Conditions associated with congenital annular constricting bands include autoamputation, acrosyndactyly of the hands and toes, clubfeet, lymphedema, pseud-arthrosis, and nail deformities. The association of congenital annular constricting bands and angular deformities of long bones is exceedingly rare. Reported is one such case treated with fibrous release and crescentic osteotomy.

A description is given of a prosthesis for replacement of tumour-destroyed diaphyseal bone in the lower extremity. It can be used in a salvage procedure and as an alternative to intramedullary nailing or plating procedures. The method is not complicated provided that mechanically sound devices are used and technical errors are avoided. The operation time is short and the patients can be mobilized early.

A syndrome of brachydactyly (absence of some middle or distal phalanges), aplastic or hypoplastic nails, symphalangism (ankylois of proximal interphalangeal joints), synostosis of some carpal and tarsal bones, craniosynostosis, and dysplastic hip joints is reported in five members of an Italian family. It may represent a previously undescribed autosomal dominant trait.

BACKGROUND: Open tibial fractures are mostly the result of high-energy traumas and often involve severe injuries with extensive bone and soft tissue loss, damage of muscles and neurovascular structures. Over recent- years, - the growth of Ortho-Plastic teams, as a well-coordinated bone, joint and soft tissue treatment, contributed to change the approach to these fractures and to achieve higher successful results in lower limb salvage. Unfortunately, many hospitals cannot benefit of a combined team in emergency, and the orthopedic surgeon is forced to manage personally these kinds of traumas.

Psoriasis is a complex disease involving the skin, nails and musculoskeletal structures. Psoriatic arthritis (PsA) affects peripheral joints, entheses, the synovial sheaths of tendons and the axial skeleton. PsA is now recognised as a potentially debilitating disease, with bone erosions and deformities affecting about half of patients and arising early after disease onset. Because PsA usually coincides with or follows the development of psoriasis, dermatologists are in a strategic position to diagnose and manage early PsA. In general, patients with severe psoriasis appear to have a higher risk

Background: Massive post-traumatic segmental bone defects exceeding 5 cm-particularly those resulting from Gustilo-Anderson type IIIB open fractures-present a formidable reconstructive challenge. Autologous bone graft, though biologically superior, is restricted in this context by limited harvest volume and donor-site morbidity. Nonvascularized structural bone allografts provide unrestricted graft volume and immediate mechanical support, but data on their performance in severely compromised post-traumatic environments remain sparse. Methods: This retrospective case series evaluated three patie

Four-corner fusion is a reliable surgical option for symptomatic scapholunate advanced collapse and scaphoid nonunion advanced collapse. There are multiple current techniques with potential complications, including nonunion, hardware failure, impingement, and cartilage disruption. Despite these potential complications, 4-corner fusion can result in good functional outcomes and patient satisfaction. We present a cartilage-sparing technique using snap-off compression pins for 4-corner fusion. Potential benefits of this technique include strong internal fixation and stable compression with implan

BACKGROUND: Unstable extra-articular proximal phalanx fractures are common injuries to the hand that are often treated by closed reduction and percutaneous pinning. Fracture-induced shortening of the proximal phalanx leads to an extensor lag at the proximal interphalangeal joint. We describe a biomechanical study in cadaver hands to compare the ability of each of three different pin configurations to resist shortening in unstable fractures.
METHODS: Seventeen fresh frozen hands were disarticulated at the proximal ends of the metacarpals. The second, third, and fourth proximal phalanges were t

Subtrochanteric femoral fractures are proximal femoral fractures which are located between the trochanter minor and an area of 3 cm below the minor trochanter on the femoral shaft. About 10-15% of all proximal femoral fractures correspond to this fracture site. Elderly or geriatric patients are generally affected and the injury is often the result of a fall in the home, while high-energy trauma is the cause in a small group of generally younger patients. Clinical evaluation of the affected extremity shows disability of axial weight-bearing and pain during compression and rotation of the hip jo

OBJECTIVE: This study aimed to demonstrate that the lengthening technique of an external fixator associated with locked intramedullary nail is an efficient method that decreases the duration of the external fixation and improves the rehabilitation period.
METHODS: From January of 2005 to May of 2014, 31 patients with mean lower limb discrepancy of 5.31 cm were treated. The etiologies of the deformity were femur fracture sequelae, infection, hip development dysplasia, polio, and congenital short femur.
RESULTS: The mean duration of external fixation was 2.47 months (external fixation index of

The incidence of forearm shaft fractures in children has increased in recent years. They are challenging to treat and they can result in several long-lasting complications. The treatment of children's fractures needs to be individualized to their needs. Nonoperative care will be satisfactory for young, preschool children and it is primarily treatment in stable fractures of children at every age. Injury mechanism must be understood to perform appropriate closed reduction. Immobilization using a long-arm cast needs to be focused against the deforming muscle forces - in particular those that rota

OBJECTIVE: Subtrochanteric femoral fractures are severe injuries. Although many treatment methods have been developed, controversy exists regarding the optimal management of these fractures. This study evaluated the clinical outcome of subtrochanteric femoral fractures fixed with long proximal femoral nail antirotation (PFNA-long).
METHODS: Between October 2006 and February 2008, 25 patients with traumatic subtrochanteric fractures of the femur were treated with PFNA-long. Closed reduction and fixation were performed in 20 cases. In the remaining 5 cases, closed reduction was difficult, so li

A case of a posttraumatic humeral shaft nonunion, after intramedullary stabilization with a Seidel nail, is presented. Severe osteoporosis, an oligotrophic nonunion, subclinical infection, and adhesive capsulitis of the glenohumeral joint were present. Due to the subclinical infection and severe osteoporosis, other major invasive therapeutic options such as intramedullary nailing or compression plating and bone grafting were not applicable. Nonoperative treatment was also not indicated secondary to the pain and disability present. External fixation with the Ilizarov hybrid fixator seemed to of

BACKGROUND: Medial column nails have been introduced into Charcot's neuroarthropathy reconstruction as superconstruct fixation with high fatigue strength and pullout resistance. Similar to intramedullary nailing throughout other long-bone fixation, injury to neurovascular and musculotendinous structures secondary to percutaneous interlocking screw fixation may exist. We sought to identify structures at risk for injury during the interlocking of a medial column nail.
METHODS: Medial column nails were inserted into ten cadaveric limbs. The proximal (talar), middle (first metatarsal), and distal

Management of tibia fractures by internal fixation, particularly intramedullary nails, has become the standard for diaphyseal fractures. However, for metaphyseal fractures or those at the metaphyseal-diaphyseal junction, choice of fixation device and technique is controversial. For distal tibia fractures, nailing and plating techniques may be used, the primary goal of each being to achieve acceptable alignment with minimal complications. Different techniques for reduction of these fractures are available and can be applied with either fixation device. Overall outcomes appear to be nearly equiv

The procedure and the external fixator for lengthening long bones was developed by G.A. Ilizarov in the late 1960's. This technique has, despite its proven abilities for leg lengthening and correction of angular deformities, some considerable disadvantages for the patients. Discomfort, infections and restricted weight bearing are some reasons for the development of a completely intramedullary device for leg lengthening. The device developed at the Laboratory of Biomechanical Engineering, University of Twente, is a telescopic intramedullary nail with a maximum diameter of 13 mm, which can be le
