Poor surgical technique in cervical plating leading to vertebral artery injury and brain stem infarction--case report.

BACKGROUND: Lateral mass plating is a safe fixating system for lower cervical fractures. Brain stem infarction after cervical lateral mass screw plating has not been reported in previous literature. We report a case of poor surgical technique leading to vertebral artery injury and brain stem infarction after cervical lateral mass plating. CASE DESCRIPTION: A 41-year-old male patient was transferred to our hospital because of hemiparesis and dysarthria immediately after lateral mass plating for fracture and dislocation of the fifth and sixth cervical vertebrae. Brain magnetic resonance imaging…

Brain stem glioblastoma with multiple large cyst formation and leptomeningeal dissemination in a 4-year-old girl.

The authors report a 4-year-old girl who developed brain stem glioblastoma. Meningeal irritation was present at onset. Magnetic resonance imaging revealed intracranial and intraspinal leptomeningeal dissemination, which progressed faster than the original tumor. Multiple large cysts developed at the interhemispheric and prepontine cisterns, resulting in progressive obstructive hydrocephalus. The patient survived only 5 months after presentation. Histology was verified by autopsy.

Neurodegeneration from mitochondrial insufficiency: nutrients, stem cells, growth factors, and prospects for brain rebuilding using integrative management.

Degenerative brain disorders (neurodegeneration) can be frustrating for both conventional and alternative practitioners. A more comprehensive, integrative approach is urgently needed. One emerging focus for intervention is brain energetics. Specifically, mitochondrial insufficiency contributes to the etiopathology of many such disorders. Electron leakages inherent to mitochondrial energetics generate reactive oxygen free radical species that may place the ultimate limit on lifespan. Exogenous toxins, such as mercury and other environmental contaminants, exacerbate mitochondrial electron leakag

[High-dose chemotherapy with autologous stem cell rescue in children with high-risk and recurrent brain tumors].

BACKGROUND: In the last few years, survival in children with central nervous system (CNS) tumors has slightly improved, especially in children with tumors such as medulloblastoma and those in which complete surgical resection is achieved. However, outcome remains poor in patients with incomplete surgical resection, neuroaxial dissemination, metastatic or recurrent tumors and in very young children. OBJECTIVES: To improve prognosis in patients with high-risk and recurrent tumors, new therapeutic strategies such as high-dose chemotherapy with autologous stem cell rescue (ASCR) have been develop

[Vertebrobasilar artery dissection with subarachnoid hemorrhage after brain stem infarct showing an improvement on angiography: case report].

A 66-year-old male presented with dysarthria and right hemiparesis. Cerebral angiography at onset showed obliteration of the basilar artery and dilatation in the left vertebral artery. The patient's clinical symptoms were exaggerated and he was finally diagnosed to as suffering locked-in syndrome. Magnetic resonance imaging showed a brain stem infarction from the pons to the left middle cerebellar peduncle. We determined that the patient suffered dissection of the left vertebral artery at the basilar artery and treated, using the conservative therapy him of strict blood pressure control. A sec…

Brain stem reflexes in patients with Wallenberg's syndrome: correlation with clinical and magnetic resonance imaging (MRI) findings.

In spite of the general clinical uniformity of Wallenberg's syndrome (WS), individual patients present with a slightly different clinical picture, and detailed studies with magnetic resonance imaging (MRI) show differences in the topography of the brain stem lesion. Neurophysiological characterization of the lesion in WS has been known for a long time, but there are no studies on the possible correlation between lesion topography and neurophysiological deficit. Assuming that afferents from the three branches of the trigeminal nerve reach different parts of the trigeminal nuclei, we examined th…

[Diagnostic intra-meatal acoustic neurinoma--the role of acoustically evoked brain stem potentials and other otoneurologic studies].

BACKGROUND: Today intracanalicular acoustic neuromas are diagnosed very early by magnetic resonance imaging (MRI). The purpose of this study was to test the capability of brainstem evoked response audiometry (BERA) for detecting these small tumors. PATIENTS: Seventeen of 91 consecutive cases with acoustic neuromas treated at our department suffered from purely intracanalicular tumors. For these 17 patients we reviewed their medical history, clinical otoneurologic examination, audiometry, auditory evoked brainstem audiometry, electronystagmography, and MRI studies. RESULTS: In 13 of 17 patien

Monitoring therapeutic efficacy of decompressive craniotomy in space occupying cerebellar infarcts using brain-stem auditory evoked potentials.

Brain-stem auditory evoked potentials (BAEPs) have been used to gauge effects of brain-stem dysfunction in humans and animal models. The purpose of this study was to evaluate the usefulness of BAEP in monitoring patients undergoing decompressive surgery of the posterior fossa for space occupying cerebellar infarcts. We report on serial BAEP recordings in 11 comatose patients with space occupying cerebellar infarcts undergoing decompressive craniotomy. BAEP studies were performed within 12 h after admission, 24 h following surgery and prior to extubation. BAEP signals were analyzed using latenc

Kainic acid lesions of the superior olivary complex: a horseradish peroxidase study of surviving brain-stem projections.

Lesions of the superior olivary complex of the adult ferret were made by pressure injection of kainic acid (5 nM/microliters in Locke's solution) through a glass micropipette inserted into the lower brain-stem. Small injections of kainic acid (1.5 microliters) produced a localized loss of nerve cells in the superior olivary complex in the vicinity of the pipette tip without apparent damage to fibers of passage in the trapezoid body. The extent of neural damage was determined by the absence of cell bodies in Nissl-stained sections. Integrity of fibers in the trapezoid body and other decussating

Anterior and posterior pituitary function in brain-stem-dead donors. A possible role for hormonal replacement therapy.

Blood samples were obtained, at the time of organ donation, from 31 consecutive brain-stem-dead (BSD) donors referred to one transplant coordinator during a 9-month period. Twenty-four cases (77%) had clinical diabetes insipidus (DI), which was poorly controlled with marked dehydration in a majority of cases (serum osmolality range 268-357; median 302 mOSM/kg). Serum triiodothyronine (T3) was subnormal in 25 (81%); all had normal or high serum reverse T3; and the serum free thyroxine (T4) index was subnormal in 9 (29%), and TSH was subnormal in 7 (23%). In no case were T4 and TSH both subnorma

Brain-stem auditory evoked potentials (BAEPs) and magnetic resonance imaging (MRI) in a case of facial myokymia.

The case of a 37-year-old woman with facial myokymia is reported. Magnetic resonance imaging (MRI), electromyography (EMG) and brain-stem auditory evoked potentials (BAEPs) supported the clinical diagnosis of suspected multiple sclerosis (MS). MRI showed a single area of abnormal signal in the pontine region. The authors discuss the possible relationship between the brain-stem lesion and the electrophysiological abnormalities demonstrated by BAEPs and the EMG of facial muscles.

[Auditory evoked brain stem responses in patients with normal pressure hydrocephalus following subarachnoid hemorrhage and elderly dementia].

There have been many reports that had discussed the effectiveness of shunt surgery in dementia patients. But it is not always reliable to predict such effectiveness before the operation. We used auditory evoked brain stem responses (ABR) to investigate the degree of brain damage in normal pressure hydrocephalus (NPH) and predict the effectiveness of shunt surgery. Fifteen patients with subarachnoid hemorrhage were divided into three groups, 5 cases without any symptoms of NPH, 6 cases with one of three symptoms of NPH and 4 cases with full blown symptoms of NPH which improved after shunt surge…

A Dural Extracellular Matrix Hydrogel with Neural Stem Cells Improves Recovery from Traumatic Brain Injury in Mice.

Current clinical treatment options for traumatic brain injury (TBI) to limit permanent tissue damage and secondary injury remain inadequate. Intraparenchymal transplantation of neural stem cells is a potentially powerful therapy for TBI, acting through replacement and/or protection of damaged cells. This approach could be augmented by entrapping the stem cells within a pro-regenerative hydrogel, designed to both retain the therapeutic cells at the transplant site and modulate the inflammatory response. This study reports a decellularized extracellular matrix hydrogel derived from porcine dura

Targeted Retention of Cationic Liposomes at the Inflamed Blood-Brain Barrier by Incorporating Mesenchymal Stem Cell Membrane.

Blood-brain barrier (BBB) disruption is found in many acute and chronic inflammatory encephalopathies. Timely restoration of BBB integrity is essential for controlling disease progression, especially in ischemic stroke. While various brain-targeting strategies have been developed, achieving precise BBB-targeting and sustained retention at the BBB to enable faster and more effective repair remains a significant challenge. In this study, a biomimetic strategy is developed to enhance inflamed BBB-targeting gene delivery using cationic hybrid nanovesicles derived from mesenchymal stem cell (MSC) m

Imaging of human stem cell-derived dopamine grafts correlates with behavioural recovery and reveals microstructural brain changes.

Cell therapy is a promising therapeutic intervention for Parkinson's disease (PD) and is currently undergoing safety and efficacy testing in clinical trials worldwide. The goals of this project were (1) to determine whether [18F]Fluorodopa or [18F]Fallypride imaging correlates robustly with functional recovery; and (2) to explore whether diffusion-weighted MR imaging (DWI) could detect graft-induced cytoarchitectural changes in the host brain. hfVM and hESC-derived dopamine precursor cells were transplanted into the 6-OHDA lesioned rat striatum. Tests of motor function and PET and MR imaging w

Immortalization and Characterization of GFAP-expressing Glial Cells from the Adult Mouse Hypothalamus, Cortex, and Brain Stem.

The generation of astrocyte cell lines from the hypothalamus is key to study glial involvement in hypothalamic physiology, including energy homeostasis. As such, we immortalized astrocytes from the hypothalamus of an adult male CD-1 mouse using SV40 T-antigen to generate the mHypoA-Ast1 cell line. A comparative approach was taken with two other murine GFAP-expressing cell lines that were also generated in this study: a mixed glial cell line from the cortex (mCortA-G1) and an oligodendrocyte cell line from the brainstem (mBstA-Olig1), as well as an established microglial cell line (IMG). mHypoA
