Indirect revascularisation for paediatric moyamoya disease: the EDAMS technique.

Moyamoya disease can have devastating effects on paediatric patients as a result of cerebral ischaemia. Several direct and indirect surgical methods have been devised in order to facilitate revascularisation. Debate has long ensued about which methods are most efficacious and yet safe. The authors describe their experience with a straightforward method for performing the EDAMS (encephalo-duro-arterio-myo-synangiosis) technique.

A case of moyamoya disease presenting with chorea.

CASE REPORT: We describe a 15-year-old girl with moyamoya disease whose initial manifestation was chorea-like involuntary movements. T2-weighted magnetic resonance imaging showed high signal intensity lesions in the left frontal lobe, right parieto-occipital lobes, and frontal subcortical white matter. Single-photon emission computed tomography (SPECT) showed diffuse hypoperfusion of the whole brain. Bilateral direct and indirect cerebrovascular bypass surgeries were performed. Chorea disappeared 2 days after the surgery. Follow-up SPECT demonstrated increased cerebral perfusion in the bilater

Catheter-based recanalization techniques for acute ischemic stroke.

Recent advances in endovascular interventional therapies have revolutionized the management of acute ischemic stroke. For patients who present with occluded circle of Willis vessels, timely and successful arterial recanalization is the best predictor of clinical improvement. Diagnostic neuroimaging has advanced noninvasive tools--namely, transcranial Doppler, CT angiography, and MR angiography--to screen individuals with acute neurologic syndromes rapidly for arterial occlusion, and hence to exclude from treatment those who are unlikely to benefit from or could be harmed by arterial recanaliza

Maxillary-to-petrous internal carotid artery bypass: an anatomical feasibility study.

The possibility for maxillary artery (MA) to petrous internal carotid artery (ICA) bypass was investigated. Five adult cadavers were dissected bilaterally. After zygomatic arch osteotomy, the coronoid process was sectioned at its base. An extensive infratemporal craniotomy was performed at the level of foramina ovale, rotundum and spinosum. The petrous portion of the ICA was exposed by drilling away the floor of the middle fossa, posterior to the foramen ovale and medial to the foramen spinosum. The MA was identified medial to the infratemporal crest and was followed in the pterygopalatine fos

Revascularization with split duro-encephalo-synangiosis in the pediatric moyamoya disease--surgical result and clinical outcome.

Dural arteries are potential donor arteries for cortical revascularization. In this report, a technique of indirect anastomosis using a split dura is presented. At surgery, the dura near the branches of the middle meningeal artery was split into outer and inner layers, and the split surface of the outer layer was attached to the cortical surface (split duro-encephalo-synangiosis; split DES). This procedure, combined with standard encephalo-duro-arterio-synangiosis, was applied to 25 hemispheres in 18 patients with pediatric Moyamoya disease (mean age, 6 years). Postoperative superselective ang

Revascularization of the calcarine artery in moyamoya disease: OA-cortical PCA anastomosis--case report.

A 38-year-old male presented with moyamoya disease. Occlusion of the bilateral proximal posterior cerebral arteries (PCAs) resulted in homonymous 3/4 (right half and left lower quadrant) anopsia and various mental symptoms. To prevent impending cortical blindness, revascularization to the right PCA was performed by occipital artery (OA) to calcarine artery anastomosis. His neurological state was stabilized. OA-cortical PCA (calcarine artery) anastomosis is an alternative to omentum transplantation or occipital burr hole procedures for impending cortical blindness.

Long-term clinical and neurophysiological effects of reconstructive vascular surgery for cerebral ischemia.

In a series of patients with unilateral supratentorial ischemia, clinical scores and parameters derived from computer analysis of the EEG and from measurement of the CBF were determined in the first several weeks after the stroke. Seventeen of these patients underwent a carotid-endarterectomy and 15 a STA-MCA bypass operation. Matched control patients were selected from the remaining cases. All patients, including the controls, were eligible for vascular surgery. The measurements were repeated respectively 3 months and 3 years after the first examination. Clinical improvement occurred in all g

Cortical blood flow during extracranial-intracranial bypass surgery.

Cortical blood flow was evaluated at the time of surgery with a thermal diffusion flow probe in 25 extracranial-intracranial bypass operations. The procedures were performed 14 times for carotid occlusion, 6 times for inaccessible carotid stenosis, on 3 occasions for middle cerebral occlusion, once for basilar occlusion and once for middle cerebral stenosis. Fifty-seven different cortical regions were evaluated before and after completion of the anastomosis. During temporary occlusion of the cortical vessel, no significant changes in cortical blood flow were identified. The mean flow pre-bypas

Intracranial Aneurysms After Intravenous Thrombolysis in Patient with Atrial Myxoma: A Case Study.

We administered intravenous thrombolytic therapy to a 51-year-old female patient with a 101-min stroke onset. The patient was unconscious during the manifestation of symptoms. Computed tomography angiography examination of the intracranial artery at the time of admission suggested that the left middle cerebral artery was occluded. The patient regained consciousness after the intravenous thrombolytic treatment was administered. On an urgent cerebral angiography, it was revealed that the recanalization of the left middle cerebral artery was successful. Although blood perfusion was restored, occl

Cerebral hyperperfusion syndrome after endovascular reperfusion therapy for medium vessel occlusion: A case report.

Cerebral hyperperfusion syndrome is a rare but serious complication after revascularization procedures for cerebrovascular diseases. Cerebral hyperperfusion syndrome can develop after treatment of acute ischemic stroke, including intravenous thrombolysis and endovascular treatment of large vessel occlusion. However, to the best of our knowledge, there are no previous reports describing cerebral hyperperfusion syndrome after endovascular treatment of medium vessel occlusion (eg, anterior cerebral artery A2/3 segment). We report a case of cerebral hyperperfusion syndrome after endovascular reper

Incidence and Clinical Characteristics of Moyamoya Disease in Advanced Suzuki Disease Stages.

Moyamoya disease is a progressive steno-occlusive cerebrovascular disease. Intrinsically, its shifts the brain's vascular supply from the internal carotid to the external carotid system, known as internal-to-external carotid conversion, which is categorized by Suzuki's angiographic staging system. Although Suzuki's staging system remains the global standard for assessing longitudinal angiographic progression, the clinical characteristics of patients in the advanced stages, particularly in stage 6, are not well understood. Therefore, in this study, we investigated the incidence and clinical fea

Pre-operative higher hematocrit and lower total protein levels are independent risk factors for cerebral hyperperfusion syndrome after superficial temporal artery-middle cerebral artery anastomosis with pial synangiosis in adult moyamoya disease patients-case-control study.

Superficial temporal artery (STA)-middle cerebral artery (MCA) anastomosis is a standard treatment for adult moyamoya disease (MMD) patients. Cerebral hyperperfusion (CHP) syndrome is one of the most serious complications of this procedure that can result in deleterious outcomes, but predicting CHP before revascularization surgery remains challenging. Furthermore, the hematological/serological factors associated with CHP syndrome are unknown. To investigate the correlation between pre-operative hematological/serological factors and the development of CHP syndrome after STA-MCA anastomosis with

Is There Benefit from Stenting on Cognitive Function in Intracranial Atherosclerosis?

BACKGROUND: Revascularization of stenotic cerebral arteries is hypothesized to improve cognition by increasing cerebral perfusion. AIMS: We compared cognition impairment among patients treated with percutaneous angioplasty and stenting (PTAS) and aggressive medical management (AMM) versus AMM alone in the Stenting versus Aggressive Medical Therapy for Intracranial Arterial Stenosis (SAMMPRIS) Trial. METHODS: In SAMMPRIS, 451 patients with recent transient ischemic attack or stroke attributed to 70-99% intracranial stenosis were randomized to PTAS plus AMM or AMM alone. Patients who had strok

Treatment of symptomatic middle cerebral artery stenosis with balloon-mounted stents: long-term follow-up at a single center.

OBJECTIVE: Primary stenting is a well-established treatment for coronary artery disease and has been applied to symptomatic intracranial stenosis in selected patients. This study reports a large case series of middle cerebral artery revascularization using balloon-mounted coronary stents. METHODS: The series consisted of 113 consecutive patients with symptomatic middle cerebral artery stenosis and more than 70% occlusion who underwent intracranial stenting at a single center from 2001 to 2006. Technical success, periprocedural complications, recurrent symptoms, and restenosis were retrospecti

Cerebrovascular insufficiency as the criterion for revascularization procedures in selected patients: a correlation study of xenon contrast-enhanced CT and PWI.

In order to identify patients who suffer from hemodynamic cerebral insufficiency and can benefit from cerebral revascularization procedures, xenon-CT scanning has been established to reliably measure the critical cerebrovascular reserve capacity. As a need for alternative quantification methods arises, this study aims to characterize the significance of both time-to-peak (TTP) and mean transit time (MTT) in perfusion-weighted imaging (PWI) in this particular subset of patients. Ten patients in routine preoperative work-up for cerebral revascularization were prospectively enrolled and underwent…
