The true prognosis of JRA is unknown. The best interpretation of reports to this date may be that at any given time of examination between 5 and 15 years after onset, 30-50% of children will have grossly active disease and that 70-90% of patients will be in class I-II functional status. Published studies, however, are not comparable because of differing criteria and selection of support data to be reported. Corticosteroids, although potent suppressors of symptoms and signs of disease, are not effective in curing rheumatoid arthritis in adults or children. Prolonged use of corticosteroids is attended with multiple serious side effects. In JRA, the outlook for most children is good; potentially dangerous drugs should be avoided if at all possible. During an exacerbation of rheumatoid arthritis in a 54 year old woman, an acute lifethreatening complication developed with pleuro-mediastino-pericarditis presenting as a superior vena cava obstruction-like syndrome. This was later associated with thrombosis of the left external jugular vein and a peripheral Horner's syndrome. She did also develop arterial hypertension with retinopathy and severe cardiac failure. The prevalence of cervical rheumatoid arthritis and its relationship to rheumatoid serum factors and erosive arthritis in peripheral joints has been studied in radiographs of the cervical spine and of the hands and feet drawn from 12 population samples. In a study of 112 patients (49 men, 63 women) with psoriatic arthritis, ocular inflammation was noted in 35 (31.2%). Conjunctivitis was the most common lesion, being found in 19.6% (10 males, 12 females). 227 patients with psoriasis and various forms of arthritis have been kept under review. Psoriasis and inflammatory arthropathy was present in 168 patients, of whom 94 have been followed up for more than 10 years. An arthritis indistinguishable from rheumatoid disease was present in 78%, distal joint arthritis in 16-6%, and deforming arthritis in 4-8%. Pulmonary ventilation was studied on 100 rheumatoid arthritics from a medical outpatient clinic, using wholebody plethysmography, spirography, and pneumometry. Findings were normal in 2/3 of the patients; reduced lung capacity was found in only 3-6%. Thus it is doubtful that significant restriction of ventilation is present in chronic polyarthritis. An experimental chronic mono-arthritis was induced in rabbits using adjuvants and bovine serum albumin. The results suggest that unidentified host factors (possibly genetic) influence the induction of delayed hypersensitivity to the antigen. The data further show a clear correlation between systemic cell mediated immunity and chronic synovitis. When this condition is absent, there is a poor correlation between the humoral response and the arthritis.
