As is readily apparent, there is not a consensus regarding the treatment of lateral ligamentous ruptures of the ankle. Certainly, the elimination of functional instability is the goal of treatment. What actually causes this is still open to debate, but from Harrington's work, it appears that degenerative changes can result from mechanical instability. We are all aware that many athletes compete actively with "two-plane" instability of the ankle, but it remains to be seen if they will also suffer the late degenerative changes that Harrington describes. Eighteen athletes with symptoms of recurrent sprain and instability of the ankle during activity were treated by reconstructing the lateral ligament by the Evans technique. Before the operation 55 per cent had given up all athletic activities because of pain, swelling or instability, and 17 per cent had restricted their activities. The average age at operation was 24.6 years and the average follow-up period 3.1 years. Bilateral ankle stress testing was performed on 25 subjects in a device which controlled position of the foot and the amount of force applied during the examination. Both inversion testing in the anteroposterior plane and anterior drawer testing in the lateral plane were performed in the same group of symptom-free patients. A retrospective study of 26 cases of supramalleolar fracture is presented. Types II and III were most common. All but the one had accompanying fibular fracture. The incidence of open fracture was 41.7% (10/24). The most common mechanism of injury was a high energy impacting force in the direction of axial compression. The supramalleolar fracture was subgrouped into four types. Cooling treatment with 'Cryogel' is compared with the use of crepe bandaging in patients with recent ankle sprains. All patients were followed up regularly and the results appears to favour cooling when it is used in the early period of treatment. Cooling reduced oedema, pain and disability, and shortened the recovery period.

A prothesis has been designed to replace the articulating surfaces of the human ankle joint. The prothesis is in two parts, each forming a segment of a right circular cylinder with a single axis of rotation. A syndrome of ankle pain on weight-bearing while running due to post-traumatic ossification of the tibiofibular ligament is described. Pain is caused by failure of normal downward and lateral motion of the fibula. Treatment consists of complete excision of the synostosis, followed by cast immobilization for 3 weeks.
