an acoustic window. Scans through the foramen magnum were obtained in addition to routine scans via the anterior fontanelle in 21 preterm infants. A subjective and retrospective evaluation was performed comparing clarity and definition of posterior fossa structures using the anterior fontanelle and foramen magnum. Images of the fourth ventricle, pons, vermis, cerebellar hemispheres, and c

Intraoperative facial nerve monitoring simultaneously using electromyography and mechanical pressure sensors is being used in retrosigmoid and translabyrinthine approaches for acoustic neuroma resection. Insulated electrified microsurgical instruments and air drills are used to stimulate the facial nerve with a pulsed, constant current through bone and tumor, before the facial nerve is visually encountered. Electrical stimulation is used to help locate the facial nerve, map the course of the facial nerve within tumor, warn the surgeon of unexpected facial nerve locations, and help predict faci

The active vibration of the basilar membrane as well as evoked otoacoustic emissions (EOAE) are probably based on the motile properties of cochlear outer hair cells (OHC). In the present study we examined the effects of contralateral acoustic stimulation on ipsilateral EOAE and thereby the active cochlear micromechanics. Contralateral white noise with intensities below 30 dB HL enhanced the EOAE amplitude, whereas higher sound levels reduced the evoked acoustic emissions. Similar effects of contralateral acoustic stimuli on ipsilateral EOAE were observed in patients with a conductive hearing l

Complete deafness can follow acoustic tumor surgery and results from labyrinthine injury, auditory nerve trauma, and/or vascular compromise. A central auditory prosthesis is one potential rehabilitative strategy in such patients. Anatomical studies suggest that some spiral ganglion cells may survive after vascular occlusion, and we have demonstrated responses to electrical stimulation in patients after labyrinthectomy. It was thus hypothesized that patients deafened after a hearing conservation attempt, but maintaining an intact auditory nerve, could utilize an intracochlear implant. To invest

Hair cells of the avian inner ear have been shown to regenerate following acoustic or ototoxic insult. The consequences of this regeneration on the acoustic nerve have yet to be defined. The purpose of the present study was to use TEM analysis following cochlear damage and hair cell regeneration to describe afferent and efferent neural terminals on hair cells in the newly repopulated sensory epithelium. Following acoustic overstimulation (12 h, 115 dB SPL, 1500 Hz) adult quail were sacrificed immediately (0 day), or at 2, 12, or 24 weeks. Serial thin sections were taken from the embedded papil

The authors present the clinical, radiological, pathological features, and autopsy findings of a patient with malignant triton tumor of the acoustic nerve, which probably arose from a pre-existing acoustic schwannoma. The term "malignant triton tumor" is applied to malignant schwannomas with rhabdomyoblastic differentiation. A cerebellopontine angle tumor with spinal drop metastasis occurred in this patient 10 months after near-total removal of the original tumor.

Acoustic neuromas are benign tumors originating from the Schwann cells of the eighth cranial nerve. Symptoms occur related to compression and stretching of cranial nerves which alters their function. Surgical management is by middle fossa, translabyrinthine, suboccipital or combined approach craniotomy. Prevention and early recognition of postoperative complications are important aspects of nursing care for these clients. A holistic approach to client and family nursing needs is presented. Discharge planning should include assessment of client learning and home care needs. The Acoustic Neuroma

Stapedius reflexes were tested in 21 deaf patients with the Vienna cochlear implant using intra- or extracochlear electrodes. Sinusoidal bursts of 125, 500, 1,000 and 2,000 Hz were used as stimuli presented in a randomized sequence. The contralateral acoustic reflex was monitored by synchronized sampling of the acoustic impedance of the ear. Results obtained were compared to psychoacoustic sensation values. The reflex threshold always was located close to the uncomfortable loudness level within the subjective dynamic range, or at least above the most comfortable loudness level. Practical appli

A simple procedure for measuring changes in the acoustic impedance of the ear is described. The technique has several applications, including simulation using a standard coupler of changes in real ear impedance produced by the acoustic reflex. The technique has also been used for calibrating the response time of an otoadmittance meter used in measuring the temporal characteristic of reflex activity.

The auditory brain-stem responses (ABRs) of 38 patients with acoustic neuroma (AN) were recorded pre-operatively and evaluated with regard to pathological deviations from the normal. Several qualitative and quantitative features of the ABRs were compared statistically with the results obtained from a control group for age and sex, and considering patients with various cochlear pathologies (Meniere's disease, sudden deafness, presbycusis). Retrocochlear disorders were ruled out by computed tomography or magnetic resonance imaging. Statistical analysis yielded the following results. The central

Fetal heart rate response to fetal acoustic stimulation (FAST) during the antepartum period after 25 weeks of gestational age was studied in 157 cases of normal pregnancy, 22 cases of fetal distress, 12 cases of SFD, 10 cases of fetal malformation, 9 cases of placental dysfunction, 4 cases of oligohydramnios, 12 cases of toxemia of pregnancy, and 15 cases of SLE. Five different types of response were observed: Type I through type V. 1. FAST was available to evaluate fetal well-being from 27 weeks of gestational age, although a nonstress test (NST) has been used after 31 weeks of gestational ag

In 1952, Wildervanck described the first case of what he styled the cervico-oculo-acoustic (COA) syndrome. This comprises Klippel Feil's (KF) anomaly (congenitally fused cervical vertebrae), congenital sensorineural deafness and Duane's retraction syndrome (deficient abduction with retraction on adduction). Since that original paper, there have been further reports describing this triad, either completely or incompletely. A further case of this syndrome is reported and the first report of MRI head scan findings in this condition is presented. In addition, the origin of mirror movements observe

The electrical stimulation of the acoustic nerve plays an essential role in the selection of patients for cochlea implantation. The performance of a deaf ear under the conditions of electrical stimulation can be assessed by means of the pre-operative promontory test using a transtympanic needle electrode. Five components of this test were applied to 48 patients: the determination of perception threshold and upper comfortable level for pulse and burst stimulation, the pitch test, the temporal difference test and the gap detection test. The mean values emerging from these examinations may be use

Comparison is drawn between male and female larynges on the basis of overall size, vocal fold membranous length, elastic properties of tissue, and prephonatory glottal shape. Two scale factors are proposed that are useful for explaining differences in fundamental frequency, sound power, mean airflow, and glottal efficiency. Fundamental frequency is scaled primarily according to the membranous length of the vocal folds (scale factor of 1.6), whereas mean airflow, sound power, glottal efficiency, and amplitude of vibration include another scale factor (1.2) that relates to overall larynx size. The diagnosis, size and operation results in 41 acoustic neurinomas (26 females and 15 males) are presented. In one case the tumor was bilateral. Out of the total of 41 tumors, 12.5% were small acoustic neurinomas. As a rule, at diagnosis the tumors were larger in females than in males. Ten of the small and medium size acoustic neurinomas had no brain stem and cerebellar symptoms or signs at all, only the 8th cranial nerve was affected. All large tumors already had brain stem symptoms and one-third of them also had cerebellar symptoms, lesions of further cranial nerves and increasing intracran

Acoustic reflex maximum amplitude measurements elicited both contralaterally and ipsilaterally were obtained from subjects with noise-induced hearing loss and compared with those obtained from normal-hearing subjects. The eliciting signal was a pure tone of 1 kHz presented for 1,000 ms. The groups were matched on age, sex, static immittance and ear canal volume. Acoustic reflex amplitudes were clearly reduced in noise-impaired subjects compared with normal-hearing subjects at a frequency where their hearing thresholds were normal.

In an attempt to overcome the complete auditory deprivation of young patients with bilateral acoustic nerve schwannomas (Morbus v. Recklinghausen) a planned partial resection of the tumour was carried out in 11 hearing ears. The cerebello-pontine angle was exposed by the enlarged middle fossa approach (Wigand et al., HNO 1985). A strip of tumour was left in contiguity to the facial and cochlear nerves. Hearing was initially preserved in all cases. Four of them, however, developed total deafness after 3-39 months. Recurrent tumour growth was checked by CT scans. During the observation time one
