Tract-based spatial statistics of magnetic resonance images to assess disease and treatment effects in perinatal asphyxial encephalopathy.

Biomarkers are required for efficient trials of neuroprotective interventions after perinatal asphyxia. This study aimed to determine whether diffusion tensor imaging (DTI) analyzed by tract-based spatial statistics (TBSS) may be a suitable biomarker of disease and treatment effects after perinatal asphyxia in small groups of patients. We performed TBSS from DTI obtained at 3 T from eight healthy control infants, 10 untreated and 10 hypothermia-treated infants with neonatal encephalopathy. Median (range) postnatal age at scan was 1 d (1-21) in the healthy infants, 6 d (4-20) in the cooled, and

Does perinatal asphyxia contribute to neurological dysfunction in preterm infants?

BACKGROUND: Children born preterm are known to be at risk for neurodevelopmental disorders. The role of perinatal asphyxia in this increased risk is still a matter of debate. AIM: To analyze the contribution of perinatal asphyxia in a population of preterm infants admitted to a secondary paediatric setting to neurological dysfunction in the first months after birth and to the development of cerebral palsy. METHODS: 17 preterm infants with perinatal asphyxia born before 35 weeks postmenstrual age (PMA) and 34 carefully matched preterm controls without asphyxia were studied. Neuromotor outcome

Head covering - a major modifiable risk factor for sudden infant death syndrome: a systematic review.

BACKGROUND: Some victims of sudden infant death syndrome (SIDS) are found with their heads covered with bedclothes, but the significance of this is uncertain. The aim of this review is to describe the prevalence of head covering, the magnitude of the risk and how far the suggested causal mechanisms agree with current epidemiological evidence. METHODS: Systematic review of population-based age-matched controlled studies. RESULTS: Controlled observations of head covering for the final sleep were found in 10 studies. The pooled prevalence in SIDS victims was 24.6% (95% CI 22.3% to 27.1%) compar

[Clinical study on improving the diagnostic criteria for neonatal asphyxia].

OBJECTIVE: Diagnosing neonatal asphyxia solely according to Apgar score may lead to misdiagnosis. The aim of this study was to explore new and more accurate diagnostic criteria for neonatal asphyxia. METHODS: Totally 10 376 live born neonates in our hospital were consecutively enrolled into the study. The following five items related to birth asphyxia, i.e., antepartum high-risk factors, Apgar scores, umbilical artery blood pH, organ injury, differential diagnosis on the causes of low Apgar score cases were examined and registered. The relationship among the first 4 items were analyzed. By di

Heart rate changes are insensitive for detecting postasphyxial seizures in neonates.

We studied heart rate (HR) changes during 169 seizures (mean 12 per patient, range 8 to 18) in 14 neonates with severe birth asphyxia. HR changes were found in 21 seizures (12.4%) in eight patients (HR increases in four, decreases in one, and both patterns in three patients), suggesting the existence of neonatal cerebral hemispheric connections with brainstem autonomic regulatory centers. HR monitoring appears to be insensitive for detecting postasphyxial neonatal seizures.

Severe calcinosis cutis in an infant.

We report on an infant with severe asphyxia and persistent pulmonary hypertension as a newborn. The baby received prolonged intravenous calcium gluconate therapy for hypocalcemia. At 5 weeks of age, multiple firm, indurated areas (armor-like lesions) were palpable in the subcutaneous tissues of the trunk, arms, legs, and face, particularly in skin folds. Roentgenographic study showed generalized soft-tissue calcifications throughout the body, extremities, and face. Calcinosis cutis occurs through a variety of pathogenetic mechanisms. Case reports on calcinosis cutis in infants are uncommon, an

Cerebral glucose metabolism and early EEG/aEEG in term newborn infants with hypoxic-ischemic encephalopathy.

The objective was to investigate how early electrocortical background pattern, as recorded with amplitude integrated EEG (aEEG), correlates with global and regional cerebral glucose metabolism (CMRgl) measured by positron emission tomography during the subacute phase after birth asphyxia. Nineteen term infants with hypoxic-ischemic encephalopathy were investigated. The aEEG background was evaluated at 0-6, 6-12, 12-24, 24-48, and 48-72 h postnatal age, and classified into four categories according to increasing degree of abnormality. The aEEG were also evaluated for sleep-wake cycling and epil

Prediction of adverse outcome with cerebral lactate level and apparent diffusion coefficient in infants with perinatal asphyxia.

PURPOSE: To compare the predictive value for adverse outcome of quantitative cerebral lactate level and of apparent diffusion coefficient (ADC) in infants with perinatal asphyxia in the early postnatal period. MATERIALS AND METHODS: Lactate-choline ratios determined with proton magnetic resonance (MR) spectroscopy and ADC determined with diffusion MR imaging in basal ganglia and thalami in 26 full-term neonates (age range, 1-10 days) were compared with severity of acute hypoxic-ischemic encephalopathy and long-term clinical outcome. Differences in metabolites between outcome groups were evalu

Serial protein S-100 serum levels in preterm babies with perinatal asphyxia and periventricular white matter lesions.

In the last years new diagnostic technologies were developed to assess brain development and to identify early brain injury. Some of them are very attractive methods but invasive, expensive, and time-consuming. The availability of clinically useful serum markers of risk for perinatal brain damage will easily permit the development of rational strategies for prevention of cerebral insults in neonates and more accurate prognostic counseling. In this study, protein S-100 (PS-100), a cytosolic constituent of neuroglial cells, was measured serially, during the neonatal period, in a group of preterm

Cardiac abnormalities in birth asphyxia.

Cardiac abnormalities in birth asphyxia were first recognised in the 1970s. These include (i) transient tricuspid regurgitation which is the commonest cause of a systolic murmur in a newborn and tends to disappear without any treatment unless it is associated with transient myocardial ischemia or primary pulmonary hypertension of the newborn (ii) transient mitral regurgitation which is much less common and is often a part of transient myocardial ischemia, at times with reduced left ventricular function and, therefore, requires treatment in the form of inotropic and ventilatory support (iii) tr

MR imaging of perinatal brain damage: comparison of clinical outcome with initial and follow-up MR findings.

BACKGROUND AND PURPOSE: The purpose of our study was to determine whether MR studies in the neonatal period are predictive of the neuroradiologic sequelae and clinical outcome in premature and term infants with perinatal brain injury. METHODS: Thirty subjects (15 premature and 15 term infants) with abnormalities revealed by initial MR studies were reexamined approximately 1 year after birth with both MR imaging and a neurologic assessment. All initial MR studies were performed between 35 and 45 weeks corrected age in premature infants and within 28 days of life in term infants. The initial MR

Predictive value of early continuous amplitude integrated EEG recordings on outcome after severe birth asphyxia in full term infants.

The background pattern in single channel amplitude integrated EEG recordings (aEEG) was recorded in 47 infants within the first six hours after birth to see if this could predict outcome after birth asphyxia. The aEEG background pattern during the first six hours of life was continuous and of normal voltage in 26 infants. All these infants survived; 25 were healthy, one had delayed psychomotor development. A continuous but extremely low voltage pattern was present in two infants, both of whom survived with severe handicap. Five infants had flat (mainly isoelectric) tracings during the first si

Resuscitating fresh stillbirths.

Guidelines for duration of resuscitation of freshly-stillborn term infants and their long term outcome are unclear. The predictive factors of both cerebral palsy and early neonatal demise include Apgar scores of 3 or less at 10 minutes, perinatal acidaemia and neonatal seizures. We describe the case of severe perinatal asphyxia (umbilical artery pH at birth of 6.75, Apgar scores of 0.1 and 5 0.1 5 at 1.5 and 10 minutes and neonatal hypertonia) in a term pregnancy, where the infant made an uneventful recovery and was discharged home well. Guidelines for neonatal resuscitation are discussed.
