CT-guided drainage of a brainstem abscess in a cat as an emergency treatment procedure.

CASE SUMMARY: A 3-year-old male neutered domestic shorthair cat was presented with a 1-week progressive and rapidly deteriorating history of … ebellar vermis herniation through the foramen magnum. CT-guided free-hand technique drainage of the brain abscess was performed and broad spectrum antibiotics were started based on sensitivity results. The cat recovered uneventfully from anaesthesia displaying marked improvement immediately after the procedure. Antibiotics were continued for 8 months; repeat im …

First case of cerebral abscess due to a novel Nocardia species in an immunocompromised patient.

We report the first case of cerebral abscess due to a novel species of Nocardia in a heart transplant patient and describe the antimicrobial susceptibility of this isolate. As our patient was intolerant to trimethoprim-sulfamethoxazole, we also discuss alternative therapeutic options in brain abscess due to Nocardia sp.

Retrospective Review of the Clinical Outcomes of Surgically Managed Patients with Intracranial Abscesses: A Single-Center Review.

OBJECTIVE: This study analyzed patient, radiologic, and clinical factors associated with operative brain abscesses and patients' functional outcomes. METHODS: A retrospective analysis was conducted of neurosurgical cases of brain abscesses from 2009 to 2019 at a Canadian center. Functional outcome was recorded as Modified Rankin Scale score and Extended Glasgow Outcome Scale score. Multivariate analysis was conducted to identify relevant prognostic factors. RESULTS: We identified 139 patients managed surgically for brain abscesses. Resection alone was performed in 64% of patients, whereas 26…

ACTINOMYCOSIS OF THE BRAIN; CASE REPORT AND REVIEW OF THE LITERATURE.

The available world literature (since Friedman's and Levy's comprehensive report in 1937) regarding actinomycosis of the central nervous system is reviewed. Only cases proved by culture were included in this analysis. A total of 17 cases was collected and an additional patient with this entity is described. The important differences between actinomycosis and nocardiosis are discussed. A definite diagnosis of actinomycosis was possible only when anaerobic cultures of cerebrospinal fluid or material obtained from a brain abscess yielded colonies of typical Actinomyces organisms. The characteristi…

Brainstem abscess complicating tetralogy of Fallot successfully treated with antibiotics alone.

Medically treated brainstem abscess in a 11 year old boy with tetralogy of Fallot is reported. There was a complete resolution of the lesion without any neurologic sequelae during parenteral antibiotic therapy with crystalline penicillin, chloramphenicol and metronidazole. The pathogenesis and management of cardiogenic brain abscesses in general and brainstem abscess in particular has been reviewed.

Stereotactic biopsy and drainage of a brainstem abscess caused by Listeria monocytogenes.

Listerial rhombencephalitis and brain abscesses are rare, but potentially life-threatening conditions. Early initiation of antibiotic therapy is crucial, but establishing the diagnosis of listerial brainstem abscess can be difficult. Stereotactic biopsy and drainage of space-occupying abscesses of the brainstem should be considered especially in cases of rapid clinical deterioration. We successfully performed stereotactic biopsy and drainage of a listerial brainstem abscess in a 42-year-old male patient who deteriorated despite antibiotic treatment, demonstrating that this approach is suitable…

Streptococcus intermedius cerebellar abscess as first presentation of sarcoidosis: a case report.

BACKGROUND: Cerebellar abscesses are rare but potentially life-threatening infections that require prompt diagnosis and management. Streptococcus intermedius is a recognized cause of brain abscesses, particularly in individuals with underlying conditions that compromise immune function. Sarcoidosis, a systemic granulomatous disease, may predispose patients to opportunistic infections due to immune dysregulation. To our knowledge, this is the first reported case of a cerebellar abscess caused by S. intermedius as the initial manifestation of sarcoidosis. CASE PRESENTATION: A 45-year-old Caucas…

Ceftazidime-resistant Burkholderia pseudomallei masquerading as isolated atypical neuromelioidosis.

A middle-aged recently diagnosed uncontrolled diabetic from Mumbai, India presented with fever and headache for 8 weeks. Two weeks prior to his symptoms, he travelled to Lonavala during the monsoons and gave history of indoor swimming in a chlorinated pool. Investigations showed isolated neurological involvement with multiple brain abscesses. Abscess cultures grew a non-lactose fermenter. Automated systems failed to identify it. After assessment of his background history and detailed microbiological analysis of the organism, specific investigations were requested which confirmed the suspected …

Aspergillosis of the central nervous system.

The authors report an unusual case involving a 38 year-old man who developed a intracranial abscess caused by Aspergillus of the parietal lobe. Cerebral aspergilloma of an initial pulmonary origin developed in a patient with "Fungus Ball" secondary to tuberculosis sequelae. The diagnosis was made through the isolation of Aspergillus from the secretion of the brain abscesses. The patient was treated with drainage of the abscesses and Amphotericin B. He presented a progressive regression of the radiological images (brain and pulmonary) over a period of 55 days. This report emphasizes the importa…

[A case of Rendu-Osler-Weber syndrome and pulmonary arteriovenous fistula].

A 65-year-old man with Rendu-Osler-Weber syndrome was admitted to the department of brain surgery at our hospital because of left hemiplegia and a right cerebral mass seen on a computerized tomogram of the brain. A brain abscess was found during surgery. Then the patient had pneumonia. He received antibiotics and recovered, but his PaO2 remained low. He was transferred to our department for evaluation of hypoxia. Thoracic computerized tomography showed a nodular lesion connected to a vascular shadow. Angiographic examination showed a pulmonary arteriovenous fistula and other vascular abnormali…

Role of diffusion-weighted imaging and in vivo proton magnetic resonance spectroscopy in the differential diagnosis of ring-enhancing intracranial cystic mass lesions.

OBJECTIVES: Proton magnetic resonance spectroscopy (PMRS) and diffusion-weighted imaging (DWI) were compared to determine which technique is … line (n = 2), and alanine (n = 3) were seen. The sensitivity of DWI and PMRS for differentiation of brain abscess from nonbrain abscess was 0.72 and 0.96, respectively, whereas the specificity was 1 for both techniques. CONCLUSION: Demonstration of restricted diffusion on DWI with reduced ADC is highly suggestive of brain abscess; however, in the absence of re …

Cerebral Nocardia Infections Following Lung Abscesses in a Kidney Transplant Recipient: A Case Report and Literature Review.

BACKGROUND: Bacterial, viral, and fungal infections are common following human kidney transplantation due to non-specific immunosuppressive therapy. Nocardiosis, though not common, is an important cause of morbidity and mortality among renal transplant recipients. CASE REPORT: We present a typical case from our region of multiple brain abscesses caused by Nocardia farcinica, along with a discussion of its diagnosis and treatment. A brain MRI of a 60-year-old Uyghur man, who had been on long-term immunosuppressive therapy, confirmed multiple brain abscesses. He was diagnosed with cerebral noca…

Cladophialophora bantiana and Candida albicans mixed infection in cerebral abscess of an HIV-negative patient.

Cladophialophora bantiana is a black fungus recorded rarely as a cause of brain abscess. We describe the first case seen in Vietnam. A 35-year-old, previously healthy male, HIV negative, came to the hospital with a clinical diagnosis of brain tumor. He underwent surgery to remove the well-delineated circumscribed lesion. Pathology, directed physical examination and mycological examination revealed an abscess due to Cladophialophora bantiana, and Candida albicans. After two weeks of treatment with Fluconazole 400 mg per day intravenously, the clinical response remained unsatisfactory. Therapy w…

The diagnostic dilemma of cerebellopontine angle lesions: re-evaluating your diagnosis.

An unusual presentation of a life-threatening cerebellopontine abscess shows the importance of re-evaluating existing diagnoses when the clinical picture changes. Despite being rare, brain abscess is a potentially fatal condition that requires early radiological and surgical intervention. It has a varied presentation, with many cases showing no localising typical features, making it hard to diagnose at presentation. A high index of suspicion should be applied to cases exhibiting rapidly changing neurology. This is the first reported case of cerebellopontine abscess with no primary infective fo…

[Commitment of the central nervous system by Eikenella corrodens. Literature review about a clinical case].

We present the case of a 22-year-old patient with no morbid history who developed a brain abscess secondary to endocarditis due to Eikenella corrodens. The diagnosis was established by detecting the microorganism by universal polymerase chain reaction in fluid from the brain collection. Trans-esophageal echocardiogram study confirmed vegetations in the mitral valve. External ventricular shunt placement was required due to emptying of abscess to brain ventricles and secondary hydrocephalus. He received 80 days of effective antibiotic treatment and his evolution was favorable with complete resol…
