Balloon osteoplasty--a new technique for reduction and stabilisation of impression fractures in the tibial plateau: a cadaver study and first clinical application.

PURPOSE: Fractures of the tibial plateau are among the most severe injuries of the knee joint and lead to advanced gonarthrosis if the reduction does not restore perfect joint congruency. Many different reduction techniques focusing on open surgical procedures have been described in the past. In this context we would like to introduce a novel technique which was first tested in a cadaver setup and has undergone its successful first clinical application. METHODS: Since kyphoplasty demonstrated effective ways of anatomical correction in spine fractures, we adapted the inflatable instruments and…

Balloon osteoplasty--a new technique for minimally invasive reduction and stabilisation of Hill-Sachs lesions of the humeral head: a cadaver study.

PURPOSE: Traumatic shoulder dislocation may be complicated by concomitant bony injury of the glenoid rim or the humeral head. In Hill-Sachs lesions, reconstruction techniques vary widely and range from open reduction to tendon transposition or humeral head derotation. These operations are extensive and have questionable outcomes. With the expertise from vertebral compression fracture reduction by kyphoplasty, we examined in a cadaver feasibility study whether reduction of the Hill-Sachs lesion via hydraulic lift might be an anatomical and minimally invasive treatment option. We postulated that…

Minimally invasive trans-mediastinal endoscopic approach to insert phrenic stimulation electrodes in the human diaphragm: a preliminary description in cadavers.

OBJECTIVE: Diaphragm pacing by phrenic nerve (PN) stimulation is currently used for patients with central respiratory paralysis to be weaned from mechanical ventilation. Electrodes are inserted either through bilateral thoracotomy or through four ports laparoscopy. The aim of this experimental work is to demonstrate the feasibility of trans-mediastinal bilateral implantation of PN electrodes using a flexible gastroscope introduced through a cervical incision in human cadavers. METHODS: Ten refrigerated and non-embalmed cadavers were used. The gastroscope was introduced through a cervical inci…

Detection of bony defects around cementless acetabular components in total hip arthroplasty: a DEXA study on 10 human cadavers.

BACKGROUND: We studied the ability of DEXA to detect bony defects around cementless acetabular components in total hip arthroplasty. The aim of our study was to provide a tool for detection and quantification of osteolytic lesions for the planning of revision cases. METHODS: We measured BMC in 10 human pelvis specimens retrieved from post mortem. We created standardized defects behind inserted acetabular components and compared the ash weights of the removed bone to the corresponding BMC measurements. RESULTS: We found a good correlation between the BMC differences measured and the correspon

The risk of optic nerve injury in retrobulbar anesthesia: a comparative study of 35 and 40 mm retrobulbar needles in 12 cadavers.

PURPOSE: This study was designed to demonstrate the increased risk of optic nerve injury by the 40 mm needle when fully inserted into the orbit. METHODS: Retrobulbar anesthesia needles 35 and 40 mm long were inserted into the orbits of 12 well-embalmed cadavers, as for typical retrobulbar anesthesia. The needle was seen directly through a fenestration of the orbital roof and by dissection of the orbital structures overlying the optic nerve. RESULTS: In all orbits the 40 mm needle reached and in seven cases significantly pushed against the optic nerve and could obviously penetrate its sheaths…

Clinical implications of anomalous muscle insertion relative to jaw movement and mandibular dysfunction: the anterior belly of the digastric muscle in a cadaver.

This case report of an anomalous insertion of the anterior belly of the digastric muscle in a cadaver considers the components involved in mandibular movement as well as the radiologic changes that could be expected in asymmetric function of the temporomandibular joints. The occlusal scheme of the cadaver was noted and, after transcranial radiographs were taken, both joints were anatomically dissected. While the left condyle-disc relationship appeared normal and properly approximated, the right joint (which was assumed to be translating in an anterior, inferior, and medial direction on each op…

Thoracic epidural insertion using extended reality hologram assistance: a randomised trial of regional anaesthesia skill performance on the soft embalmed Thiel cadaver.

BACKGROUND: Medical extended reality projects a 3D digital image onto live surface anatomy via a wearable device while allowing operators to interact with the real world. We have developed a 3D hologram of the thoracic spine from CT images of a soft embalmed Thiel cadaver and visualised spinal anatomy through a Microsoft Hololens2 headset. The primary objective of this study was to test whether thoracic epidural training performance on the cadaver was improved using extended reality compared with the landmark insertion technique. METHODS: The study comprised psychometric evaluation of anaesth…

Reality matters: in-field validation of fresh frozen cadaver-based airway training in HEMS - the BOAH Initiative: a prospective observational cohort study.

BACKGROUND: Endotracheal intubation (ETI) remains the gold standard for emergency airway management. The performance indicator is the first-pass success rate (FPSR). Providers without routine practice in elective airway management need adapted training to gain proficiency and maintain a high level of competence. Fresh frozen cadavers (FFCs) have been shown to be effective in training and can be modified to recreate difficult airway features seen in helicopter emergency service (HEMS). METHODS: In this prospective before-and-after study, the BASELINE phase consisted of recording both the perfo…

Relevance of diatom testing on closed organs of a drowned cadaver who died after receiving treatment for 10 days: A case report.

A male in his late adolescence fell into the sea and was found 50 min later with cardiopulmonary arrest. He was revived approximately 260 mi … ered false negative for confirming drowning death since diatoms can also be detected in non-drowned cadavers on dry land. This suggests that diatoms might not reach the closed organs via circulation and that the diatom test of closed organ samples might no longer be necessary to confirm drowning deaths.

Varus talar tilt combined with an internal rotation pivot stress assesses the supination instability vector in lateral ankle ligaments' injury - cadaver study.

BACKGROUND: The lack of consensus on the relevance of the varus talar tilt test (VTTT) might be due to the divergence between the insufficiency vector of lateral ankle instability and the direction of this clinical test. Our hypothesis is that the VTTT is more accurate to diagnose lateral ankle ligaments rupture when it's applied with a pre-positioning of the foot in internal rotation (IR). METHODS: We compared, in 12 cadaver ankles, the varus opening during a classic VTTT with the same test starting in an IR pivot, using a new arthrometer. RESULTS: The classic VTTT caused a 13° tilt after A…

Anatomic tunnel placement can be achieved with a modification to transtibial technique in single bundle anterior cruciate ligament reconstruction: A cadaver study.

Placing the tunnels in the anatomic positions is important for successful restoration of knee function after anterior cruciate ligament reconstruction (ACLR). It has been shown that it is difficult to place the tunnels in the anatomic position using the transtibial technique. The purpose of this study was to evaluate the effect of each step of our modified transtibial technique (mTT) on the positioning of the femoral tunnel so as to assess whether the mTT could achieve anatomic placements of the tunnels without tibial tunnel expansion. Ten fresh-frozen cadaveric knees were used. First, the tib…
