Familial 'sleep apnea plus' syndrome: report of a family.

We describe a familial disorder consisting of sleep apnea, anosmia, colorblindness, partial complex seizures, and cognitive dysfunction. The phenotypic expression of the syndrome suggests an autosomal dominant inheritance with incomplete penetrance.

Central sleep apnea reversed by continuous positive airway pressure.

Continuous positive airway pressure (CPAP) is an accepted and highly effective method used in the treatment of obstructive sleep apnea (OSA). We have explored the potential utility of CPAP in a patient with sleep apnea without a major contribution of airway obstruction as measured by the usual criteria. The patient showed dramatic clinical and laboratory improvement in central apnea when treated with CPAP. A low level of CPAP (7.5 cm H2O) resulted in the reduction of central apneas from 281 to 32 per night with no significant change in obstructive apneas, which increased from 43 to 59 per nigh…

Bimodality electrophysiologic evaluation of brainstem in sleep apnea syndrome.

Previous electrophysiologic studies in the sleep apnea syndrome (SAS) have used only a single modality (brainstem auditory evoked responses [BAERs]) and yielded conflicting, inconsistent, and inconclusive results. We utilized both BAERs and somatosensory evoked responses in 12 patients with SAS and found normal central conduction times in all patients. These data argue against a significant structural alteration in both rostral and caudal brainstem, insofar as the auditory and somatosensory pathways are concerned, in patients with SAS.

Prolonged expiratory phase in sleep apnea. A unifying hypothesis.

The mechanisms responsible for the various sleep apnea patterns have not been elucidated. In 5 hypersomnolent patients, we analyzed 613 events that would have been labeled mixed and central apneas had usual recording sensitivity been employed. Recording air flow at high sensitivity revealed that in most of these events expiratory flow was present throughout most of the prolonged period during which there was no inspiratory effort. In 378 events, resumption of inspiratory effort was unaccompanied by air flow (occluded events), whereas in 144 events resumption of inspiratory effort was accompani…

Abnormal cerebrovascular responses to CO2 in sleep apnea patients.

The responses of common carotid blood flow (CCF), pressure (BP), and resistance (R) to variations in respiratory gases were compared during waking periods in 10 sleep apnea patients (SA) and 10 healthy controls (N) of similar age. Respiratory gases were altered by 3-min CO2 rebreathing (RB), 3-min hyperventilation (HV), and 4-min hypoxia (HYP) procedures. CCF was measured continuously by a 5-MHz pulsed Doppler duplex scanner and R was calculated using brachial BP. During RB, which increased end-tidal PCO2 (PACO2) by 15 mm Hg, SA had a lower CCF and greater BP response and therefore a significa…

Sleep apnea and nocturnal hypoventilation after western equine encephalitis.

That viral encephalitis can produce daytime hypoventilation is well appreciated, but an association of this infection with the development of a prolonged disturbance of breathing during sleep once the ventilatory disorder during wakefulness has resolved, has not been reported. We describe a young male who required assisted mechanical ventilation in association with western equine encephalitis. Normal waking ventilation returned over 3 to 4 wk, but frequent apneas and severe hypoventilation persisted during sleep with a near absence of waking ventilatory response to chemical stimuli. Over 3 mon…

Pathophysiologic mechanisms causing sleep apnea and hypoventilation in infants.

Complete absence of chemoreception in the medulla is characterized by sustained hypoventilation. Except for the first months of life, this deficit does not, of itself, cause apnea. Both in infants who lack central chemical drive, as well as those who are only partly deficient, it is necessary to invoke at least one further pathophysiologic event. Various alterations in gas exchange, especially during sleep, which promote hypoxemia can contribute to a positive feedback effect on ventilation. If gas exchange is further impeded by upper-airway obstruction, only a massive arousal response separate…

Ventilatory studies in two older infants with prolonged apnea.

Two infants, both born before term, were found apneic, cyanotic and limp at home when they were 9 weeks and 2 weeks old, respectively. Their respiration was monitored in the hospital and found to be remarkably periodic during sleep, and was in one case accompanied by pronounced bradycardia. The periodic breathing and apnea seemed to be caused by a decreased oxygen tension which can induce an instability of the central respiratory control mechanisms.

Low-dose theophylline therapy in idiopathic apnea of prematurity.

The effects of theophylline at a dose of 2 mg/kg/day (with resultant serum concentrations of 2.8 to 3.9 micrograms/ml) were observed in seven premature infants with severe idiopathic apnea. The frequency of apneic episodes decreased 62 +/- 9% (SEM) and the frequency of episodes of bradycardia decreased 73 +/- 9% following 24-48 hours of this therapy. No changes in ventilatory response to CO2 resting respiratory minute volume, arterial blood gas values, resting heart rate, or serum electrolyte concentrations were observed. Theophylline at this dose appears to be a safe and effective treatment f…

The No-Apnea score for early obstructive sleep apnea detection in a sleep clinic: a study of diagnostic accuracy and comparative performance of three screening instruments.

This cross-sectional study investigated the No-Apnea tool's diagnostic ability to detect obstructive sleep apnea (OSA), across different severities (respiratory disturbance index [RDI] ≥ 5, ≥ 15, ≥ 30) compared to polysomnography (gold standard) in 2,087 consecutive patients attending a sleep clinic (age: 45.92 ± 13.46, male: 70.8%). We further compared the diagnostic performance of No-Apnea, NoSAS, and STOP-Bang tools. Across all RDI levels, No-Apnea showed superior sensitivity (96.9-99.0%, based on RDI thresholds), negative predictive value (NPV) (47.8-91.2%), negative likelihood ratio (0.10…

[The influence of obstructive sleep apnea syndrome on the orthodontic treatment decision in children and adolescents. Part 2 : Which orthodontic treatments for children with apnea?].

INTRODUCTION: Sleep-disordered breathing could affect 10% of an orthodontic population. The integration of obstructive sleep apnea syndrome (OSAS) diagnosis could influence the choice of orthodontic techniques or their implementation, with the aim of improving ventilatory function. MATERIAL AND METHOD: The author summarizes the clinical studies using dentofacial orthopedics, alone or in combination with other interventions, in pediatric OSAS or the repercussions of orthodontic interventions on upper airways. RESULTS: For the same orthodontic anomaly, in particular, transverse maxillary defic…

Effect of adenotonsillectomy in children with obstructive sleep apnea and major psychiatric disorders on obstructive apnea-hypopnea index and Epworth Sleepiness Scale scores.

STUDY OBJECTIVES: This study evaluates the effectiveness of adenotonsillectomy in the treatment of obstructive sleep apnea in children with major psychiatric disorders as measured by polysomnography and the Epworth Sleepiness Scale (ESS) at a tertiary children's hospital. Adults with major psychiatric disorders often have higher rates of obstructive sleep apnea and decreased response to treatment. The goal was to determine if children with serious mental illness had outcomes similar to their adult counterparts. METHODS: A retrospective chart review was undertaken to identify children with obs…

Medium Increased Risk for Central Sleep Apnea but Not Obstructive Sleep Apnea in Long-Term Opioid Users: A Systematic Review and Meta-Analysis.

STUDY OBJECTIVE: Opioids are associated with higher risk for ataxic breathing and sleep apnea. We conducted a systematic literature review and meta-analysis to assess the influence of long-term opioid use on the apnea-hypopnea and central apnea indices (AHI and CAI, respectively). METHODS: A systematic review protocol (Cochrane Handbook guidelines) was developed for the search and analysis. We searched Embase, Medline, ACP Journal Club, and Cochrane Database up to November 2014 for three topics: (1) narcotics, (2) sleep apnea, and (3) apnea-hypopnea index. The outcome of interest was the vari…

Retatrutide Once Weekly for Treatment of Obstructive Sleep Apnea.

BACKGROUND: Retatrutide, an investigational triple agonist of glucose-dependent insulinotropic polypeptide, glucagon-like peptide 1, and glucagon receptors, may be useful for treating obstructive sleep apnea (OSA). METHODS: In two phase 3, double-blind, randomized, placebo-controlled trials nested within larger basket trials of once-weekly subcutaneous retatrutide, we assigned adults with moderate-to-severe OSA and a body-mass index (the weight in kilograms divided by the square of the height in meters) of at least 27, without (TRIUMPH-1-OSA) or with (TRIUMPH-2-OSA) type 2 diabetes, to receiv…

"Phenotyping obstructive sleep apnea: a pathway to precision medicine".

PURPOSE: The apnea-hypopnea index (AHI) has long dominated the diagnosis of obstructive sleep apnea (OSA); however, mounting evidence indicates that AHI alone cannot capture the disorder's heterogeneity. This review synthesizes current evidence on polysomnography-based phenotyping and endotyping into a practical framework for precision OSA management. METHODS: PubMed/MEDLINE and Scopus were searched from inception through December 2025 using terms related to OSA, phenotyping, endotyping, and polysomnographic parameters. Randomized trials, cohort studies, cluster analyses, and machine learning …
