[Anatomic changes in the biliary tract and pancreas in patients with diabetes mellitus diagnosed by endoscopic retrograde cholangiopancreatography].

Structural changes in both biliary tract and pancreas have been assessed with endoscopic retrograde cholangiopancreatography in 100 diabetic patients divided into subgroups depending on the type of diabetes mellitus, i.e. type I, type II and III-pancreatic. Control group included 100 randomly selected patients without diabetes mellitus in whom endoscopic retrograde cholangiopancreatography has been performed for various indications. Structural changes in the biliary tract and pancreas have been more frequent in diabetic patients than in the control group (47 and 75% vs 32 and 30%, respectively…

[Clinical and experimental studies on stone detectability of cholangiography and choledochoscopy. Prevention of retained biliary-tract stones].

UNLABELLED: Preoperative diagnostic accuracy was significantly improved following a recent development of diagnostic imaging techniques. However, the incidence of retained biliary tract stones is far from satisfactory low. Retrospective studies on operative and postoperative cholangiography, and choledochoscopy were carried out to determine the limitation of each modality in preventing the retained biliary tract stones. The effectiveness of postoperative stone extraction technique for retained biliary tract stones was reevaluated by a follow up study of 213 cases with retained biliary tract st…

Contrast enhancement of liver parenchyma and biliary tract related to liver function at delayed computed tomography.

Computed tomography (CT) of the liver was performed in 53 patients before, during and 4 to 6 hours after intravenous administration of approximately 60 g of iodine in metrizoate. At delayed CT, contrast medium was observed in the extrabiliary ducts in 20 patients (38%) and in the gallbladder in 28 (68%) of the 41 patients with no previous cholecystectomy. The increase in attenuation of liver parenchyma at delayed CT compared with the non-enhanced baseline CT ranged between 2 and 26 HU (mean 13.9). The material was divided into three groups with regard to liver function: normal, intermediate an

Abdominal contrast tomography in the evaluation of the gall-bladder and biliary tract (with emphasis upon jaundiced patients).

Tomography of the right upper quadrant after the rapid intravenous infusion of a high dose of contrast medium is a valuable method of investigation of hepato-biliary disease. Gall-bladder wall opacification may be normal, but if the wall is thicker than 2 mm and if irregular, it indicates inflammatory or calculous disease. Gall-bladder area greater than 35 cm2 indicates bile duct obstruction. This also causes the ducts to appear as lucent bands within the opacified liver parenchyma. Secondary deposits in the liver may also become visible. Various patterns of these findings are described which

Excessive Dissection of Nonmetastatic Tumor-Draining Lymph Nodes Impairs Immunotherapy Efficacy in Recurrent Biliary Tract Cancer.

PURPOSE: Nonmetastatic tumor-draining lymph nodes (TDLNs-) are central to initiating and sustaining antitumor immunity. The impact of their surgical removal on immunotherapy efficacy in recurrent biliary tract cancer remains unclear. We investigated the effect of TDLNs- dissection extent on treatment outcomes in this population. EXPERIMENTAL DESIGN: This real-world study retrospectively analyzed clinical and survival data from 101 patients with recurrent biliary tract cancer who received immunotherapy across five Chinese hospitals (2018-2023). Patients were stratified by extent of TDLNs- diss…

Adjuvant Chemotherapy Regimens in Resected Biliary Tract Cancers: National, Comparative, Observational Study (TOG/GI-SAFRADJU-2501).

The recommended adjuvant chemotherapy (adj-ChT) regimen for resected biliary tract cancers (BTC) is capecitabine (Cape); yet, the recommendation is based on limited evidence. Although varied adj-ChTs have been employed in practice, robust real-world data is scarce. We conducted a national, multicenter, hospital-based registry study to evaluate adj-ChTs in resected BTCs. Patients who received adj-ChT (± radiotherapy) between 2010 and 2024 were included. Recurrence-free (RFS) and overall survival (OS) were analyzed by adjusted Cox-regression and propensity score-based inverse-probability-of-trea…

Overall and Line-Specific Impact of Targeted Therapies in Advanced Biliary Tract Cancer After Chemoimmunotherapy Failure.

BACKGROUND & AIMS: The effectiveness and optimal timing of matched targeted therapy after first-line chemoimmunotherapy in biliary tract cancer (BTC) remain uncertain. We evaluated access to matched agents and line-specific outcomes. METHODS: This retrospective study included 1,358 patients with advanced BTC treated with first-line cisplatin, gemcitabine, and durvalumab at 55 centers in 12 countries. Actionable alterations were defined as ESCAT tier I. The principal post-progression analysis excluded patients receiving best supportive care only and compared matched targeted therapy with activ…

Survival outcomes with matched targeted therapy in advanced biliary tract cancer: a large global cohort analysis.

The prognosis of advanced biliary tract cancer remains poor despite up to 40% of tumors harboring actionable molecular alterations. This international collaborative study from Mayo Clinic, Duke Cancer Institute, National Cancer Center East (Japan), and the SCRUM-Japan GOZILA/MONSTAR-SCREEN projects evaluated clinical outcomes in 1049 patients with advanced BTC based on tumor molecular profiling and matched treatment selection. Patients were categorized into non-actionable, actionable with matched targeted therapy (matched), and actionable without matched therapy (unmatched) groups. Among 358 p…

Ex Vivo Tumor-Derived Organoid Pharmacotyping Identifies Personalized Therapeutic Options for Patients with Biliary Tract Cancer.

UNLABELLED: Biliary tract cancers (BTC) pose clinical challenges due to poor chemotherapy response and aggressive disease course. We evaluated patient-derived tumor organoid-based drug sensitivity testing as a tool to guide therapy. In this multicenter study, 26 tumor organoids were successfully derived from 43 patients with BTC and tested with an average of 50 cancer-directed therapies using the Clinical Laboratory Improvement Amendments-certified PARIS assay. Despite most organoids being from late-stage disease, 24/26 (92.3%) exhibited strong sensitivity to one or more targeted agents. Activ…

Molecular Tumor Board-Guided Targeted Treatments for Biliary Tract Cancers in a Publicly Funded Healthcare System.

Comprehensive molecular profiling can identify alterations in biliary tract cancer (BTC) potentially treatable with targeted therapies. However, the impact of whole-genome and transcriptome sequencing (WGTS) on therapeutic decision-making in a public healthcare system is unknown. Here, BTC patients prospectively received WGTS to inform clinical care at a large Canadian academic cancer center. We characterized the proportion of targetable alterations, the treatment recommendations generated by a molecular tumor board, targeted therapies received, patient outcomes, and the financing of these tre…

Attempts to Establish a Helicobacter bilis Biliary Tract Infection Model via Oral Administration of Helicobacter bilis.

Background Although several studies have reported an association between Helicobacter bilis (H. bilis) and human biliary tract cancer, in vivo studies are necessary to further assess this phenomenon. In this study, we developed an animal model of H. bilis infection, determined whether oral exposure to H. bilis induces biliary carcinogenesis, and evaluated colonization and pathological changes. Methods Seven-week-old female Syrian golden hamsters were administered H. bilis via the intragastric route using a sonde and euthanized after two, four, 12, 24, and 48 weeks. We assessed the rate of bact…

Efficacy of S-1 Adjuvant Chemotherapy for Resected Biliary Tract Cancer: A Retrospective Propensity-Matched Analysis.

INTRODUCTION: Surgical resection is considered an effective cure for biliary tract cancer (BTC); however, the prognosis is unsatisfactory despite improved surgical techniques and perioperative management. The recurrence rate remains high even after curative resection. The efficacy of adjuvant chemotherapy in pancreatic and gastric cancers has been previously reported, and the feasibility of adjuvant therapy with S-1 has recently been reported in patients with resected BTC. We aimed to retrospectively investigate the effects of adjuvant chemotherapy with S-1 on resected advanced BTC. METHODS: …

Clinicopathologic and genomic characteristics of biliary tract carcinomas with TERT promoter mutations among East Asian population.

Telomerase reverse transcriptase gene promoter (TERT) mutations are biomarkers that predict survival and responses to immune checkpoint inhibitors in various malignancies. However, their prevalence and clinicopathologic characteristics in biliary tract carcinomas are largely unknown. We performed a comprehensive genomic profiling of formalin-fixed paraffin-embedded tumor tissue from 485 carcinomas, including intrahepatic (n = 220), perihilar (n = 54), distal biliary tract (n = 110), and gallbladder (n = 101) cancers, using next-generation sequencing. TERT mutations were observed in 50 out of 4…
