Diagnosing heel pain in adults.

Heel pain is a common condition in adults that may cause significant discomfort and disability. A variety of soft tissue, osseous, and systemic disorders can cause heel pain. Narrowing the differential diagnosis begins with a history and physical examination of the lower extremity to pinpoint the anatomic origin of the heel pain. The most common cause of heel pain in adults is plantar fasciitis. Patients with plantar fasciitis report increased heel pain with their first steps in the morning or when they stand up after prolonged sitting. Tenderness at the calcaneal tuberosity usually is apparen

Ultrasonography for assessment of subcutaneous nodules.

OBJECTIVE: To characterize a variety of subcutaneous lesions by their ultrasonographic (US) appearance, and establish these images as a starting point to measure changes with treatments. METHODS: Twenty-six patients with 48 subcutaneous nodular swellings of various types were imaged using a portable US machine equipped with a 10 MHz linear transducer. All patients had a known diagnosis of a rheumatic disease. We used US to examine subcutaneous lesions and the underlying cortical surface of the bone or joint. Two measurements of some tophi and rheumatoid nodules were done on different dates to

Arteriovenous fistula of the lateral superior and inferior geniculate arteries. A unique cause of a "recurrent prepatellar bursa.

We report a previously undocumented case of an arteriovenous fistula of the superior and inferior lateral geniculate arteries presenting with what was thought to be a recurrent prepatellar bursa requiring repeated drainage and excision. The diagnosis was made by arteriography. Surgical exploration and ligation of the feeding and draining vessels, via a posterior approach, alleviated the symptoms. Vascular injury should be suspected in patients presenting with recurrent postoperative hematomas or hemarthroses following surgery on the knee.

Epidemiology and natural progression of articular temporomandibular disorders.

The reported prevalence of temporomandibular disorders (TMDs) differs from study to study, probably because of methodologic errors and lack of standardized definitions of TMDs and their characteristics. Classification of TMDs should be in accordance with classification of synovial joint disorders as generally used by rheumatologists and orthopedic surgeons, in which articular disorders are distinguished from nonarticular disorders. Articular temporomandibular disorders appear to be self-limiting in a very high percentage of cases. Because of this nonprogressive nature, the outcome of nonsurgic

[Arthroscopic revision of the sub-acromial space in impingement syndrome].

Degenerative lesions of the subacromial structures can cause chronic anterior pain and disability of the shoulder. They are most frequently found in patients whose arms are repeatedly exposed to force while raised above head level, e.g. during tennis, volleyball or swimming. This is often due to impingement of the supraspinatus tendon, the subacromial bursa or the biceps tendon against the anterior edge and lower surface of the anterior third of the acromion, the coracoacromial ligament and, in some cases, the acromioclavicular joint. Open anterior acromioplasty as described by Neer is therefo

Overuse injuries in sports. A review.

Because knowledge of overuse syndromes is limited, the diagnosis and treatment of these conditions are a challenge to sports medicine physic … nflammatory processes. Overstress of bone results in stress fractures, apophysitis and periostitis. Bursitis and joint overstress problems are also discussed briefly. General guidelines for establishing the appropriate diagnosis are: the initial stages of therapy require rest, often a modification or scaled down exposure to the athlete's usual performance …

Frozen shoulder: Diagnosis and treatment of adhesive capsulitis.

Adhesive capsulitis, commonly referred to as frozen shoulder, is a frequent cause of shoulder pain and stiffness in middle-aged adults and is associated with substantial functional limitation. Although traditionally considered a self-limited condition, growing evidence suggests that many patients experience prolonged symptoms and incomplete recovery. Diagnosis is primarily clinical, based on characteristic history and examination findings demonstrating global restriction of passive glenohumeral motion, with imaging reserved for exclusion of alternative pathology. Management is predominantly no

Vitamin C as a Potential Prophylactic Measure Against Frozen Shoulder in an In Vivo Shoulder Contracture Animal Model.

BACKGROUND: Frozen shoulder is a common, painful, and movement-restricting condition. Although primary frozen shoulder is idiopathic, secondary frozen shoulder can occur after trauma or surgery. Prophylactic and therapeutic options are often unsatisfactory. Vitamin C (ascorbic acid) is a potent physiological antioxidant and likely inhibits the activation of nuclear factor κB, which plays a decisive role in inflammatory reactions. HYPOTHESIS: Because of its anti-inflammatory effects, vitamin C may be valuable in the prevention of secondary frozen shoulder. STUDY DESIGN: Controlled laboratory

Rehabilitative treatments in adhesive capsulitis: a systematic review.

INTRODUCTION: Adhesive capsulitis is a disease of unknown etiology. Conservative therapy is based on the use of multimodal techniques (instrumental physical therapy, exercise, physiokinesitherapy and anti-inflammatory drug therapy). Yet, there is no consensus on which conservative therapy treatment is best for the management of the patient with adhesive capsulitis. The aim of this study is to define the state of the art and guide specialists in choosing effective treatments for adhesive capsulitis. Instrument assisted soft tissue mobilization in adhesive capsulitis: A randomized clinical trial.

INTRODUCTION: Shoulder adhesive capsulitis is a common pathology in middle aged population, physical therapy being the mainstay treatment for it. Various conventional treatment modalities have been proven to help in this condition. Instrument Assisted Soft Tissue Mobilization (IASTM) is a considerably new technique, which is being used widely for various sports related injuries for a faster recovery. This study proposes to evaluate the effect of IASTM as an added treatment for improving pain, range of motion and functional ability in patients with adhesive capsulitis. METHOD: 30 shoulders wer

Functional status in adhesive capsulitis - yoga vs. physiotherapy: a quasi experimental study.

OBJECTIVES: Adhesive capsulitis results in a multidimensional disability which demands a polygonal approach. Although physiotherapy proves an essential for a complete and comprehensive recovery in adhesive capsulitis, evidence also suggest the role of yoga in the management, hence this study investigate their effectiveness. METHODS: This is a quasi-randomised control study. A pre-diagnosed subacute and chronic adhesive capsulitis of shoulder, between the age group of 35-60 years of both genders of total 40 participants were randomly divided into Group 'A' (yogasana) and Group 'B' (physiothera

Normative size of the osseous part of calcaneal bursa and its comparison with other calcaneal articular areas.

The retro-calcaneal bursa presents a synovial and a non-synovial osseous part of variable dimensions. Studies objectively measuring the variability of the size of this osseous bursal surface cannot be found in literature. The objective of this study was to investigate (i) the dimension variability of the bony part of the bursa and (ii) the relationship of this surface to other articulating areas of the calcaneus. A digital planimeter was used to measure the bursae (n=86) and other articular surface areas of the calcaneus and statistically compared with ANOVA and correlation estimations. The os
