Subjective experiences of catatonia in acute psychiatric inpatients: An exploratory study.

BACKGROUND: The subjective experience of patients during catatonic episodes is a relatively neglected area of research. The aims of this study were to contribute empirical data to the literature by assessing catatonic patients' subjective experiences and to explore how patients' subjective experiences vary according to their underlying medical and psychiatric conditions. METHODS: All 2,281 patients admitted to an acute psychiatric unit in a regional hospital during a 21-month period were screened for catatonia using the 14-item Bush-Francis Screening Instrument. Patients with catatonia were a

Unipolar Mania: Prevalence and Patient Characteristics.

OBJECTIVES: Recurrent [hypo]mania without major depressive episodes ("unipolar mania" [UPM]) is an uncommon form of major affective disorder related to bipolar disorder (BD). We characterized UPM patients and estimated the prevalence of their characteristics based on prolonged times-at-risk. METHODS: Using standard bivariate and multivariate statistics, we compared the characteristics of 63 consecutive UPM patients to 1210 other BD patients over prolonged, close, prospective follow-up at expert mood disorder centers. RESULTS: UPM was uncommon (4.95% of 1273 BD cases during 18.2 years at risk

In-hospital catatonia, delirium, and coma and mortality: Results from the delirium and catatonia prospective cohort investigation.

BACKGROUND: Catatonia, a form of acute brain dysfunction typically linked with severe affective and psychotic disorders, occurs in critical illness with delirium and coma. Delirium and coma are associated with mortality, though catatonia's relationship with mortality is unclear. We aim to describe whether catatonia, delirium, and coma are associated with mortality. METHODS: We enrolled a convenience cohort of critically ill adults (N = 378) at an academic medical center. We assessed catatonia, delirium, and coma using the Bush-Francis Catatonia Rating Scale, the Confusion Assessment Method fo

Diabetes treatment for persons with severe mental illness: A registry-based cohort study to explore medication treatment differences for persons with type 2 diabetes with and without severe mental illness.

It has been argued that persons with severe mental illness (SMI) receive poorer treatment for somatic comorbidities. This study assesses the treatment rates of glucose-lowering and cardiovascular medications among persons with incident type 2 diabetes (T2D) and SMI compared to persons with T2D without SMI. We identified persons ≥30 years old with incident diabetes (HbA1c ≥ 48 mmol/mol and/or glucose ≥ 11.0 mmol/L) from 2001 through 2015 in the Copenhagen Primary Care Laboratory (CopLab) Database. The SMI group included persons with psychotic, affective, or personality disorders within five yea

[Importance of electroconvulsive therapy (ECT) in forensic psychiatry].

BACKGROUND AND OBJECTIVE: Electroconvulsive therapy (ECT) is recommended in guidelines for the treatment of severe affective and psychotic disorders that also can be found in patients in forensic psychiatric commitment; however, there are no systematically collected data concerning the use of ECT in forensic psychiatry. The aim of this study was to investigate the current state and estimated need for ECT in forensic psychiatry in Germany. MATERIAL AND METHODS: A questionnaire was designed consisting of questions about the current practice and estimated need for ECT in this population. This qu

Two Sides of the Same Coin: A Case Report of First-Episode Catatonic Syndrome in a High-Functioning Autism Patient.

Background: Catatonic phenomena such as stupor, mutism, stereotypy, echolalia, echopraxia, affective flattening, psychomotor deficits, and social withdrawal are characteristic symptoms of both schizophrenia and autism spectrum disorders (ASD), suggesting overlapping pathophysiological similarities such as altered glutamatergic and dopaminergic synaptic transmission and common genetic mutations. In daily clinical practice, ASD can be masked by manifest catatonic or psychotic symptoms and represent a diagnostic challenge, especially in patients with unknown or empty medical history. Unclear diag

Concordance of child self-reported psychotic experiences with interview- and observer-based psychotic experiences.

AIM: Valid instruments for the early identification of psychotic experiences (PE) and symptoms in youths are urgently needed for large-scale preventive interventions. A new section of The-Development-and-Well-Being Assessment (DAWBA) measuring child self-reported PE has yet to be validated. The current study aimed to investigate the concurrent validity of DAWBA-based self-reported PE (PE-S) with regard to interview-based measures of PE (PE-I). METHODS: Participants were 1571 (47.8% male) children of age 11 to 12 years from the Copenhagen Child Cohort 2000 (CCC2000) with complete data from bot

Psychiatric disorders in 22q11.2 deletion syndrome are prevalent but undertreated.

BACKGROUND: Chromosome 22q11.2 deletion syndrome (22q11DS) is a common genetic disorder with high rates of psychosis and other psychopathologies, but few studies discuss treatment. Our aim was to characterize the prevalence and treatment of major psychiatric illnesses in a well-characterized sample of individuals with 22q11DS. METHOD: This was a cross-sectional study of 112 individuals aged 8 to 45 years with a confirmed diagnosis of 22q11DS. Each participant was administered a modified Schedule for Affective Disorders and Schizophrenia for School-Age Children (K-SADS) and the Structured Inte

Seasonal effects on human striatal presynaptic dopamine synthesis.

Past studies in rodents have demonstrated circannual variation in central dopaminergic activity as well as a host of compelling interactions between melatonin--a scotoperiod-responsive neurohormone closely tied to seasonal adaptation--and dopamine in the striatum and in midbrain neuronal populations with striatal projections. In humans, seasonal effects have been described for dopaminergic markers in CSF and postmortem brain, and there exists a range of affective, psychotic, and substance abuse disorders that have been associated with both seasonal symptomatic fluctuations and dopamine neurotr

[Receptor mapping in living human beings by means of positron emission tomography].

PET can map neurotransmitter synthesis, storage, release, binding to receptors, and re-uptake in the brain with tracer concentrations in the picomolar or nanomolar range. Tracers are analogues of naturally occurring precursors or ligands, or are drugs, which bind with varying degrees of specificity to receptor subtypes in the brain. Tracers have been synthesised for many transmitter systems, but dopaminergic and serotonergic neurotransmissions are the main foci of current efforts to selectively trace synthesis, storage, re-uptake, or post-synaptic binding of neurotransmitters. Common measures

Use of practitioner-based complementary therapies by persons reporting mental conditions in the United States.

BACKGROUND: To our knowledge, this study provides the first national estimates for use of practitioner-based complementary treatments by US residents reporting mental conditions. METHODS: A total of 16038 respondents to the 1996 Medical Expenditure Panel Survey were asked about visits for 12 complementary medical services (eg, chiropractic services and herbal remedies). Bivariate and multiple regression models examined use of these therapies in individuals reporting a mental condition (n= 1803), fair or poor mental health status (n=992), and 1 of 4 chronic medical conditions (n = 3262) and in

[Course of atypical psychoses].

An attempt was made to classify the atypical psychoses according to cross-sectional psychopathological criteria. We were able to separate two groups from a group of benign schizophrenics. The latter served more as a comparison group and were in every way similar to the nuclear group, except for a better social prognosis. Group C (atypical manic depressive illnesses) which was closer to manic depressive illness than the other two groups, differs from it by thought disorders and delusional experiences. In the further course of the illness, after a few schizophrenic symptoms had appeared once or

Adolescent Idiopathic Scoliosis and Mental Health Disorders: A Narrative Review of the Literature.

UNLABELLED: Adolescent idiopathic scoliosis (AIS) is the most common type of scoliosis. The condition begins in puberty, affects 1-4% of adolescents, and disproportionately affects young women. Our aim was to comprehensively examine the association between AIS and risk for depression, anxiety, eating disorders, psychotic disorders, and personality dysfunctional mechanisms. METHODS: Literature review of related articles published in PubMed, Google Scholar, and Scopus up to 15 July 2021. RESULTS: A total of 30 studies were deemed eligible, examining the effects of AIS upon mental health, and u

Chronic GM2 gangliosidosis masquerading as atypical Friedreich ataxia: clinical, morphologic, and biochemical studies of nine cases.

A progressive spinocerebellar degenerative disorder was characterized in nine patients, aged 11 to 37 years, from four unrelated Ashkenazi Jewish families; affected individuals had markedly deficient beta-hexosaminidase A activity. Symptoms included early onset of cerebellar signs (tremor, incoordination, and dysarthia) and, with maturity, the development of upper and lower motor neuron disorders, marked dysarthia, and ataxia. Three older patients, aged 26, 32, and 37 years, had dementia or recurrent psychotic episodes. Membrane-bound lamellar cytoplasmic inclusions, consistent with lysosomal

Catatonic syndrome during COVID-19 - a case study.

Catatonia is a neuropsychiatric condition involving qualitative psychomotor and volitional disorders. As a nosological unit, it is included in the international classifications of diseases - ICD-10 and DSM-5, but diagnostic criteria vary greatly between these classifications. COVID-19 is an infectious disease that primarily affects the respiratory system, sometimes other organs as well. There are individual reports of COVID-19 coexisting with catatonia in the literature. This case involves a young man who has been hospitalized with symptoms of acute psychotic disorder. During diagnosis, SARS-C

An actigraphy study investigating sleep in bipolar I patients, unaffected siblings and controls.

OBJECTIVES: Disturbances in sleep and waking patterns are highly prevalent during mood episodes in bipolar disorder. The question remains whether these disturbances persist during phases of euthymia and whether they are heritable traits of bipolar disorder. The current study investigates objective sleep measures in a large sample of bipolar I patients, non-affected siblings and controls. METHODS: A total of 107 bipolar disorder I patients, 74 non-affected siblings, and 80 controls were included. Sleep was measured with actigraphy over the course of 14 days. Seven sleep parameters were analyze

Analysis of OPCRIT results indicate the presence of a novel 'social functioning' domain and complex structure of other dimensions in the Wielkopolska (Poland) population.

OBJECTIVE: The main goal of our study was to examine factor structure of schizophrenia and bipolar affective disorder in the Wielkopolska population, using dimension reduction techniques. METHODS: Schizophrenia (n=443) and bipolar affective disorder (n=499) patients were assessed using Operational Criteria Checklist (OPCRIT). Principal component analysis and Maximum Likelihood Factor analysis were carried out to obtain factor structure with significance level for the factor loadings exceeding 0.4. Varimax and promax rotations were used to identify the meaningful factors. RESULTS: Rotated sol

Depressive disorder with psychotic symptoms as psychiatric presentation of sporadic Creutzfeldt-Jakob disease: a case report.

Creutzfeldt-Jakob disease (CJD) is a rare neurodegenerative disease and shows a wide range of early clinical manifestation. We report the case of Ms. A, a 50-year-old woman, who exhibited an atypical psychiatric presentation of CJD at the beginning of the disease, as a depressive disorder with psychotic features. We discuss this case, the clinical attitudes that can lead to misdiagnose such presentation of CJD with other psychiatric diseases, and propose to repeat complementary investigations when the diagnostic is evocated, even if these tests are often negative or nonspecific in the early st

Presentation of the steroid psychoses.

This study suggests that patients receiving daily doses of 40 mg of prednisone or its equivalent, are at greater risk for developing steroid psychosis. Psychotic reactions were twice as likely to occur during the first 5 days of treatment as subsequently. Premorbid personality, history of previous psychiatric disorder, and a history of previous steroid psychosis did not clearly increase the patient's risk of developing psychotic reaction during any given course of therapy. Steroid psychoses present as spectrum psychoses with symptoms ranging from affective through schizophreniform to those of
