We present the case of a healthy 18-year-old female who presented with history of an acute onset, painful, swollen right ankle joint. Microbiologic samples from the ankle aspirate grew Neisseria meningitidis. Septic arthritis associated with extra-articular abscess is rare. We report on 2 non-rheumatic patients with septic arthritis of the knee associated with calf abscess. Magnetic resonance imaging showed a distinct leakage pathway from the knee joint in each patient. One was a ruptured popliteal cyst (posteromedial). We report here the case of a sacro-iliitis due to Kingella kingae in a 35 year-old patient. This case report points out the pathogenic potency in adults of this organism which is known as a cause of invasive infections in young children. Inoculation of blood culture vials with clinical specimens and the use of nucleic acid amplification have recently improved the sensitivity to identify Kingella kingae and to diagnose infections which need an efficient antimicrobial therapy.

Musculoskeletal symptoms in Lyme disease are very common at all stages of the disease. Lyme arthritis, whether intermittent or chronic, is a hallmark of late Lyme disease. This may cause severe joint pain and swelling especially confined to one or a few joints, most notably the knee. Antibiotic therapy is very effective in treating Lyme arthritis in the majority of cases. However, a small proportion of individuals will develop persistent chronic arthritis which is likely mediated through immunologic mechanisms. In these patients treatment strategies should include anti-inflammatory medications

Brucellar septic arthritis has become extremely rare in Western developed countries since bovine brucellosis has been successfully eradicated. A 44-year-old diabetic man with isolated septic arthritis of the left acromioclavicular joint (A-C) caused by Staphylococcus aureus is described. He was admitted to the Department of Rheumatology with clinical symptoms of left shoulder arthritis and fever. Physicians can expect to see more mycobacterial bone and joint disease in North America as a result of increased travel, immigration, and use of immunosuppressive medications. The first step in treating infections caused by these organisms is to consider the diagnosis early in the course of illness. Long-standing untreated mycobacterial infections typically cause significant bone destruction and loss of function. The treatment of mycobacterial bone and joint infection requires prolonged antibiotic therapy, often in conjunction with surgical intervention, particularly for spinal tuberculosis.

Cryptococcus neoformans is an opportunistic pathogen that occasionally causes indolent disease in the immunocompetent. Cryptococcal arthritis is especially rare, and diagnosis is typically delayed. Fungal arthritis should be considered in cases of chronic, indolent joint inflammation.
